A copy of three (3) Property Easements granted to Mr. Roy in October, 2005 from Eleanor Sell (former owner of Gene Herbert s property), Edward Murphy and Brian Murphy in order to install one residential meter for his home. 5 Mr, Louis Roy November 20,2006 Page -2- If you deposit with the Utility, in advance, the total sum of $38,386.00, a tap fee of $300.00, and an $85.00 security deposit, the Utility will apply for the necessarypermit applications and begin construction as soon as possible. We anticipate this project will take approximately two months to complete. If the actual cost to construct the extension is less than the estimated cost, the Utility will refund the difference to you, the original depositor. If the actual cost exceeds the estimated cost, the Utility will bill you for the difference. You will be required to sign an Application for Senice when the tap fee and deposit are made to the Utility. This main line extension estimate is made in accordance with PSC Water Rule 5.5 of the Rules for the Government of Water Utilities dated October 24, 2003, a copy of which is attached in its entirety. Also attached is a duplicate copy of Water Rules 1-1 through 1.6. Please sign and date the enclosed statements indicating your receipt and review of both sections of the PSC regulations. Based upon the information you provided, the proposed extension would be 2 pipe which would connect onto the blow-off valve at the end of your existing waterline along that road. The unit costs represent installed prices and are based upon my best estimate at this time.
