Name of Work For the period Nature of work 01 Storing contractor for Islampur ICDS Project, U/Dinajpur 1(one) year from the date of acceptance of tender by DLTC, U/Dinajpur. A. GENERAL TERMS AND CONDITIONS :- Receiving & storing the dietary articles and other ICDS materials, if any for conducting ICDS activities 1) Contractor must have his/her own /hired (on lease basis) godown. It should be in pucca building, neat & clean, well-ventilated, damp proof, having dunnage facilities and floor space should be concrete. 2) Godown should be situated in the close vicinity of the Project Office i.e. within a radius of 3(three) kms from the Project Office. 3) Godown should be located on truckable roadside and there should have the facility or space for loading and unloading. 4) Measurement of the godown should be within the area of not less than 1200 sq. ft. so that the ICDS materials can be kept in countable manner. 5) For any loss during storage, the cost will be realized either from the bills of storing agent or from his security money. 6) Contractor will have to call on the office of the undersigned once in a week to obtain information about arrival of food stuff and other official works. 7) Contractor will have to maintain a register as per the direction issued by the undersigned & he will be bound to be present on any day for physical verification of the godown. 8) The godown should exclusively be used for ICDS Purpose. 9) Contractor will have to abide by necessary instruction issued by the CDPO regarding safe storage, preparation of reports, returns & challans and their timely submission to the office.he will have to deliver the food stuffs to Carrying Contractor with proper receipt as per order of the CDPO at the time of distribution. 10) For storing monthly rate of total floor space of the store is invited and the rate should be inclusive of unloading, weighing,rebagging and all other allied incidental charges. 11) Rate quoted should be specific(in figures and words) and unconditional. No over-writing or erasing in the rate schedule shall be allowed. Any correction/overwriting must be authenticated 12) Project Level Tender Committee(PLTC) is the recommending authority. Final approval will be given by the District Level Tender Committee(DLTC), Uttar Dinajpur.So, both the committees reserve the right to reject any or all tenders. 13) Regular payments will be made subject to the availability of fund. It is binding upon the contractor to continue work throughout the period of contract and irregular payment shall not be accepted as a reason or plea to discontinue the work by the contractor. Request for enhancement of rate during the contract period will not be entertained.

2 14) Any breach of contract, activities prejudicial to the interest of the project, noncompliance of govt. orders etc. shall lead to the forfeiture of earnest money/security money or both as would be deemed fit by the Authority. 15) No co-operative formed by the staff of any ICDS Project shall be allowed to participate in the tender. 16) No contrctor shall be allowed to participate both in tenders for Storing and Carrying. If any contractor is found to have participate in the tender violating this conditions, his candidature shall be liable to be cancelled, even after finalization of the tender. 17) The lowest rate may be ignored if the godown is found to be unsuitable for ICDS purposes and the location is beyond radius of 3( three) kms from the Project Office. 18) Period of contract will be for 1(one) year from the date of acceptance of tender by the District Level Tender Committee, U/Dinajpur. 19) In case, the Contractor delays in carrying out the works deliberately, the CDPO Islampur will make own arrangement to complete the works and the payment that would be made in excess of the approved rate herewith will be realised from bills/earnest money/security money. 20) In case of Co-operative certificate of exemption regarding earnest money/security money duly recommended by ARCS and an undertaking from Chief Executive Officer of the co-operative to be submitted regarding participation of the said co-operative in the tender. Annual Audit report relating to current functioning of the said co-operative must be produced. 21) Contractor should have bank Solvency of Rs /-( Four Lakh) only 22) Defaulter and blacklisted contractors should not participate in this tender process. 23) Strictly follow the column B while submitting the tender. B. SUBMISSION OF TENDER :- 1) No tender form will be issued from this office end and all tenderers must submit rates in their own letter head in the envelope B as per proforma given in ANNEXURE-I. 2) Duly filled in proforma must be accompanied by the following Technical Bid (Prequalification Documents) papers in the envelope Maked A. :- i) Curent & valid ST( VAT), income tax clearance certificate, trade licence,professional tax etc (Xerox copies with attestation). ii) Earnest Money worth Rs 30000/-(Rupees thirty thousand only) in the form of NSC (duly pledged)/bank draft/dcr only in favour of C.D.P.O. Islampur, Uttar Dinajpur. iii) Sketch map of the godown ( site plan should be vetted by SAE mentioning the land particulars on which the godown is situated ). Area should not be less than 1200 sq.ft.& height not less than 12ft.If the godown is on lease basis, the agreement copy with no-objection certificate of the godown owner. iv) In case of Co-operative Socities certificate from CEO regarding participation, exemption certificate on earnest money & security money from ARCS, Annual Audit report regarding current functioning of the Societies etc. No objection regarding participation from ARCS will have to be submitted. 3) The envelop containing tender papers as mentioned in clause B(2) must be placed in separate sealed envelope to be marked A ( Tender Papers for Storing of food stuffs & other materials for Islampur ICDS Project) ) and the Rate in Annexure-I should be placed in other sealed envelope to be marked B (Tender rate for Storing of food stuffs & other materials for Islampur ICDS Project). Both the envelopes A and B should be placed in bigger envelope duly sealed &.superscribed in capital letters with SEALED TENDER FOR STORING CONTRACTORSHIP FOR ISLAMPUR ICDS PROJECT : U/DINAJPUR along with Tender Notice No. & date and full address of the Tenderer. The Envelop marked B will be opened only when the tender papers in envelope marked A will be found valid in all respect. In the event of tender papers in A being not found valid, the tender rate shall not be considered.

