3 EXECUTIVE SUMMARY From April 5 through April 9, 204, IHI Environmental conducted a pre-demolition inspection of the Millcreek Apartments, located at 395 East 3045 South, South Salt Lake, Utah. Mr. Kyle Kershaw, Director of Finance with the City of South Salt Lake, requested this inspection to identify asbestos-containing materials (ACM) and Universal, Hazardous, and Toxic Wastes materials that need to be addressed before the building is demolished. The following ACM and Universal, Hazardous, and Toxic Wastes materials were identified during this inspection and assessment. ACM Wall system (38,86 square feet) Sink undercoating ( units) Light fixture wire insulation (4 units) Vinyl floor sheeting (250 square feet) Exterior stucco (800 square feet) Universal, Hazardous, and Toxic Wastes Mercury-vapor fluorescent light tubes (24-2-foot, 6 4-foot) Ballasts containing PCBs (32) Mercury-vapor fluorescent light bulbs (7 bulbs) Containers of liquid waste (20) Mercury-containing thermostat (3 units) Refrigeration units containing CFCs (2 A/C units, 2 - refrigerators) Table in this Asbestos Inspection and Assessment Report contains detailed information about the ACM, and Section 2 (Findings) in the Universal, Hazardous, and Toxic Waste Inspection Report contains detailed information about the Universal, Hazardous, and Toxic Wastes. 4 CONCLUSIONS ACM IHI recommends that a Utah-certified asbestos abatement contractor remove and properly dispose of all the ACM in this building prior to demolition. Universal, Hazardous, and Toxic Wastes Protocols for identification and disposal of hazardous materials have been developed by the Salt Lake County Health Department (SLCHD). These protocols require building owners to identify and remove all Universal, Hazardous, and/or Toxic Wastes from buildings before they are demolished. Disposition of these materials must follow EPA Guidance outlined in 40 CFR 273 (Standards for Universal Waste Management) and 40 CFR 26 and 262 (Resource Conservation and Recovery Act), and Department of Transportation regulations outlined at 49 CFR 73 (Shippers - General Requirements for Shipments and Packaging). As such, IHI recommends that the materials identified during this inspection be removed and disposed of by properly trained and licensed contractors. COST ESTIMATES IHI s cost estimates to remove the ACM and remaining hazardous materials outlined above are: ACM $74,055 Hazardous Materials: $,842 These estimates do not include the costs for asbestos abatement design and management consulting services The report that follows this Executive Summary should be read in its entirety because it includes important information, such as material descriptions and locations, regulatory requirements, and building specific recommended response actions. 5 .0 INTRODUCTION From April 5 through April 9, 204, IHI Environmental conducted a pre-demolition inspection of the Millcreek Apartments, located at 395 East 3045 South, South Salt Lake, Utah. Bulk samples of suspect asbestos-containing material were collected by Mr. John Larson (State of Utah, Asbestos Inspector Certification Number ASB-0894).. Facility Description The building is a three-story, twelve-unit apartment building. Building size is approximately 2,000 square feet. Construction date is unknown. The construction is wood frame with wood siding and stucco on exterior walls. Interior finishes are gypsum board walls and ceilings with random locations of texturing. Flooring finishes include concrete, carpeting, vinyl floor tile, vinyl floor sheeting, and ceramic tile. The roof area has tar and felt build-up on a wood substrate. The facility s heating system consists of individual gas-fired heating units and wall-mounted refrigerated cooling units..2 Scope of Work The objective of this inspection was to locate, identify, quantify, and document the accessible suspect ACM, universal, hazardous and toxic wastes, and to provide a cost estimate for proper removal. The inspection was limited to accessible surfaces, structures, and mechanical systems with minor destructive sampling of wall cavities and floors. ACM is any material containing more than one percent asbestos, as determined by polarized light microscopy (PLM). 2.0 METHODOLOGY Suspected ACM was identified and characterized in terms of homogeneous areas. A homogeneous area is defined as a single material, uniform in texture and appearance, installed at one time, and unlikely to consist of more than one type or formulation of material. All suspect ACM was touched to determine its friability. Friable ACM, when dry, can be crumbled, pulverized, or reduced to powder by hand pressure. 6 The inspector photographed each homogeneous area sampled. Photographs are included in Appendix B of this report. 2. Bulk Sample Collection Bulk samples were collected from accessible homogeneous areas of suspected ACM for subsequent laboratory analysis to determine actual asbestos content. Sampling was generally conducted in a manner that minimized damage and unsightly marks in the building and did not create a health hazard for building occupants or inspectors. The number of samples collected from each homogeneous area generally followed the Environmental Protection Agency (EPA) Asbestos Hazard Emergency Response Act (AHERA) regulations (see 40 CFR part 763, subpart E), and Utah Division of Air Quality (DAQ) Rules, R (Asbestos), which general follow the AHERA guidelines. Although AHERA regulations do not apply to this property, the AHERA regulations provide the most comprehensive and acceptable guidance for conducting field inspections for ACM. The AHERA regulations require the inspector to collect between three and seven samples of surfacing materials, depending on the size of the homogeneous area. Likewise, inspectors collect enough samples of thermal system insulation and miscellaneous materials to confidently characterize the material s composition. 2.2 Bulk Sample Analysis All bulk samples were analyzed by Dixon Information in Salt Lake City, Utah, using PLM in accordance with the EPA Method for the Determination of Asbestos in Bulk Building Materials (EPA-600/R-93-6). Dixon is accredited by the National Institute of Standards and Technology (NIST) under the National Voluntary Laboratory Accreditation Program (NVLAP). 2.3 Bulk Sample Results Table identifies all material sample numbers and locations, the homogeneous area number and description, and the quantity of the material. 