1 BID ADDENDUM FAILURE TO RETURN THIS BID ADDENDUM IN ACCORDANCE WITH INSTRUCTIONS MAY SUBJECT YOUR BID TO REJECTION Bid Number: 40-RQ Bid Opening Date/Time: August 28, :00PM Description: High School Youth Risk Behavior Survey EST Contact: Tymica Dunn Addendum Number: 1 Addendum Date: August 23, 2018 INSTRUCTIONS: 1. Return one properly executed copy of this addendum with bid response or prior to the Bid Opening Date/Time listed above. 2. The following questions are hereby answered: Question: Is NCDPI, in conjunction with the CDC, seeking vendor support in survey design? Answer: No. The design of the web-based surveys must match as closely as possible the CDC-approved paper version of the surveys. Question: The IFB states: Upon completion of the surveys, vendor must extract the web survey data and prepare the files for submission to the CDC. Is NCDPI also interested in accessing survey data and reports for its own use through the web portal? If so, what expectations does NCDPI have regarding survey reporting and data analysis? For example, who will view the reports? How, exactly, will the reports be used by NCDPI once the survey has closed? Answer: No. The data will be sent to the CDC for reports to be generated through their established process. Question: What is the anticipated budget for this project? Answer: N/A Question: The IFB states: Vendor response to this IFB shall include each of the following items and these items should be arranged in the following order: Within this structure, where, precisely, should the vendor provide a proposed scope of services, etc., demonstrating its ability to fulfill the IFB s requirements? Answer: You may provide that information on a separate piece of paper. This IFB specifically states what is needed in the Scope of work.
2 Question: When does the agency plan on announcing its final decision on awarding the contract? Answer: An award will be made and posted to the IPS website within days after the bid opening. Question: What is the anticipated term of this Scope of Work? Answer: Please refer to page 11 of 30 under Contract Term for this information. Question: What is the anticipated go-live date for this survey tool? Answer: Data collection must begin no later than January 17, Question: What is timeline for obtaining CDC-approval for administering web-based surveys? Answer: The questionnaires must be configured in the web-based administration system and test data files must be created and submitted for review by October 15, Feedback will be provided by November 5, 2018 and final revisions are due by November 19, The deadline for notification from CDC for approval of the web-based administration is December 3, Question: By what date must the web-based survey administration tool be ready for presentation for CDCapproval? Answer: October 15, 2018 Question: What is the consequence if CDC-approval is not obtained according to the aforementioned timeline? Answer: If CDC approval is not obtained for the web-based administration of the surveys, the contract will not be awarded. Question: Approximately how many teacher survey packets will the vendor be required to compile and ship? Approximately 3,000 teacher survey packets will need to be compiled and shipped. Answer: Approximately 3,000 teacher survey packets will need to be compiled and shipped. Question: Will NCDPI be responsible for specifying to the vendor to which schools and teachers survey packets must be shipped? Answer: Yes Question: Will paper-pencil survey administration by the vendor be required as a part of this project? If so, will the vendor be responsible for creating, compiling and shipping these materials to school sites? Will the vendor be responsible for collecting paper-pencil survey results? Will the vendor be responsible for analyzing paper-pencil survey results?
3 Answer: No, backup paper and pencil survey administrations and collection of those will be overseen by NCDPI. No, the vendor will not be responsible for analyzing paper and pencil survey results. Question: What is the anticipated window (date-wise) for survey administration? Answer: The anticipated window for survey administration is January 17, 2019 to June 14, Question: What, if any, additional State-specified requirements/deliverables will need to be met prior to golive? This is in reference to any required security scans, penetration tests, NCDPI approvals, etc. Answer: None. Question: What are the support responsibilities that will be required of the vendor? Answer: The vendor must provide a line of direct contact throughout the process to provide technical support to the state YRBS coordinator in addition to providing weekly status reports of survey administrations once the web-based surveys begin no later than January 17, Question: Should the Unit Cost and the Extended Cost be the same amount? Answers: Yes. Question: Does the State expect any additional information aside from the Vendor submitting Attachment A in the pricing proposal? If so, what additional information would the State like to evaluate? Answer: Question: The State expects 70,000 North Carolina students to participate in the survey. Can the State provide a list of the High Schools and Middle School that have participated in the most recent administration of the YRBS? Answer: The state needs to know that the vendor has experience collecting student level data but I think everything else should be covered in the scope of work provided. Question: The State expects 70,000 North Carolina students to participate in the survey. Can the State provide a list of the High Schools and Middle School that have participated in the most recent administration of the YRBS? Answer: The state will not provide a list of schools selected in The schools selected for the YRBS change during each administration cycle. In 2017, 177 high schools and 195 middle schools participated in YRBS. They are representative of the entire state.
4 Question: Who is the incumbent? Answer: There is not a current incumbent. Question: What was the prior contract value? Answer: N/A Question: How many schools will participate in the North Carolina High School Youth Risk Behavior Survey? Answer: It is difficult to say for certain, but I would expect to have similar numbers as we did in 2017, so about 177 high schools and 195 middle schools are expected to participate in the YRBS in Question: Will the State consider striking language in the section below? b) Commercial General Liability - General Liability Coverage on a Comprehensive Broad Form on an occurrence basis in the minimum amount of $1,000, Combined Single Limit. Defense cost shall be in excess of the limit of liability. Answer: Agency will review Terms and Conditions with awarded vendor. Question: Will the State consider the tracked changes below? GENERAL INDEMNITY: The Vendor shall hold and save the State, its officers, agents, and employees, harmless from liability of any kind, including all third party claims and losses accruing or resulting to any other person, firm, or corporation furnishing or supplying work, Services, materials, or supplies in connection with the performance of The Contract, and from any and all third party claims and losses accruing or resulting to any person, firm, or corporation that may be injured or damaged by the Vendor in the performance of The Contract and that are attributable to the negligence or intentionally tortious acts of the Vendor provided that the Vendor is notified in writing within 30 days that the State has knowledge of such claims. The Vendor represents and warrants that it shall make no claim of any kind or nature against the State s agents who are involved in the delivery or processing of Vendor goods or Services to the State. The representation and warranty in the preceding sentence shall survive the termination or expiration of The Contract. The total aggregate liability of Vendor for claims under or in connection with this Contract, regardless of the form of the action or the theory of recovery, shall be limited to the Contract Price.
