1 HUNTINGTON PARK GARAGE ASSESSMENT May, 2012 APPENDIX A Closed Parking Space Report OBSERVATION REPORT DATE: 4/24/12 PRESENT AT SITE: Aaron Lobas, Daniel Imlay - URS JOB NAME: Cuyahoga County Huntington Park Garage JOB NUMBER: TIME: 8:30am WEATHER: Overcast TEMP: 50 degrees OBSERVER: Aaron Lobas Areas Observed: At the request of the Cuyahoga County of Public Works, URS was requested to review six areas closed to traffic/parking within the Huntington Park Garage due to structural concerns brought forward by Cuyahoga County employees. The observations were requested to determine what would be required to reopen the closed parking spaces. The five areas of concern are as stated in the Cuyahoga County Department of Public Works memo _HPG FINAL closed spots to BT.docx and repeated here as follows: 1. Level 2 along the north wall between wall sections E8 and E9 2. Level 3 between columns C4 and C5 3. Level 3 along west wall between columns C1 and D1 4. Level 3 between column D2 and wall section E4 5. Level 4 between columns B2 and C2 6. Level 4 between columns C22 and C23 The structural observation of each area along with recommendations to reopen the closed spaces is provided below: Area 1: Level 2 along the north wall between sections E8 and E9: The end condition of precast T-beam B-22 is distressed at its connection to the northern concrete corbel formed into the cast-in-place concrete north wall. Precast T-beam B-22 has two stems supported at the concrete corbel. Currently two parking spaces are closed beneath the end condition of precast T-beam B- 22. It would be permissible to reopen the parking spaces if shore posts are added beneath each stem of precast T-beams B-21 and B-22. The shore post should be located tight to the underside of each joist stem and
2 HUNTINGTON PARK GARAGE ASSESSMENT May, 2012 located roughly 1-6 south of the stem bearing at the concrete corbel. All shoring should continue between floors to the slab on grade at the base of Level 4. Area 2: Level 3 between columns C4 and C5: The girder/wall corbels which support the precast T-beams in this area have degraded. The girder/wall corbel is located along the north edge of the ramp along column line C, between columns C4 to C6. Since the main entrance to the Level 4 occurs along column line C between columns C4-C5, shoring of this column would eliminate the entrance to Level 4. It is recommended at this time to leave the five parking spaces in this area closed until a determination of the capacity of the girder supporting these spaces can be reviewed. Area 3: Level 3 along west wall between columns C1 and D1: The wall corbel that supports the precast girder which in turn supports precast T-beams in this area has degraded. Currently two parking spaces are closed in this area. It would be permissible to reopen the parking spaces if shore posts are added beneath the Level 3 supporting precast girder at its intersection to concrete corbel C1. While the wall in this area is badly delaminated, the wall is not a bearing wall (except at the location of the concrete corbel). The shore post should be located tight to the underside the concrete girder and located roughly 1-6 east of the concrete corbel. A photo (photo #2) of concrete corbel C1 is below:

The girder supporting the floor in this area has significant degradation. It is recommended at this time to leave the three parking spaces in this area closed until a determination of the capacity of the girder supporting these spaces can be reviewed. Area 5: Level 4 between columns B2 and C2: A concrete T-beam located on the north side of the pier cap at column intersection C2 has degraded. Currently six parking spaces are closed in this area. It would be permissible to reopen the parking spaces if shore posts are added beneath each stem of the precast T-beam depicted. The shore post should be located tight to the underside of each joist stem and located roughly 1-6 north of the stem bearing at the concrete pier cap. A photo (photo #3) of the end condition of precast T-beam is below:

Large longitudinal cracks are located along the length of the precast girder. It is recommended at this time to leave the two parking spaces in this area closed until a determination of the capacity of the girder supporting these spaces can be reviewed. End of observation and comments. Additional commentary to Appendix A: A structural analysis was performed on some precast concrete girders to verify the original design was adequate to safely support the structure. The results of the girder analysis indicate the existing girders analyzed can safely support the structure as originally designed. Due to deterioration and loss of reinforcement cross section, it is recommended that all shoring remain in place until after all immediate repairs are completed. As indicated within Appendix A, all shoring must continue to the lowest elevation of the parking garage.

