After a qute has been agreed upn, the Lessee must cmplete a Facilities Rental Agreement (FRA) and submit a depsit f 50% f the ttal rental fee. N dates will be held withut the apprpriate fees and signed cntract. All events must be bked at least ne (1) mnth in advance f the event date. Events may nt ccur during nrmal gallery hurs unless specifically apprved by the Directr r Deputy Directrs. This includes all time spent setting up the event. Lessee is respnsible fr all guests at their event and assumes all liability shuld damage, carelessness, theft r ther unfreseen incidents ccur during their rental f the Space. This includes vilatin f all plicies stated in this dcument. Lazarus Building Security team has permissin and authrity frm the Space t enfrce any and all Lazarus Building plicies nt stated here and all plicies upheld by the Space as well. FOOD & ALCOHOL: OSU Urban Arts Space AND The Ohi State University must apprve the presence f alchl fr all events at least six (6) weeks in advance f event date. The Urban Arts Space apprves the hire f all bartenders. In accrdance with the State f Ohi Divisin f Liqur Cntrl permit and the Urban Arts Space guidelines, alchl may nly be prvided by and sld by the Urban Arts Space. The Directr and Deputy Directrs reserve the right t limit sales per individual per hur, require a valid state issued license fr service, and limit the kinds f alchl served, i.e., wine and beer service nly. Lessees, exhibitrs, r visitrs may nt bring alchl int the Urban Arts Space fr cnsumptin. In accrdance with ur Department f Health Fd License and the Urban Arts Space guidelines, fd intended fr public cnsumptin may nly be prvided by a catering service. Fd nt prepared in a facility that is nt inspected by the Department f Health may nt be brught int the space fr public cnsumptin. Fees: Upper Gallery Rental ($175 per hr), Lwer Gallery Rental ($300 per hr), Upper & Lwer Gallery Rental ($400 per hr). All recgnized Ohi State University student rganizatins (as cnfirmed by Events Student Assistant r Deputy Directrs) can hst events at the Space free frm Space rental fees assuming that their event is free, pen t the public, educatin-related (as determined by the Directr) and held within nrmal gallery hurs. General Plicies After a qute has been agreed upn, Lessee must cmplete a Facilities Rental Agreement (FRA) and submit a depsit f 50% f the ttal rental fee. N dates will be held withut the apprpriate fees and signed cntract. The FRA must be returned t the Space with an riginal signature (verbal agreement, electrnic, r scanned signatures are nt valid). Unless therwise specified, all payments will be made t and in the name f The Ohi State University (UAS). Acceptable frms f payment are check, mney rder, cash, r credit card. All events must be bked at least ne (1) mnth in advance f the event date. The Space reserves the right t cancel an event if the facility rental frms are nt held in cmpliance r if payments are nt received n time. Rental Hurs Events may nt ccur during nrmal gallery hurs unless specifically apprved by the Directr r Deputy Directrs. This includes all event set up time. On Tuesdays, Wednesdays, Fridays and Saturdays this means events can begin setting up n earlier than 6PM. On Thursdays, set-up may begin at 8PM. The Space is available fr event rentals 7AM t 10PM utside f gallery hurs. Lessee will be charged fr time assciated with set-up and clean-up. Fr all events, Lessee s guests must exit the Space by 10PM, and cleanup must be cmpleted by 11PM. In accrdance with the State f Ohi Laws and Lazarus Building cdes smking is prhibited inside the building and within 25 feet f entry ways. Designated smking areas are acrss Twn St., High St. Frnt St. r State St. Lessee is respnsible fr all guests at their event and assumes all liability shuld damage, carelessness, theft r ther unfreseen incidents ccur during their rental f the Space. Lazarus Building Plicies There is NO smking allwed anywhere within 25 feet f the Lazarus Building. Designated smking areas are acrss the