3 GMA Certification Pty Ltd 1. Quote/Invoice Explained Quite often a quote will be given to you before forms are completed. If, when the submission is received it differs from the quote given then we reserve the right to make the necessary changes. This can include items such as Added structures shown on the forms which were not clear on the plans Works being assessed are as constructed (already built) Works are being carried out by owner builder It is GMA Certification Pty Ltd policy to not pay any outgoings (incl but not limited to council applications, searches etc) without first receiving payment. It is GMA Certification Pty Ltd policy that no application continues through to the assessment process without first receiving payment. 2. How many sets of plans to lodge GMA Certification is striving to become a paperless office; therefore electronic format is our preferred method. As a result only 1 copy is required for each set of plans (Architectural Plans, Engineering Plans, Soil Test, etc). All paper format documents are scanned into our electronic database. 3. What is a Registered Plan or Survey Plan & Title? A registered plan is a plan of your property showing the true dimensions and layout of your boundaries. The plan, surveyed at sub-division stage, is then registered with the Department of Environment and Resource Management (DERM). If you need to obtain a copy of your registered plan & Title, visit for details. The title provides written information on the easements, such as the dimensions and who the easements belong to. A registered plan & title will show any easements and access restrictions on the allotment at the time the plan was registered. However, additional plans showing easements on your property can be lodged at a later date. When obtaining a copy of the original registered plan & Title make sure you ask for any other registered plans that show any additional easements on you property. In some cases you end up with 2 or 3 plans all showing details of easements on you property. Structures or land surface alterations cannot be built inside councils easements but may be built up to their boundaries in some cases and other cases the council requires that there be a minimum distance that is required for the structure to be built away from the easement boundary. The easement must be kept free of obstructions in the event that access is needed to the easement. 3 of 7

4 GMA Certification Pty Ltd 4. What is a Site plan? A site plan shows the location of all structures on the property. The following details must be shown on the site plan: Location of the proposed structure Setbacks from boundaries these distances should always be to the outermost projection of the structure Any existing building should be clearly marked Property description (Lot & Registered Plan No.) Location of the road and street name Location and dimensions of any easements Total floor area of proposed structure Site coverage (total area of all structures on the property) Scale of plans Contours of the property Building platform level and finished floor level Positions where the heights of retaining walls Driveway location and grade Location of swimming pool fences, gates and filtration equipment including pipelines (where applicable) Location of sewerage lines/manholes/connection points, septic tanks trenches disposal areas Size and location of downpipes and methods of disposal PLEASE NOTE: It is important that all plans are drawn by appropriately licensed persons and must include all required information such as Detailed Site Plan, Floor Plan, Elevation, Section, Timber Sizes, Tie-down, and Bracing. The plans must show all relevant information to enable us to determine compliance with the Building Code of Australia, Australian Standards and Local Authority Town Planning Schemes. The failure to provide information or documentation on receipt of the application could result in the delay in the determination of the application and may incur additional charges for us to obtain the information. 5. What is a Competent Person? Is someone other than the Building Certifier who carries out inspections (usually the engineer who designed the engineering plans). In order for this person to carry out inspection they are required to be deemed a Competent Person by GMA Certification Group PRIOR to the inspection being carried out 4 of 7

5 GMA Certification Pty Ltd 6. When does Queensland Building Services Authority(BSA) insurance apply? Licensed registered Builders undertaking work over $3300 in contract value require BSA insurance (swimming pools do not require BSA insurance). A copy of the Confirmation of Insurance Certificate is to be provided PRIOR to the release of your approval. You should contact Queensland Building Services Authority (QBSA) for all enquiries regarding fees and methods of payment When is an Owner Builder Permit Required? Owner Builders undertaking Building work over $11000 in value require an Owner Builder Permit. A copy of the Owner Builder Permit is to be provided PRIOR to the release of your approval. 8. When does Portable Long Service Leave Levy (Q-Leave) apply? For Building and Construction work where the cost of works is $ or more. The levy is calculated based on the cost of works price and can be paid at your Local Post Office or online at A copy of the payment receipt is to be provided PRIOR to the release of your approval. 9. Application Forms? The application forms which may be required are attached. These forms must be completed in full. Contract of Engagement (The Integrated Planning Act requires that when you engage a Private Certifier it must be by a written contract) Form 1 (IDAS) Common details for all applications, must be completed in full. Form 2 (IDAS) Building work assessment, must be completed in full. Energy Assessment Supplementary Information This form needs to be completed with all new dwellings, room additions/alterations/extensions. This form only needs to be completed if you are engaging GMA Certification to provide the energy assessment. Form 1 (Plumbing) Plumbing & Drainage Act 2002 must be completed when applicable This form only needs to be completed if you wish GMA to submit the plumbing application to your Local Council on your behalf A fee applies for this service. 5 of 7