8 Huntington Garage Standpipe System Study August 05, 2011 Section 1 Introduction URS Corporation has prepared a study reviewing the condition of the existing standpipe system in the Huntington Parking Garage. The existing parking garage, located at 1370 Ontario Street, Cleveland, Ohio, is an underground parking garage that serves Cuyahoga County Justice Center Complex. The study was conducted at the request of Cuyahoga County Department of Public Works. All of the information reported in this study is based on a preliminary site visit conducted on July 8, 2011, information received from Cuyahoga County Department of Public Works, and fire protection design drawings of the parking garage dated May 4, 1974, prepared by Prindle, Patrick and Partners Architects, Engineers, Planners. All observations and recommendations were based on a visual inspection and an evaluation of a hydrostatic test 1 report. URS assumed the current occupancy classification (Group S-2) remains the same as the original design. This report is a limited review of the codes & standards applicable to replacement of a standpipe system only. This report excludes a comprehensive code analysis for the parking garage. 1 A test of a closed piping system and its attached appurtenances consist of subjecting the piping to an increased internal pressure for a specified period of duration to verify system integrity and leak rates. (NFPA ) 1
9 Huntington Garage Standpipe System Study August 05, 2011 Section 2 Existing Parking Garage Information The Huntington Parking Garage, located at 1370 Ontario Street in Cleveland, Ohio, was constructed in 1976 as a four-story underground parking garage to serve the Cuyahoga Justice Center Complex. The lowest story of the garage is more than 30 feet below grade from the highest level of fire department vehicle access. The parking garage consists of concrete columns, beams, slabs and concrete block walls. There are seven enclosed exit stairways inside the garage. The parking garage is unheated. The parking garage is protected throughout with an automatic dry sprinkler system 2 and a manual dry standpipe system 3. Each exit stairway is equipped with a vertical standpipe that has a 2 1 / 2 hose valve connection on every main landing. The seven vertical standpipes are interconnected by a horizontal pipe loop on the second level (one level below the street level). Isolation valves are located at the second level of each vertical standpipe. The existing manual dry standpipe system has two separate free-standing three-way fire department connections on the west and south sides of the building. Fire department connections are tied into the horizontal loop through underground piping. A sketch indicating locations of fire department connections can be found in Appendix C. While the sprinkler system valves are electronically supervised, the dry manual standpipe system valves are equipped with chains that are intended to lock each standpipe isolation valve in the open position. 2 A sprinkler system employing automatic sprinklers that are attached to a piping system containing air or nitrogen under pressure, the release of which permits the water pressure to open a valve known as a dry pipe valve, and water then flows into the piping system and the opened sprinklers. (NFPA ) 3 A standpipe system that has no permanently attached water supply that relies exclusively on the fire department connection to supply the system demand. (NFPA ) 2
10 Huntington Garage Standpipe System Study August 05, 2011 Section 3 Applicable Codes and Standards Cleveland Fire Department is the Authority Having Jurisdiction (AHJ). The jurisdiction currently adopts the following building and fire codes for new and replacement standpipe systems: Ohio Building Code 2007 (OBC) Ohio Fire Code 2007 (OFC) NFPA 13 Standard for the Installation of Sprinkler Systems 2007 Edition (referenced by OBC and OFC) NFPA 14 - Standard for the Installation of Standpipe and Hose Systems 2007 Edition (referenced by OBC and OFC) These codes were used as the basis of the evaluation. 3.1 Ohio Fire Code Installation Fire protection systems shall be maintained in accordance with the original installation standards for that system. Required fire protection systems shall be extended altered, or augmented as necessary to maintain and continue protection whenever building is altered, remodeled, or added to Existing buildings Existing structures with occupied floors located more than 50 feet above or below the lowest level of fire department access shall be equipped with standpipes installed in accordance with paragraph (E)(905) of this rule. The fire code official is authorized to approve the installation of manual standpipe systems to achieve compliance with this paragraph where the responding fire department is capable of providing the required hose flow at the highest standpipe outlet. 3.2 Ohio Building Code Repairs to other systems, components, and materials Repairs to existing systems and materials or buildings components not otherwise provided for in Section 3403 shall not be required to meet the provisions for new construction, provided such work is done in accordance with the conditions of the existing system is not less safe than when originally installed and is approved. 3
