6 Checklist of Required Submittal Items: FINAL PLAT/RE-PLAT Please collate seven (7) packets of each of the following for distribution to reviewers: Exhibit A: Project narrative Exhibit B: Boundary survey (A legal description and boundary survey signed and sealed by a registered land surveyor in the State of Florida.) Exhibit C- Title Opinion (A title opinion of an attorney at law licensed in Florida or a certification by an abstractor or a title company showing that record title to the land as described and shown on the plat is in the name of the person, persons, corporation, or entity executing the dedication.) Exhibit D- Articles of Corporation/Articles of Organization (If Corporation or LLC is applicable) Exhibit E: Current Deed Exhibit F: Preparation of Plat The plat shall be prepared in accordance with the requirements of Florida State Statute Chapter 177, Land Boundaries, Part I, Platting, as amended. In addition, the following requirements shall be adhered to: 1. The plat shall be prepared on sheets eighteen (18) inches by twenty-four (24) inches in size and to a scale sufficient in size to be legible, one hundred (100) feet to the inch preferred. 2. All easements shall be shown on plat. 3. Standard certificates, approval forms, declarations and notes shall be printed on the plat in a permanent manner. 4. All required signatures on the plat shall be signed with black permanent waterproof ink. 5. The plat shall include a vicinity map to show the subdivision's location in relation to the nearest arterial roadway The plat shall incorporate all stipulations, easements, changes and modifications required to make the approved subdivision plan and infrastructure plan conform to these regulations. Exhibit G: Flood Notation (If a property is found to be in the "FEMA/FIRM and SWFWMD approved floodplain area" and accepted by the City, it shall be noted on the plat.) Exhibit H: School Concurrency Determination (If School Concurrency Determination is not included, application will not be accepted.) Exhibit I: Addressing Plan delineating all lots and approved street names. Digital files: The entire submittal package in PDF format on a CD. Also, please submit all project related spatial information in either CAD.dwg, GIS Shapefile or File Geodatabase format on the disk as well. The data must have coordinates in at least 4 corners of the petition area. The Coordinate system must be: A Projected Coordinate System, State Plane, NAD 1983 StatePlane Florida West FIPS 0902 (US Feet).

8 PROCEDURE FOR MYLAR APPROVAL Immediately after plat is approved by Commission, the applicant is to provide (1) signed copy of the Mylar to be prepared as follows: PREPARATION The plat shall be prepared in accordance with the requirements of Florida State Statute Chapter 177, Land Boundaries, Part I, Platting, as amended. In addition, the following requirements shall be adhered to: (1) The plat shall be prepared on sheets eighteen (18) inches by twenty-four (24) inches in size and to a scale sufficient in size to be legible, one hundred (100) feet to the inch preferred. (5) The plat shall include a vicinity map to show the subdivision's location in relation to the nearest arterial roadway. *The following signature lines are required on the plat: Planning and Zoning Advisory Board, By Chairperson City Attorney, By City Attorney City Surveyor, By City Surveyor City Engineer Mayor, North Port City Commission City Clerk, Attest DIGITAL FILES All applications for plat shall include digital files on one (1) compact disk (CD), GIS and AutoCAD requirements plus digital PDF and.dwg files. All disks shall have a project label and date. If there are any changes/revisions to the plat that was submitted with the application, all digital files and paper copies shall be updated and copies with the all changes incorporated will be provided to the Planning Division. RECORDING OF PLAT Upon having obtained the approval of the City Commission, the developer shall submit the signed Mylar with one (1) print copy of the mini-plat for the subdivision which has been approved, to the City within ten (10) days of final approval. The City shall secure all required Rev

9 signatures and record the plat within thirty (30) days of receipt of the approved plat from the applicant. (1) All expenses for the recording of the plat shall be paid by the applicant prior to the City recording the plat. (2) Failure to present the Mylar, with one (1) print copy of the mini-plat for the subdivision plan which has been approved, to the City within ten (10) days of final approval will render the plat approval void. Plats deemed void shall be resubmitted in compliance with the regulations that are in effect at the time of resubmission. No documentation from the previous submission will be carried over from the City's files and all fees for submission of plats shall apply. PROCESS FOR RECORDING 1. Final Mylar is to be signed by the applicant and then delivered to the Planning Division with a check made payable to Sarasota County Clerk of Courts for recording fees ($30 for the first page and $15 per page thereafter). 2. Upon receipt of Final Mylar and check, the Planning Division will contact Engineering Division at Public Works and City Surveyor (Alan Fish), Planning and Zoning Advisory Board Chair, and the City Attorney for signature via , and copies Patsy Adkins (City Clerk), Cynthia Kelly (Records Coordinator) that Mylar is in Planning offices for signature. 3. Alan Fish reviews/signs Mylar and seals it. If there are any discrepancies on the Mylar, applicant will make any corrections, and Mylar will be reviewed by Alan Fish for a final review and signature. When approved, Alan Fish signs and seals the Mylar in accordance with Chapter 177 of State Statutes as the City Surveyor. 4. City Engineer signs and seals the Mylar in accordance with Chapter 177 of State Statutes. 5. After the above signatures/seals are obtained, then tech hand delivers the Mylar to the City Clerk s Office for the Commission Mayor signature. 6. The City Clerk s Office will contact, Commission Mayor, to sign the Mylar. 7. After the Mylar is signed by the Commission Mayor, the Records Coordinator will take the Mylar for recording to the County. Rev

10 8. Upon return from the County, the Records Coordinator will provide a copy of the receipt verifying the recording of plat and the plat book and page number to the Planning Division. 9. The receipt with the plat book and page number is placed in the file, behind the front cover. 10. A copy of the receipt and plat book and page number is provided to the applicant. 11. The project file is now considered closed and project is filed away. Rev
