Information for Guests of Lions Transplant House Welcome from the Trustees of Lions Transplant House A very warm welcome to the Lions Transplant House. We hope that you will find the House a home-away-from home at this stressful time. All of the occupants of the House are liver transplant patients and their families - please introduce yourself to them. Please contact one of us if you have any concerns about the House. History of the Lions Transplant House When the New Zealand Liver Transplant Unit was being set up, many people realised the importance of providing an accommodation facility for liver transplant patients and their families - a Transplant House. We thought that Transplant House should provide something of the feeling of being at home and not just to be a place to stay. Although the New Zealand Liver Transplant Unit receives funding from the Government to provide accommodation for patients from out of Auckland if needed, there was no funding to set up a Transplant House. The NZLTU approached the Lions Club and proposed that Lions sponsor Transplant House (as well as other major set up costs of the New Zealand Liver Transplant Unit). Through the great generosity of the people of New Zealand and the Lions, including Lions International, a very large amount of money was provided. This house was purchased in May of 1999 and refurbished from those funds. The House was opened by the Prime Minister (The Right Honourable Helen Clark) and the President of Lions International in November 1999. In 2000 the Lions Transplant House was gifted to a Charitable Trust ( The Lions Transplant House Trust, PO Box 110-015, Auckland Hospital). The Trust is responsible for operating the House and is not part of the NZLTU. The vision of the Trustees is reflected in the Mission Statement of the Trust : To provide liver transplant patients and their support persons with high quality accommodation in a home-like environment. The House opened for patients and families in May of 2000. What does the House consist of? The House consists of three fully-furnished self-contained apartments - Apartment A at the top upstairs, Apartment B in the middle at the front and Apartment C at the back. Each apartment has two bedrooms, a bathroom, a living/dining area, telephone, TV, video, CD/tape/radio system, fridge/freezer, cooktop, dishwasher, microwave, vacuum cleaner and all necessary minor appliances. Laundry facilities for Apartments B and C are in the garage and are in the bathroom of Apartment A. There is a small shared library of books, videotapes, jigsaw puzzles, games and childrens toys. The back garden is a shared space and there is a shared gas barbeque in the garage. Occupants The house is designed for patients and one immediate support person. On occasion (e.g. parents with children) more than one family member may stay with the patient. This will be at the discretion of the Trust after recommendation by the NZLTU. Apartments A (top) has a double and two single beds, Apartment B (middle) has a double and one single bed and Apartment C (back) has a double and two single beds. Visitors Occasional visitors may stay overnight within the existing numbers of beds in each 1

apartment. Outside of this, overnight visitors are not permitted. Smoking All areas of the House (including the garage and hallways) are non-smoking. Please do not smoke in the House or allow any of your visitors to smoke in the House. Shared and private spaces Each apartment is largely self-contained and is a private space for each family. There are some facilities in the garage and this area, together with the garden at the back, are shared.spaces. Please respect the privacy of the guests in the back flat if you are using the garden. Please keep these areas clean and tidy. Financial responsibilities The Trust The Trust is responsible for maintaining the House including the garden and for paying the rates, electricity and telephone rental including the costs of local calls. The Trust provides a small one-off amount of tea, coffee, sugar, toilet paper, a paper hand towel, one rubbish bin liner, liquid hand soap, dishwashing liquid and other household cleaners. This will not be replenished by the Trust during your stay. Families and patients Families and patients are responsible for personal costs of living including food and disposables, toiletries, doing personal laundry and for the cost of toll calls. There will be no charge for local telephone calls. Alarms Each apartment has its own separate burglar alarm and fire alarm. These are monitored by Chubb. The operation of the burglar alarms will be explained to you. The fire alarm system incorporates smoke detectors which will set off a loud alarm in the event of fire. There is a separate manual fire alarm system ( Break glass and flick switch ) which sounds a loud alarm but does not alert the fire department. There is a floor plan map on the right of the front door on the outside which tells the fire department which manual alarm has been set off. Telephone There are separate telephones in each apartment. Please use a Telecom Calling Card or a Clear Card for toll and cell-phone calls as all the telephones have toll bars which prevent toll and cell-phone calls from being made. The numbers are : Apartment A - top apartment upstairs - (09) 374 4351 Apartment B - middle apartment at front - (09) 374 4352 Apartment C - back apartment - (09) 374 4353 These numbers are all unlisted and will not be given out by Directory Assistance. Please give these numbers to your family and friends on a need to know basis Noise Please be considerate of the other occupants of the House and keep noise to a minimum at all times, and particularly after 10 p.m. Do not use the washing machines after 10 p.m. Parties are not permitted. Medical emergencies Please use the instructions that you have been given by the NZLTU. During normal working hours please call Auckland Hospital (379-7440) and ask for Extension 6874 (the Liver Transplant Co-ordinator). After hours ask to speak to the Liver Transplant Co-ordinator oncall. Mail The Postal address of the House is 2 Glasgow Terrace, Grafton, Auckland 1003. All mail will be delivered to the single letterbox. Please ask your families to use only this address on mail sent to you. Pets 2