street in any directin. Alchl is NOT permitted in the cvered hallway area (heren Galleria ) utside the hallway attached t Urban Arts Space. N vehicle shall be driven r parked n any sidewalk r unmarked area beside r in the Lazarus Building. This includes the Galleria area. Vehicles fund in vilatin will be immediately ticketed and twed. Lazarus Building Security team has permissin and authrity frm the Space t enfrce any and all Lazarus Building plicies nt stated here and all plicies upheld by the Space as well. Lading Dck area f the Lazarus Building may be used fr up t 20 minutes per vehicle fr necessary event drp ffs and pick ups. Lading Dck may NOT be used fr vehicle parking unless prir arrangements have been made fr special circumstances. Lading Dck gate will pen n earlier than agreed-upn time as stated in Lessee s FRA. There is NO strage, temprary r therwise, permitted in ANY area f the Lazarus building prir t r after Lessee s event. This includes (but is nt limited t), the Lading Dck area, the Galleria, the trash/recycling rm, the hallways attached t Urban Arts Space, the sidewalks and lawn areas adjacent t the Lazarus Building, elevatrs, etc. Vilatins f this plicy will be grunds fr the Space t assess additinal fees. Additinally, lessee s items r lessee s rented items will nt be prtected frm theft r dispsal at the discretin f the Lazarus Building Security team r Urban Arts Space staff members. Cancellatin Plicy Rental cancellatin refunds after cntract and payments have been submitted are as fllws: If event is cancelled 90+ days prir t event then 100% f depsit will be returned. If event is cancelled between 30 and 89 days prir t event then 50% f depsit will be returned. If event is cancelled less than 30 days prir t event, n depsit will be returned. If Lessee has submitted a 100W fr payment, the 100W will act as a depsit and will be billed fllwing the abve guidelines.

WEB uas.su.edu Event Operatins Lessees are encuraged t visit the Space within tw weeks f their event t view the current layut f the exhibitin(s) and t cnfirm event layut. Client must present all space requirements, equipment requirements, and prgram details t the Urban Arts Space Events Student Assistant r Deputy Directrs n later than TWO (2) weeks prir t the event date. Failure t meet this requirement may result in the nn-availability f equipment and/r persnnel fr this event. Equipment requests are nt guaranteed fr every event. Lighting f the Space cmes as-is in cnjunctin with current exhibitins. A set f flurescent main lights thrughut the Space may be turned n at Lessee s request but typically are nt used utside f clean up time. N alteratins may be made t ur lighting system. Lessee may bring in freestanding lighting if desired but may nt use ur light track system fr additinal lights. Sales Any sales made within the Space are subject t 30% cmmissin. This is nnnegtiable. Sales may include but are nt limited t: entrance fees t event, auctins, fd r beverage tickets, prduct sales, dnatins Raffles, bing games, r any ther game f chance in rder t raise mney is prhibited frm ccurring in the Space. Set-Up & Clean Up Urban Arts Space exhibits shall NOT be mved, rearranged, r therwise altered by Lessee in preparatin fr r during an event. Prir arrangements t have exhibitin pieces mved may be pssible and fees wuld be assessed fr this. Nthing may be set up r stred in public areas during regular perating hurs unless the Directr r ne f the Deputy Directrs f OSU Urban Arts Space has granted prir authrizatin. N alteratins may be made t the walls, flrs, ceilings, tables, chairs, r any ther prperty belnging t OSU Urban Arts Space. This includes (but is nt limited t) the use f tacks, tape, nails, screws, 3M-type hangers, rpe, paint, pens r similar items may nt be used in any rental space. A lading dck is available. It has ramps but prvides smth access t the Space. All deliveries, drp-ffs and pick-ups requiring use f the lading dck must be crdinated thrugh the OSU Urban Arts Space staff and the Lazarus Building Security team.

