REGISTRATION CHECK LIST Each registration shall contain the following information. Please consult with the Community Development Department at City Hall, 1201 Civic Center Blvd, Yuba City, or call (530) 822-5190 for assistance. All Applications: Completed General Information Form. Completed Residential including the applicant(s) and property owner(s) signatures and a detailed description of the proposed project. Proof/copy of physicians recommendation for medical marijuana and/or proof of patients physicians recommendation if applicant is a primary caregiver (as defined by the Health & Safety Code Section 11362.7). A current valid Medical Marijuana Card is acceptable. Acknowledgement from landlord/property owner (if applicable). Right of Entry for Inspection Form. Grow Area Description Form. 1

GENERAL INFORMATION City Code Title 8, Chapter 5, Article 54 describes the criteria for approving a Residential Cultivation Use. In order to be approved for a Residential Cultivation Use, the use must meet all content set forth in Sections 8-5.5415 and 8-5.5420, including: A primary caregiver or qualified patient must reside at the residence. The property must be the primary residence of the medical marijuana grower. Primary residence means that the property is occupied on a permanent basis for living, cooking, and sleeping, etc. The proposed cultivation must be ancillary to residential activities. The use of accessory buildings, outbuildings and/or greenhouses are acceptable for residential cultivation. The total square feet of cultivation area in all grow buildings is limited to an accumulated total of 75 square feet per residential property. No equipment, materials, supplies, merchandise or goods in connection with this use shall be stored or visible off of the property. From the public right-of-way, there shall be no visible exterior evidence of marijuana cultivation. Outdoor cultivation is prohibited. Medical marijuana for sale is prohibited. The property must be at least 700 ft. from any school, and 350 ft. from any childcare center, or public park. Medical marijuana cultivation areas, whether in a detached structure and/or inside a residence on the same parcel not be conducted or accessible to persons under 18 years of age. Indoor grow lights shall not exceed 1,200 watts. Gas products or generators shall not be used within any detached structure. Any detached or residential structure used for the cultivation of medical marijuana must have a ventilation and filtration system installed that shall prevent marijuana plant odors from exiting the interior of the structure. Registration shall be valid for no more than two (2) years and may be extended in increments of two (2) years. ~~~~~~~ I have read and understand the above requirements Signed 2
