7 Forms: Important Tips Carefully review all forms, the Community Stability Guide and Implementation Plan! Incomplete forms will not be eligible for points Each form is specific to a particular technique Forms MUST be signed Don t delay! Meet early with the appropriate signing entity to allow for time to properly complete and sign the form(s) Project sponsor must ensure that all documentation required by the Implementation Plan is submitted with the application 7

8 Forms: Process for Completion Thoroughly review each form Send forms to the appropriate parties Complete forms in their entirety Print completed forms Obtain signatures for each form as needed Scan and save forms and related documents as a PDF or zip files as follows: CS_Form Name_Project Name Upload forms into the AHP Online system under correct technique 8

9 Managing Projects with Multiple Sites Certain techniques require that 75% of sites and/or properties meet the criteria To earn points in these categories, the sponsor is responsible for sufficiently describing the project and project sites Supplemental letters and materials may be required The addresses of the 75% of properties/sites that meet criteria must: Appear on the form Match the addresses provided on the application s Site Screen Present any attachments that require addresses 9

10 Community Stability Point Consideration IMPORTANT: To be considered for points under Community Stability, including system autocalculated points, the sponsor must follow the steps on each of the following three slides 10

15 Local Planning (0-3 Points) Technique Points Required Documentation Plan Consistency 1 Plan Consistency Certification Form Targeted Plan Priorities 2 Plan Consistency Certification Form One point is awarded when plan representative reviews project with sponsor and certifies its consistency by using the Plan Consistency Certification Form If project is awarded one point in this way (for consistency with plan ), then the project will be considered for two additional points for meets targeted priorities, provided that adequate documentation indicates how the project meets the priority You must actively check the box to request both Plan Consistency and Targeted Plan Priorities points in the system 15

19 Distressed Community (Cont d) All designations require Distressed Community Designation Form Visit HUD website for 2018 QCT and DDA Metro or Non- Metro area PDF for details: Visit Federal Reserve website for most current NMMIG: Look for the project s census tract to see if it is eligible as either metro or non-metro QCT, DDA or NMMIG 19

20 Distressed Community (Cont d) Print only the page that indicates the project s eligibility for QCT, DDA or NMMIG Print corresponding FFIEC pages for application addresses: aspx Where required, supply waitlist and/or third party documenting address of project Scan, save and upload all forms under Distressed Community Designation in the AHP Online system 20

21 Major Disaster Declaration Technique Points Required Documentation Major Disaster Declaration 3 Major Disaster Declaration Form & FEMA Declaration Documentation Project must address housing that has been affected by disaster Declaration must be made no more than 30 months prior to Aug. 9, 2018 Designation must receive Individual Assistance or Individual and Public Assistance as defined by FEMA Certification form must be signed by an official from local, state or federal government entity 21

23 Owner-Occupied Rehabilitation Technique Points Required Documentation Owner-Occupied Rehabilitation 10 points 5 points Commit that 75% of properties in the project will be: Fully/comprehensively inspected by qualified individual Certified that all hazardous or uninhabitable conditions are corrected at project completion Certified that all major systems have reasonable (five years) remaining useful life at project completion Qualified individual certification must include: Detailed inspection process Qualifications of individual 5 or 10 None at application. Radio buttons indicate commitment level of standards Property receives needed rehabilitation and/or accessibility improvements on specific components Project does not meet the criteria outlined under 10 points 23

24 Owner-Occupied Rehabilitation (Cont d) No forms or documents are required in this Community Stability technique, but you will need to upload the Owner-Occupied Rehabilitation Form under Readiness to Proceed in the Feasibility section of the application 24

25 Creation of Homeownership Opportunities Technique Points Required Documentation Homeownership rate <50% in census tract 3 FFIEC website documentation Application must include Federal Financial Institutions Examination Council (FFIEC) census tract data for all project addresses 75% of properties must be located in census tracts with homeownership rates <50% 25

28 Blighted Property Technique Points Required Documentation Blighted Property Redevelopment 3 Blight Certification Form At least 75% of the land/structures must meet FHLBank Pittsburgh s definition of blight Project must have $25K of hard rehabilitation costs per rehabilitated unit (if rehabilitation project) The project site must be known and identified The Blight Certification Form must be signed by a state or local government representative, an architect or an attorney 28

29 Brownfield Property Technique Points Required Documentation Brownfield Designation 3 Brownfield Certification Form At least 75% of the property must qualify as Brownfield Project must have $25K hard rehabilitation costs per rehabilitated unit (if rehabilitation project) Brownfield Certification Form must be signed by a representative of the state entity responsible for Brownfield determination 29

30 Historic Property Rehabilitation Technique Points Required Documentation Historic Property Rehabilitation (historic preservation) 3 Property of Historic Significance Form Project must have $25K hard rehabilitation costs per rehabilitated unit (if rehabilitation project) Property must meet definition of historic significance as noted on the form Property of Historic Significance Form must be signed by State Historic Preservation Office 30

31 Preservation of Subsidized Housing Technique Points Required Documentation Preservation of Existing Subsidized Housing 3 Preservation of Existing Subsidized Housing Certification Form Project must have $10K hard rehabilitation costs per rehabilitated unit (if rehabilitation project) Project must be within two years of permitted repayment or expiration of the restricted-use requirement Preservation of Existing Subsidized Housing Certification Form must be signed by the original funder(s) 31

32 Mixed-Income Housing Technique Mixed-Income Housing A. Rental project: 20% or more of units > 60% of the area median income (AMI) B. Owner-occupied project: 20% or more of units > 80% AMI C.
