Qualified Person s Statements and Certifications As the qualified person, I certify that: A phase one environmental site assessment of the RSC property, which includes the evaluation of the information gathered from a records review, site reconnaissance, interviews, a report and any updates required, has been conducted in accordance with the regulation by or under the supervision of a qualified person as required by the regulation. As of 2015/08/06, no phase two environmental site assessment is required by the regulation for the RSC property and based on the phase one environmental site assessment for the RSC property, in my opinion, it is not necessary for any other reason to conduct a phase two environmental site assessment for the RSC property. As of 2015/08/06, in my opinion, based on the phase one environmental site assessment, there is no evidence of any contaminants in the soil, ground water or sediment on, in or under the RSC property that, if the RSC property were put to any of the types of property uses listed in subsection 1 (2) of the regulation, are likely to interfere with any of those types of property uses. I am a qualified person and have the qualifications required by section 5 of the regulation. I have in place an insurance policy that satisfies the requirements of section 7 of the regulation. I acknowledge that the RSC will be submitted for filing in the Environmental Site Registry, that records of site condition that are filed in the Registry are available for examination by the public and that the Registry contains a notice advising users of the Registry who have dealings with any property to consider conducting their own due diligence with respect to the environmental condition of the property, in addition to reviewing information in the Registry. The opinions expressed in this RSC are engineering or scientific opinions made in accordance with generally accepted principles and practices as recognized by members of the environmental engineering or science profession or discipline practising at the same time and in the same or similar location. Phase One Conceptual Site Model 741, 747 Sheppard Avenue West, Toronto 1 Existing buildings and structures A vacant dwelling, with a basement, was present at 747 Sheppard Avenue West, while the property at 741 Sheppard Avenue West was a vacant parcel of land. The former dwelling at the northwest portion of the site was demolished in 2014. The surface cover over much of site is sand and gravel with vegetation. An asphalt paved driveway is located northwest of the existing dwelling. 2 Water bodies in the Study Area No water bodies were identified within the Phase One Study Area. 3 Areas of natural significance in the Study Area. 4 Drinking water wells at the Phase One property No areas of natural significance were identified in the Study Area. No drinking water wells were identified within the Phase One Study Area. 5 Roads within Study Area Roads within the Phase One Study Area are indicated in Figure No. 1 and 2. 6 Property uses adjacent to Phase One Property 7 Areas of Potentially Contaminating Activity (PCA) 8 Areas of Potential Environmental Concern (APEC) Adjacent property uses are indicated in Figure No. 2. Potentially Contaminating Activity identified within the Phase One Study Area, relates to a former paving contractor's yard which included a maintenance/repair facility at 748 Sheppard Avenue West. PCAs include PCA#57 : Vehicles and Associated Parts Manufacturing, PCA#30 : Importation of Fill Material of Unknown Quality No Areas of Potential Environmental Concern were identified within the Phase One Property. 9 Description of PCAs The PCA at 748 Sheppard Avenue West, located 30m north/northwest of the Site relates to the presence of a former paving contractor's yard. According to Ministry of the Environment and Climate Change, a Record of Site Condition (RSC), #211406, was submitted on 2013-12-06 for intended Residential used. A review of the RSC indicated that a Phase Two ESA was carried out at the property, due to environmental concerns related to former maintenance/repair facility (toos and equipment maintenance PCA#57) and paving contractor at west side of 748 Sheppard Ave W, and at the central portion due to former coal storage(pca#30), and at the southern portion due to de-icing activities. Apart from the de-icing activity, which was expected from Sheppard Avenue West, the other APECs were from onsite activities. No other offsite PCAs were identified. All soil and groundwater samples met the applicable Table 3 Site Condition Standard criteria and no excavation or remediation was undertaken. Therefore, offsite impacts/migration of potential contaminants is not expected. Therefore, this was not considered as a PCA that would cause an APEC onto the subject site. 10 Description of APECs and Contaminants of Concern (COC) No Areas of Potential Environmental Concern were identified within the Phase One Property. 11 Underground utilities Subsurface water and gas utility mains are located along the east side of the dwelling at 747 Sheppard Avenue West. Overhead hydro wires are located at the northeast corner of the dwelling. 12 Regional or site specific geological and hydrogeological information Surface drainage is expected to infiltrate into the ground and/or run off onto the roadways. Review of Toronto Region Conservation Authority (TRCA) watershed map indicated that the Site is located in

the Don River watershed. Based on the topographic map (Natural Resources of Canada The Atlas of Canada Toporama) and TRCA watershed map, the Site is located at an approximate elevation of 185m above mean sea level. The contours indicated a down-gradient in the easterly to southeasterly direction towards Don River West branch. Groundwater flow is expected to be in the easterly to southeasterly direction towards Don River West branch, located more than 1km from the site boundary. According to the Ontario Geophysical Survey OGS Earth application, the Phase One Study Area is situated within a coarsetextured glaciolacustrine deposits with sand, gravel, minor silt and clay as well as an undifferentiated older tills which may include stratified deposits. The bedrock geology in the vicinity of the Study area is the Georgian Bay/Blue Mountain/Billings Formation (Collingwood/Eastview Member), generally consisting of shale, limestone, dolostone, and siltstone. According to City of Toronto Interactive Maps, no water bodies or areas of natural significance are located in the vicinity of the Study Area. The site is not in proximity to the Oakridges Moraine. 13 Uncertainty or absence of information. A review of the Ontario Geological Survey Map P. 2204, Geological Series, Quaternary Geology of Toronto and Surrounding Area of Southern Ontario indicated that the depth of bedrock is approximately 77m below existing grade, (approximate geodetic elevation of 110m asl. Information from Freedom of Information office was not received at the time of preparation of the Phase One ESA. However, the lack of information is not expected to change the environmental conclusions of this ESA.
