3 Welcome Ohio Health Choice (OHC) is one of Ohio s first and largest Preferred Provider Organizations (PPO). A PPO serves as a link between health care purchasers (you or your employer), health plan administrators (insurance companies or third party payers), and health care providers (doctors and hospitals). As a PPO, OHC combines the best features of traditional health insurance with the cost savings of a managed care program. You get the benefits of both with the freedom of choice you deserve. The OHC network provides you with access to a comprehensive range of health care services to cover virtually all of your medical needs. For information regarding your coverage, benefits or eligibility, please check with your Personnel Department or call your insurance carrier. Ohio Health Choice is not an insurance company or third party administrator and does not process claims or make claims payments. The providers listed in this directory are independent contractors, and OHC is not responsible for health care provided or the omission, delay or unavailability of health care by these providers. While this listing of providers is complete as of the date indicated within the directory, it is subject to change. More current information may be obtained by calling the OHC Customer Service Department at (440) Outside the Cleveland area, call or checking our website at 2. This directory does not guarantee that all services or all providers will be covered under you health care plan. You must consult your certificate of insurance or other health plan descriptions to determine what type of providers and services are covered. Failure to verify your plan design or network status may result in nonreimbursed medical expenses. 3. Some hospital based providers such as emergency room physicians, radiologists, anesthesiologists and pathologists rendering services at network hospitals may not be members of the OHC network. Please refer to the Statewide Hospital Listing for the status of these providers. 4. Hospitals in the network may provide additional services such as mammograms, urgent care services, laboratory, home health care, etc. as part of their agreement with OHC. For further information on these services, please contact your local hospital or contact us for network status. 5. Some of the OHC PPO providers may provide services that are not part of the OHC network (i.e., surgical procedures in a doctor s surgical suite). Please contact the provider s office to determine how services will be billed. Contact your benefits office to determine if your coverage is affected. How to Use the Network Select a provider from the directory or call Customer Service at (440) (outside the Cleveland area call ) for assistance in locating a provider. Confirm that your provider is currently part of the OHC network. Always show your membership card to the receptionist when you arrive for your appointment. Some participating providers may require that you pay for all or part of your expenses at the time of service. Any medical expenses not covered under your health benefit plan (i.e., copays, deductible, and coinsurance) are generally payable at the time of service. If your provider needs to refer you to a facility or another health care provider, remind the provider that you are part of the OHC network and be sure the provider refers you to a participating provider when possible. If you use a non-participating provider, your benefits may be reduced. If you have any questions regarding OHC providers, call Customer Service at (440) (outside the Cleveland area call ).