11 Huntington Garage Standpipe System Study August 05, 2011 Section 4 Observations and Testing 4.1 Observation: In multiple locations of the horizontal standpipe loop, corroded steel pipe with significant wall loss was observed. See picture # Observation: In multiple locations of the horizontal standpipe loop, completely dissolved pipe hangers were observed. See picture # Observation: First floor horizontal standpipe loop pipe hangers were not spaced per NFPA 13 requirement. See picture # Observation: The free-standing Fire Department Connection on the south side of the building was missing signage. See picture # Testing: A standpipe hydrostatic test was conducted on May 21, 2011 by Cuyahoga County pipe fitters. The test failed to meet NFPA 25 performance criteria for a successful hydrostatic test. Mike Gallagher, Fire and Safety Inspector of Cuyahoga County, witnessed the test and stated the following result in his test report: As part of the testing, the pipe fitters attempted to charge the system to city water pressure (about 70 psi), but the system failed, causing a pipe to burst and a half dozen leaks before the system even reached half capacity. It is important to know that in order to pass a five-year standpipe system test, the system must hold 200 psi of water pressure for two hours. Consequently, it is clear from this testing failure that the pipes in the Huntington facility are deteriorated beyond a simple repair. Photographs of the existing standpipe system can be found in Appendix A. 4
12 Huntington Garage Standpipe System Study August 05, 2011 Section 5 URS Recommendations If the observed conditions in Section 4 are not addressed, the existing standpipe system may experience pipe collapse if water is pumped into the system by a fire pumper truck. The pipe collapse could potentially cause the parking garage to flood. URS recommends that the existing manual dry standpipe be repaired. The repair would entail complete replacement of the horizontal standpipe piping and the underground pipes between fire department connections and horizontal loop. Excavation and backfill are needed to replace the underground piping. Vertical standpipes appear to be in good condition (by visual inspection only), but can only be reused if the vertical standpipes can pass a hydrostatic test, which should be performed during the design phase to make this determination. In addition, corrosion-resistant signage for the fire department connection should be furnished and installed. URS also recommends that the isolation valves be electronically supervised. Electronic valve supervision is a requirement of current code for new systems and allows monitoring of the valve positions at the fire alarm panel and supervising station. While repair of an existing system does not require compliance with current code, installation of tamper switches (electronic valve supervision) is recommended. URS recommends review of the proposed design with Cleveland Fire Department prior to preparation of construction documents for repair or replacement. The estimated design & construction cost to undertake this repair to the standpipe system is $477,856. Additional detail for the cost breakdown is provided in Appendix B. 5
13 Huntington Garage Standpipe System Study August 05, 2011 Section 6 Design Criteria 6.1 Class I standpipe systems shall be wet systems except where piping is subject to freezing. (NFPA ) 6.2 Location of hose connection: 2 1 / 2- inch hose connections shall be provided at each intermediate landing between floor levels in every required exit stairway. (NFPA ) 6.3 Minimum size of pipes: Class I standpipes shall be at least four inches in size. (NFPA ) 6.4 Minimum flow rate: Because the parking garage is fully sprinklered and has seven hose connections on each floor, the minimum standpipe flow rate shall not exceed 1000 gpm. (NFPA & ) 6.5 Minimum design pressure: hydraulically designed standpipe systems shall be designed to provide the water flow rate of 1000 gpm at a minimum residual pressure of 100 psi at the outlet of the hydraulically most remote inch hose connection. (NFPA ) 6.5 NFPA 14 requires 750 gpm at the hydraulically most remote hose connections. (NFPA ) If the existing vertical standpipe cannot achieve such water flow, a 3 X 2 1 / 2 inch test connection at the most remote location shall be required. 6