Electronic Processes GMA Certification are striving to be a paperless office, therefore electronic format is our preferred method for submission and issuing applications and approvals. Applications that come in paper format are scanned into the system, and only one physical copy of the plans are kept in the office for inspection purposes. New Applications A dedicated address has been set up specifically for applications to be submitted. This address is: applications@gmacert.com.au Please note we only need one copy of each document. When the is received, the application is entered into our database in the same process that a physical hard copy submission would be. Additional Incoming Information If at any stage more documentation is required, this can be ed in to the applications address. applications@gmacert.com.au Approvals When the application is ready to be issued to the applicant, the approval will be sent by . Inspections Bookings Inspections can be booked by . A reply is sent back to confirm the booking. The inspection address is: inspections@gmacert.com.au 24 hours notice is still required for all inspection bookings, except for remote areas where we ask for 48 hours notice. When booking inspection please include: Type of inspection Property Address including lot number Date of preferred inspection Site contact person and phone number Any additional notes relating to inspection (i.e an inspector to contact you to arrange access) No guarantee can be given on times requested on inspections as there are constraints on the travel times and distances, however our inspectors will try to see that all notes regarding time of inspections are met. This Contract serves to satisfy the provisions of Chapter 6 of the Building Act 1975 (Act) for the provision of building certification services GMA Certification Group Pty Ltd (GMA) is authorised under s138 of the Act to undertake private certifying functions The Applicant has requested GMA to provide building certification services in respect to the works described at 3 for the address as shown at 4 below 1.4. APPLICANT OBLIGATIONS The Applicant shall: Provide all relevant drawings, plans, statutory plans and other relevant documentation to GMA and declares that the plans that form part of this engagement are a true and accurate representation of the proposed development and illustrate all features on the land including existing building and structures, vegetation and services either above and/or below the ground 6.2. Pay the agreed amount in accordance with this contract or any signed agreement between the applicant and GMA 6.3. Attend any meetings if required by GMA 6.4. Comply with any notices that GMA issues 6.5. Provide any certificates as may be requested by GMA 6.6. Provide all information that the applicant can reasonably obtain to enable GMA to fulfil its contractual obligations 6.7. Act in good faith, in accordance with the Act and in a cooperative fashion CONTRACTUAL TERMINATION If: The applicant fails to pay any money owing to GMA after 7 days of the money becoming payable; or 7.9. The applicant has an execution levied against it, assigns or attempts to assign its estate for the benefit of its creditors, intends to or attempts to makes a composition or Scheme of Arrangement with creditors, has a winding up order made against it, intends or attempts to pass a resolution for winding up, goes into liquidation, has a manager or receiver appointed, has a mortgagee taking possession of any part of the property, has an administrator or provisional liquidator appointed becomes insolvent or bankrupt; or The applicant breaches the contract in any respect; or Does not permit the issuing of the final inspection certificate or certificate of classification within 60 days from the date of practical completion; GMA May terminate this contract by sending written notice to the applicant stating the breaches. Termination will take effect when the applicant receives the notice In the event of termination of this agreement for any reason, any fees held shall be applied to the outlays and services rendered by GMA up to the date of termination In the event of the application lapsing under Ch6 part 5, division 5 of the Sustainable Planning Act or s95 of the Act this contract will also lapse In the event of termination of this agreement for any reason, the Applicant must comply with the s144of the Act regarding notification of termination and appointment of replacement certifier. TERMS OF PAYMENT 8.1. Unless by prior arrangement, all fees are to be paid before the application will be processed Additional fees for re-inspections etc., are to paid prior to the inspection, unless by prior arrangement 8.3. Any Enforcement Action required to be imposed by any Act, Regulation or other statutory instrument will be charged on a cost recovery basis GMA and the applicant agree that time is of the essence for any payment stipulated hereunder, and any payments that are overdue shall attract an interest rate of 24% p.a. You MUST complete ALL questions that are stated to be a mandatory requirement unless otherwise identified on this form. Note, the assessment manager has discretion to accept an application as properly made despite any noncompliance with the requirement to provide mandatory supporting information under section 260(1)(c) of the Sustainable Planning Act 2009 Applicant details Where the applicant is not a natural person, ensure the applicant entity is a real legal entity. Question 1 Schedule 3 of the Sustainable Planning Regulation 2009 identifies assessable development and the type of assessment. Where schedule 3 identifies assessable development as various aspects of development the applicant must identify each aspect of the development on Tables A, B and C respectively and as required. Question 6 Section 263 of the Sustainable Planning Act 2009 sets out when the consent of the owner of the land is required for an application. Section 260(1)(e) of the Sustainable Planning Act 2009 provides that if the owner s consent is required under section 263, then an application must contain, or be accompanied by, the written consent of the owner, or include a declaration by the applicant that the owner has given written consent to the making of the application. If a development application relates to a state resource, the application is not required to be supported by evidence of an allocation or entitlement to a state resource. However, where the state is the owner of the subject land, the written consent of the state, as landowner, may be required. Allocation or entitlement to the state resource is a separate process and will need to be obtained before development commences. Question 7 If the premises is listed on either the Contaminated Land Register (CLR) or the Environmental Management Register (EMR) under the Environmental Protection Act 1994 it may be necessary to seek compliance assessment. Schedule 18 of the Sustainable Planning Regulation 2009 identifies where compliance assessment is required. Question 10 The Building and Construction Industry (Portable Long Service Leave) Act 1991 prescribes when the portable long service leave levy is payable. The portable long service leave levy amount and other prescribed percentages and rates for calculating the levy are prescribed in the Building and Construction Industry (Portable Long Service Leave) Regulation IDAS form 1 Application details Version December 2016

15 Question 10a The portable long service leave levy need not be paid when the application is made, but the Building and Construction Industry (Portable Long Service Leave) Act 1991 requires the levy to be paid before a development permit is issued. Building and construction industry notification and payment forms can be completed on the QLeave website at For further information contact QLeave on Privacy The information collected in this form will be used by the Department of Infrastructure, Local Government and Planning (DILGP), assessment manager, referral agency and/or building certifier in accordance with the processing and assessment of your application.