Pets are not permitted. Keys Every family will be given a set of keys (a key for the garage, for the front door and for your own apartment). Please do not cut any extra keys. If you need another set of keys please contact a Trustee. Every Trustee has their own full set of keys. Please lock your apartment and turn on the alarm whenever you go out Rubbish There are small rubbish bins in each apartment. Please buy bin liners for these, and when they are full please tie them securely and put them in the green wheelie bins at the side of the House. Each apartment has its own wheelie bin and separate blue bins for recyclables (glass, plastic). Please recycle these materials. Please put out your own wheelie bin and recyclables every Thursday morning. Please keep wheelie bins off the driveway so access is not impeded. In addition, clean paper (newsprint and magazines) is collected by Paperchase at the same time. Please put paper in bundles at the kerbside next to the wheelie bin and recyclables. Cleaning Each family is responsible for keeping their apartment clean. Each apartment has a vacuum cleaner, dust-buster, mop and bucket and a range of cleaning fluids to use for this. If there is a major spill on the carpets, please contact Jane Streat at once. In addition, the Trust will provide a commercial cleaner who will clean each apartment from 2:30 to 4:00 p.m. once a week. Apartment A (top) will be cleaned on Tuesday, Apartment B (middle) on Wednesday and Apartment C on Friday. This may sometimes occur when you are out. The House Manager (Jane Streat, who is also a Trustee) will be on-site Wednesdays and Fridays around lunchtime. If there are any household problems please contact her at any time. If Jane Streat is not available please call one of the other trustees. Carparking There is enough parking space for three cars in the area in front of the house for guests (and occasionally their visitors). There is no space for carparking in the garage area. Please do not park on the footpath. Food The Trust provides a little tea, coffee and sugar for when you first arrive. Please keep these replenished as you use them. The nearest supermarket is at 277 Broadway, Newmarket - about a 20 minute walk from the House. The Trust may be able to help with transport for grocery shopping - please contact Jane Streat. Library There is a small library of books and videotapes for the shared use of the guests of the House which are kept in the foyer inside the front door. Please return the books and tapes after you have finished with them. Laundry Sheets, pillowcases, handtowels, towels, bathmats and teatowels are all supplied. There is a spare change of sheets for every bed in your unit. There is further linen in a cupboard in the garage if your require it. Please do all your own laundry. 3

Friends of the House If you or one of your family or friends would like to volunteer (Friends of the House) to help in a variety of ways contact Jane Streat. Garden The gardens will be developed and maintained by Friends of the House. If you want to do a little light work in the garden you are very welcome. Please contact Jane Streat. We have a few garden tools and two garden hoses which you can borrow. Donations to the Trust Warehouse, a cinema complex and a swimming complex including pool, sauna and gymnasium. The Domain This is a large public park adjacent to Auckland Hospital. Inside the Domain is the War Memorial Museum (which has a café on the ground floor), the Wintergardens, the duckpond and numerous walking trails. Outhwaite Park This is a small public park in Carlton Gore Road just around the corner from the House. It has a small playground for children. The Lions Transplant House Trust is a Charitable Trust registered for tax purposes under the Charitable Trusts Act. The Trust receives donations and from time to time some family members or friends may wish to donate to the Trust. These donations are not expected but would be gratefully received. Please contact a Trustee. Local facilities Grafton shopping area This small shopping area is five minutes walk from the house and includes a number of cafes, take away food outlets, a delicatessen, hairdresser, florist, bakery, travel agent, garage and petrol station Auckland Hospital In the Foyer on the Ground Floor of the Hospital are an EFTPOS facility, a retail pharmacy, Muffin Break cafe, Florist, Post shop Newmarket This large shopping area is twenty minutes walk from the House and includes a large number of retail outlets and services. 277 Broadway is a large shopping mall which has a Woolworths supermarket and various food and retail shops. Elsewhere on Broadway there are numerous major retail stores including a 4