3 4) Envelope containing tender should be addressed to the Sub-Divisional Officer, Islampur Sub-Division, P.O.- Islampur, Dist.- Uttar Dinajpur and the same will be received by registered post only. Last date of receiving tenders by regd. Post only will be upto 2.00 p.m. The Committee will not take any responsibility for any late delivery /submission of the tender by postal authority or in any way. C. OPENING OF TENDER :- Tender will be opened on at 1.00 P.M. at the office of the Sub- Divisional Officer, Islampur, U/Dinajpur. Date & time may be altered if any unavoidable circumstances arise. Tenderers may remain present at the time of opening. Tenderers have to produce all required documents in original wherever applicable at the time opening and as and when it will be necessary. D. SECURITY MONEY :- Successful tenderers shall have to deposit security money worth Rs 75000/-(Rupees seventy thousand ) only in the form of NSC(duly pledged) in favour of C.D.P.O. Islampur, U/Dinajpur. He will have to enter into an agreement of non-judicial stamp worth Rs 50/-(Fifty) in presence of S.D.O. Islampur. Security money will be forfeited if the contractor fails to perform the duties/execute the works as per agreement. Security money will be released after a reasonable time from the expiry of the period of the contract provided the contract has satisfactorily been fulfilled. GENERAL TERMS AND CONDITIONS :- Receiving from storing agent and finally delivering the dietary articles and other ICDS materials, if any, to the AWCs for conducting ICDS activities 1) For any loss during carrying of food stuffs & other materials,the cost will be realized either from the bills of carrying agent or from his security money. 2) Contractor will have to call on the office of the undersigned once in a week to obtain information about despatch of food stuff to AWCs as per instruction or other official works. 3) Contractor will have to maintain a register as per the direction issued by the undersigned & he will be bound to be present on any day at the office of the undersigned for carrying status report. 4) Contractor will have to abide by necessary instructions issued by the CDPO regarding preparation of reports, returns & challans and their timely submission to the office. For each centre he has to prepare 3 (three) copies of challans --- one copy to be kept with AWW, 2 nd copy to be submitted to the office alongwith the statement of GP wise delivery and 3rd copy at contractors disposal. The cost of Challans will have to be borne by the contractor himself. Office will not supply the same. 5) Contractor shall have to complete delivery of the food stuffs/materials to different Anganwadi centres from Project godown within 10 (ten) days from the date of receipt of delivery order from the office. Supply must be made in the day time during working hours of the Anganwadi centres. Unauthorised receipt of food will be treated as unaccounted. 6) For carrying, rate per quintal of net weight of food stuff is invited and the rate should be inclusive of loading, unloading, weighing and all other allied incidental charges. 7) Rate quoted should be specific(in figures and words) and unconditional. In case a tenderer will arrange a vehicle on contract basis, it should be a registered one. 17) Empty packets should be collected from centre level for handing over the same to food supplier and report to be submitted. No separate cost will be given for this matter. 18) In case of Co-operative certificate of exemption regarding earnest money/security money duly recommended by ARCS and an undertaking from Chief Executive Officer of the co-operative to be submitted regarding participation of the said co-operative in the tender. Annual Audit report relating to current functioning of the said co-operative must be produced. 19) Contractor should have Bank solvency of at least Rs /- ( Five lakh) only 20) Defaulter and blacklisted contractors should not participate in this tender process. 21) Strictly follow the column B while submitting the tender.
