The HUD 103 form is to be completed and inserted in tenant file. After inspection tenant receive two keys and moves in. Property manager or clerk creates work order for maintenance to light the pilot and check for gas leaks in unit. Interim Process Report a change in household composition, the tenant has to come in the property manager s office to report the change within 10 days. The tenant needs to bring into the office the following: Income Verification (Current Pay Check Stubs & Employer complete mailing address) TANF (Most recent print out) Unemployment Compensation (Statement if approved or denied) Child Support Award Letter or print out During the interim process the tenant must complete the following: Public Housing Application Tenant Reported Changes form Personal Declaration Employment Verification Job Termination Verification (If applicable) After verification forms have been verified and management has entered updated information into yardi the tenant is mailed a notice of rent adjustment. EIV should be printed and inserted in tenant file. A zero income checklist form if tenant is reporting no income in the household.

PH Reexamination Process Housekeeping notice should be mailed and inspection should be completed before re-exam Tenant is sent a letter to come into the property management office for their reexamination. PH Move Out Process Tenant gives a 10-day Notice Tenant brings keys into property management office Property management inspects unit. (Management takes pictures of unit if needed) Property management completes Form 103 and conduct yardi procedures for move out. HUD Form 103 should be mailed along with the Move-out statement Unit is assigned to maintenance
