26 REGULAR MEETING TUESDAY, JULY 3, 2018 The Board of County Commissioners of Medina County, Ohio, met in regular session on this date with the following members present: Adam Friedrick Patricia G. Geissman William F. Hutson Mr. Friedrick offered the following resolution and moved the adoption of same, which was duly seconded by Mrs. Geissman. RESOLUTION NO ACCEPTING AND AWARDING THE BID FOR THE REPLACEMENT OF CULVERT NO. 24 ON ABBEYVILLE ROAD (C.H. 47) IN LIVERPOOL TOWNSHIP, MEDINA COUNTY FOR THE MEDINA COUNTY ENGINEER WHEREAS, bids for the replacement of Culvert No. 24 on Abbeyville Road (C.H. 47)in Liverpool Township, for the Medina County Engineer, were received and publicly read on Tuesday, June 19, 2018, and referred to the Medina County Engineer for review; and WHEREAS, the County Engineer has recommended the bid for the replacement of Culvert No. 24 be awarded to N & N Construction Company, Denman Road, Wakeman, Ohio NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Medina County, Ohio, that the bid of N & N Construction Company is hereby accepted. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Medina County, Ohio, that the bid for the replacement of Culvert No. 24 for Medina County Engineer be awarded to N & N Construction Company, for the price of $263,503.25, as set forth in Exhibit A attached hereto. RESOLUTION NO ACCEPTING AND AWARDING THE BID FOR THE REPLACEMENT OF CULVERT NO. 32 ON PAWNEE ROAD (C.H. 28) IN CHATHAM/SPENCER TOWNSHIP, MEDINA COUNTY FOR THE MEDINA COUNTY ENGINEER WHEREAS, bids for the replacement of Culvert No. 32 on Pawnee Road (C.H. 28)in Chatham/Spencer Township, for the Medina County Engineer, were received and publicly read on Tuesday, June 12, 2018, and referred to the Medina County Engineer for review; and WHEREAS, the County Engineer has recommended the bid for the replacement of Culvert No. 32 be awarded to HSH Construction & Excavating Inc., P.O. Box 150, Wadsworth, Ohio NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Medina County, Ohio, that the bid of HSH Construction & Excavating Inc. is hereby accepted. RESOLUTION NO AUTHORIZING THE PURCHASE OF 6,500 GALLONS OF REGULAR UNLEADED GASOLINE FOR THE ENGINEERING CENTER WHEREAS, the Engineering Center is in need of purchasing 6,500 gallons of regular unleaded gasoline, and WHEREAS, the following price quotations for said purchase were received by phone on July 2, 2018: Regular Unleaded Great Lakes Petroleum, Cleveland, OH Cuyahoga Landmark, Strongsville, OH Petroleum Traders, Fort Wayne, IN No Bid Ports Petroleum, Wooster, OH RKA Petroleum, Romulus, MI Mansfield Oil, Gainesville, GA No Bid WHEREAS, Great Lakes Petroleum has been determined to be the lowest bid price of per gallon for regular unleaded gasoline, plus any applicable taxes. NOW, THEREFORE, BE IT RESOLVED by the Medina County Board of Commissioners that the above referenced purchase of gasoline from Great Lakes Petroleum be approved. RESOLUTION NO SALES TAX DISTRIBUTION TO THE VARIOUS DISTRICTS LOCATED IN MEDINA COUNTY WHEREAS, the electors of Medina County have approved a one-half of one percent sales tax to provide additional revenue to the county school districts for permanent improvements, and WHEREAS, The Medina County Commissioners have been given the responsibility to disburse this money based on the percentages adopted by the Community Improvement Board, and WHEREAS, it has now been determined that the second quarter 2018 distribution should be made to the various school districts in the following amounts: Black River Local School District $ 70, Brunswick City School District 756, Buckeye Local School District 237, Cloverleaf Local School District 254, Highland Local School District 335, Medina Joint Vocational School 109, Medina Board of DD 3, Medina City School District 722, Norwayne Local School District 6, Rittman EVSD 2, Wadsworth City School District 507, Total $3,005, NOW, THEREFORE, BE IT RESOLVED by this Board of County Commissioners that the amounts noted above be distributed to the school districts located within Medina County. RESOLUTION NO APPROVAL OF AN AGREEMENT FOR A SERVICE COORDINATOR BETWEEN MEDINA COUNTY FAMILY FIRST COUNCIL EARLY INTERVENTION PROGRAM AND MEDINA COUNTY HEALTH DEPARTMENT WHEREAS, an agreement from the Health Department to provide health and development services for Medina County Family First Council Early Intervention Program has been presented to the Board of County Commissioners for approval, and WHEREAS, the term of this agreement will be July 1, 2018 through June 30, 2019, at an hourly rate of $47.71 for a health and development specialist; not to exceed $73,950.50, plus $2, for mileage and training expenses, and WHEREAS, the Board of Commissioners wishes to have these services provided. NOW, THEREFORE, BE IT RESOLVED by this Board of County Commissioners that the health and development services agreement with the Medina County Health Department is hereby approved. RESOLUTION NO APPROVAL OF AN AGREEMENT FOR A SERVICE COORDINATOR BETWEEN MEDINA COUNTY FAMILY FIRST COUNCIL EARLY INTERVENTION AND MEDINA COUNTY BOARD OF DEVELOPMENTAL DISABILITIES WHEREAS, an agreement from Medina County Board of Developmental Disabilities to provide a service coordinator for Medina County Family First Council Early Intervention program has been presented to the Board of County Commissioners for approval, and WHEREAS, the term of this agreement will be July 1, 2018 through June 30, 2019, for service coordinator services, not to exceed $44,430.88; and cell phone related expenses, not to exceed $2,027.12; for a total agreement