7 3.0 CONCLUSIONS AND DISCUSSION The materials identified in Table 2 contain asbestos. As such, disturbance of these materials in Utah is regulated by EPA, Utah DAQ, and/or OSHA and must be removed by a Utahcertified asbestos abatement contractor prior to the start of any renovation or demolition activities that will disturb them. Table 2 includes the recommended response action. 3. Regulatory Guidance The Environmental Protection Agency, Occupational Safety and Health Administration, and the Utah Division of Air Quality regulations define ACM as materials containing more than one percent of asbestos, as determined by PLM. All three agencies regulate ACM in Utah, but only if the material contains more than percent asbestos. If a material contains less than percent, the EPA and Utah DAQ do not regulate removal or disposal. However, activities involving materials containing less than percent asbestos are regulated by OSHA. The OSHA construction regulation pertaining to asbestos (29 CFR 926.0) indicates that any asbestos work including work with materials containing between zero and percent asbestos that has the potential of producing airborne concentrations of asbestos that exceed the permissible exposure limit (PEL) must be performed applying specific precautions outlined in the regulation. 3.2 Inaccessible Areas/Materials Suspect materials that were hidden or inaccessible may not have been characterized by this inspection. Therefore, any material not identified in this report as having been tested should be treated as suspect ACM until it has been sampled by a Utah-certified inspector and analyzed by an accredited laboratory applying EPA methods. 4.0 PROJECT LIMITATIONS This project was performed using, as a minimum, practices consistent with standards acceptable within the industry at this time, and a level of diligence typically exercised by industrial hygiene consultants performing similar services. The procedures used in this investigation attempt to establish a balance between the competing goals of limiting investigative and reporting costs and time, and reducing the uncertainty about unknown conditions. 8 derived from the scope, costs, time, and other limitations, the conclusions should not be construed as a guarantee that all hazardous materials have been identified and fully evaluated. IHI s professional opinions are based in part on the interpretation of data from discrete sampling locations that may not represent conditions at non-sampled locations. IHI assumes no responsibility for omissions or errors resulting from inaccurate information, or data, provided by sources outside of IHI or from omissions or errors in public records. Furthermore, it is emphasized that the final decision on how much risk to accept always remains with the client since IHI is not in a position to fully understand all of the client s needs. Clients with a greater aversion to risk may want to take additional actions while others, with less aversion to risk, may want to take no further action. 63 .0 INTRODUCTION From April 5 through 9, 204, IHI Environmental conducted a pre-demolition inspection of the Millcreek Apartments, located at 395 East 3045 South, South Salt Lake, Utah. Mr. Kyle Kershaw, Director of Finance with the City of South Salt Lake, requested this inspection so that these materials could be identified and removed before demolition of this building. The following materials were included in the scope of this inspection: batteries pesticides mercury thermostats and lamps PCB light ballasts and oil-containing transformers identified and/or potential hazardous wastes CFC-containing air conditioning units or refrigerators other containerized toxic or special wastes in the building The building is a three-story, twelve-unit apartment building. Building size is approximately 2,000 square feet, and the construction date is unknown. 2.0 METHODS IHI conducted a visual inspection of all accessible areas throughout the building, but did not conduct any destructive inspection to characterize any material. 3.0 FINDINGS The following materials were identified in the building: Material Location* Quantity Mercury-vapor fluorescent light tubes Throughout the building 24 (2 ) 6 (4 ) Est. Removal Cost Apartments, 4, and 8 3 Units $90 Throughout Random locations throughout 2 Refrigerators 2 A/C Units $, Units $40 Electronic waste Apartments 6 and 0 2 Units $20 Automotive tires Storage closets 3 Units $ CONCLUSIONS AND DISCUSSION The Salt Lake Valley Health Department requires that building owners follow EPA Guidance outlined in 40 CFR 273 (Standards for Universal Waste Management) and 40 CFR 26 and 262 (Resource Conservation and Recovery Act), and Department of Transportation regulations outlined at 49 CFR 73 (Shippers - General Requirements for Shipments and Packaging) for the removal, transport and disposal of these wastes. These protocols require building owners to identify and remove all universal, hazardous, and/or toxic wastes from buildings before they are demolished. As such, IHI recommends that all identified materials be removed and disposed of by properly trained and licensed contractors. 5.0 COST ESTIMATE The estimated cost for the removal, packaging, transportation, and proper waste disposal of the remaining materials is $,842. 65 6.0 PROJECT LIMITATIONS This Project was performed using, as a minimum, practices consistent with standards acceptable within the industry at this time, and a level of diligence typically exercised by EH&S consultants performing similar services. The procedures used attempt to establish a balance between the competing goals of limiting investigative and reporting costs and time, and reducing the uncertainty about unknown conditions. Therefore, because the findings of this report were derived from the scope, costs, time and other limitations, the conclusions should not be construed as a guarantee that all universal, toxic and/or hazardous wastes have been identified and fully evaluated. Furthermore, IHI assumes no responsibility for omissions or errors resulting from inaccurate information, or data, provided by sources outside of IHI or from omissions or errors in public records. It is emphasized that the final decision on how much risk to accept always remains with the client since IHI is not in a position to fully understand all of the client's needs.
