In order for your proposal to be considered responsive for this RFP this amendment, in addition to your proposal, must be received in the DNR Procurement Office prior to the Deadline for Receipt of Proposals (RFP Closing Date and Time). The following changes/additions are required: 1. REPLACE the Cost Proposal shown in the RFP with the REVISED Cost Proposal attached to this amendment. Offerors must submit the REVISED Cost Proposal attached to this amendment within their Cost Proposal package to be considered responsive. There are no further changes to this RFP at this time. 2. This amendment is a Mandatory Return amendment. In order for your proposal to be considered responsive for this RFP this amendment, in addition to your proposal, must be received in the DNR Procurement Office prior to the Deadline for Receipt of Proposals (RFP Closing Date and Time). 2 RFP REVISED Cost Proposal This Cost Proposal has been revised by Amendment 2. Cost proposals must be submitted on this REVISED Cost Proposal form. The cost proposal forms must be in a sealed envelope separate from the technical proposal(s). Costs offered must include all costs associated with providing the service, to include but not limited to, overhead, profit, labor, supplies, materials, equipment, etc. The costs offered here will be used only for award and evaluation purposes. The costs offered must reflect the costs provided on fee schedules, calculation of fees worksheets, and other cost related information within the offered proposal. The State does not guarantee a minimum or maximum quantity of services to be provided or dollar amount to be spent or earned under any contract resulting from this RFP. Offerors are required to provide a standard fee schedule for the services required within this RFP. Offerors may, at their discretion, attach a supplemental page explaining the calculation of fees if the space provided isn t adequate. The Total Contract Cost shown on line 4 of this Cost Proposal is the cost that will be evaluated by the State. This cost must equal the Total Fees/Costs of lines 1 through Construction Management: Provide Construction Management fee calculations and total cost for the following projects. Construction management costs are contracted on a fixed fee basis there is no reimbursement for expenses such as travel or overhead. All costs should be included in the Total Construction Management Fee figure. Leasing: Provide leasing fees and calculations for the following new lease and lease renewal, fees should be based on a percentage of the lease value.

Provide methodology for fee calculations for tenants that do not pay rent or pay a reduced rent.