amount of $46,458.00, and WHEREAS, the Board of Commissioners wishes to have these services provided. RESOLUTION NO AUTHORIZING THE EXECUTION OF AN AGREEMENT WITH JANE BARNETT FOR INDEPENDENT CONTRACTOR SERVICES FOR THE FAMILY FIRST COUNCIL WHEREAS, the Medina County Board of Commissioners did previously adopt Resolution No which established a Family First Council pursuant to Ohio Revised Code Section , and WHEREAS, it was subsequently determined that the Council was in need of obtaining the services of a Family First Coordinator to assist with the administrative functions of the Early Intervention Program and for the provision of independent evaluation and consultation services, and WHEREAS, an agreement with Jane Barnett to provide these consulting services has been prepared for authorization by the Medina County Board of Commissioners, and WHEREAS, the agreement is for the time period of July 1, 2018 through June 30, 2019, and WHEREAS, the agreement with Jane Barnett as the Family First Coordinator for Early Intervention Program at compensation rate of $3, per month, for an amount not to exceed $40,200 during this contract period. NOW, THEREFORE, BE IT RESOLVED by this Board of County Commissioners that the agreement with Jane Barnett for the provision of independent contractor services for the Family First Council be approved. BE IT FURTHER RESOLVED that the president of the board be authorized to execute these agreements on behalf of the Medina County Board of Commissioners. RESOLUTION NO APPROVAL OF AN AGREEMENT FOR INTAKE & REFERRAL AND SERVICE COORDINATOR SERVICES BETWEEN MEDINA COUNTY FAMILY FIRST AND CATHOLIC CHARITIES OF MEDINA COUNTY WHEREAS, an agreement to provide intake and referral and service coordinator services from Catholic Charities for Medina County Family First Council Early Intervention Program has been presented to the Board of County Commissioners for approval, and WHEREAS, the term of this agreement will be July 1, 2018 through June 30, 2019, for intake and referral staff and service coordinators; not to exceed $252,302, plus $8,000 for mileage and training expenses, plus $1000 for evaluation and assessment purchases, and WHEREAS, the Board of Commissioners wishes to have this service provided. NOW, THEREFORE, BE IT RESOLVED by this Board of County Commissioners that an intake & referral and service coordinator services agreement with Catholic Charities of Medina County is hereby approved. RESOLUTION NO ACCEPTING AN APPLICATION FOR THE USE OF A PROCUREMENT CARD REQUESTED BY THE VETERANS SERVICE OFFICE WHEREAS, the Board of County Commissioners per ORC Section and ORC Section adopted by resolution procurement card and credit card policies, and WHEREAS, these policies require that each elected official and department submit an application to the Board of County Commissioners to obtain approval for the use of requested credit cards and/or procurement cards, and WHEREAS, an application has been received from the Veterans Service Office of Medina County requesting the use of a credit card, with an account number to be issued, and NOW, THEREFORE, BE IT RESOLVED by this Board of County Commissioners of Medina County, Ohio, that the Veterans Service Office of Medina County is hereby authorized to use the procurement card policy for a credit card. RESOLUTION NO AUTHORIZING EXPENDITURES RELATED TO A MEDINA COUNTY PUBLIC TRANSIT SPONSORED MEETING WHEREAS, Medina County Public Transit will be hosting the Federal Transit Administration for their first Triennial Review on July 24-25, 2018, and WHEREAS, the Board of County Commissioners wishes to allow Medina County Public Transit to host this meeting, and to provide light food and refreshments in an amount not to exceed $100, and NOW, THEREFORE, BE IT RESOLVED that this Board of County Commissioners of Medina County, Ohio, hereby approve the purchase of food and refreshments for this meeting hosted by the Medina County Public Transit. RESOLUTION NO DECLARING MEDINA COUNTY PROPERTY AS EXCESS PROPERTY WHEREAS, requests have been received from various departments of Medina County asking that various vehicles, pieces of office equipment, computers, furniture, and miscellaneous items be declared as excess property, and WHEREAS, the Board of County Commissioners, under O.R.C , finds by resolution that the county has various surplus items previously acquired for the use of county officials which are not needed for public use or are unfit for the use for which they were acquired, the Board may sell such property at public auction or other means, and WHEREAS, some items are either non-functioning, obsolete, or broken and have been determined to have no monetary value, and WHEREAS, the Board of County Commissioners, pursuant to O.R.C (G), finds by resolution that the county property listed below in Table A is now unfit for the use for which it was acquired and has no value, and that the items listed below in Table B be declared excess property, and be made available to be sold by internet auction or other means. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Medina County Ohio that the item listed below in Table A be declared excess property, and that the various Medina County Departments listed below be authorized to discard said property. BE IT FURTHER RESOLVED that property listed in Table B be declared excess property and be made available to the public by internet auction, pursuant to Resolution No , adopted by the Board on February 21, 2012 and that each of the items shall minimally be available for bids by the public for a period of ten days.