WEB uas.su.edu All rentals must be delivered and picked up during the Set-Up and Tear Dwn times utlined in the FRA. Lessee will be charged additinal fees fr strage f items utside this time. Fees fr strage f event items utside f the agreed upn event hurs is $100 per day. These fees will nt be prrated. Strage may NOT ccur prir t event time per Rental Hurs under General Plicies seen n Page 1. Client must exit premises by agreed-upn time r risk facing additinal staffing and facility rental charges. Alchl Plicy OSU Urban Arts Space AND The Ohi State University must apprve the presence f alchl fr all events at least six (6) weeks in advance f event date. This is nt negtiable. (See: Ohi Revised Cde 4303.18) If requested and if the service f alchl is apprved, the Urban Arts Space can hire bartending staff at the expense f the Lessee. All bars shall clse and entertainment shall end thirty (30) minutes prir t the end f the event, and Lessee will encurage guests t depart by the clsing time f the event. Any persns cnsuming alchl are required t abide by the Ohi Liqur Laws. Lessee assumes all respnsibility and liability and will hld The Ohi State University and the Urban Arts Space unaccuntable frm any liability that may result frm illegal practice r vilatin f these laws. Intxicated persns, habitual excessive drinkers, and minrs will nt be served and will be mnitred. Events serving alchl may require cntracting an ff-duty plice fficer thrugh The Ohi State University. The Directr r Deputy Directrs f Urban Arts Space will make the determinatin if this is necessary r the Lessee may make a request fr the service directly. Lessee will be ntified if service is deemed necessary and will be respnsible fr the assciated fees.

Fd Plicy In accrdance with ur Department f Health Fd License and the Urban Arts Space guidelines, fd intended fr public cnsumptin may nly be prvided by a catering service. Fd prepared in a facility that is nt inspected by the Department f Health may nt be brught int the space fr public cnsumptin. The Urban Arts Space kitchen is nt fr available fr Lessee s use. Smke and humidity can damage art wrks. Please cnsult with the staff if yur caterers plan t prvide fd that requires heat frm any type f heat surce. Vendrs Lessee is welcme t cntact vendrs fr services including, but nt limited t: catering, entertainment, music, phtgraphy, flrals, décr, lighting, etc. OSU Urban Arts Space will need the names and phne numbers f these utside vendrs tw (2) weeks prir t the event, entered n the Event Terms dcument. Lessees wh cntact vendrs must relay t them all relevant infrmatin frm this dcument, the FRA, Event Terms and all emails and verbal agreements Lessee has made with Space staff members. Lessee assumes all respnsibility fr vendrs and will ensure their cmpliance with all regulatins set frth by the Space befre set-up f an event ccurs. All deliveries and pickup times must be crdinated with Urban Arts Space staff. Lessee r a representative fr the Lessee must be present t sign fr deliveries. Urban Arts Space staff will NOT sign. It is understd and agreed upn that Lessee and vendrs, agents, emplyees, and attendees use OSU Urban Arts Space at yur their risk, and OSU Urban Arts Space shall nt be liable fr any damage arising frm persnal injuries sustained by any such persns n the premises f OSU Urban Arts Space. If vendrs will arrive befre the lessee, they must check in with the Space staff and/r event captain prir t set up. Urban Arts Space staff will nt sign fr any deliveries n behalf f utside events. Arrangements shuld be made t have a representative frm yur event available at time f delivery.

WEB uas.su.edu Space Rental Rates Rental Rates are cmprised f gallery rental fees and mandatry staffing fees. Clients are required t cver staffing fees f $17 per persn, per hur during their event and as needed fr set-up and/r cleanup. Gallery staffing is prvided t mnitr the gallery, set up equipment prvided by the Space, answer questins and be dcents fr the artwrk. Gallery staff is nt respnsible fr event peratins utside f the Space s prvided facility management and equipment. Gallery staff will NOT sign fr any vendrs r event equipment being drpped ff. Staffing Fees are based n yur event specificatins including (but nt limited t) event type, presence f alchl, estimated guest cunt, etc. These fees are nnnegtiable and will be determined by the Deputy Directr(s) and the Events Student Assistant. Gallery Rental Fees: Upper Gallery Rental ($175 per hur) Maximum Capacity: Seated 125; Standing 200 (cntingent upn exhibitin). Lwer Gallery Rental ($300 per hur) Maximum Capacity: Seated 200; Standing 300 (cntingent upn exhibitin). All Gallery Rental ($400 per hur) Maximum Capacity: Seated 325; Standing 500 (cntingent upn exhibitin). Tech Supprt includes ne prjectr, ne speaker set, and ne Macintsh laptp. Technical supprt shuld be requested ne mnth prir t event. ($25 per event & cntingent upn exhibitin) All recgnized Ohi State University student rganizatins (as cnfirmed by Events Student Assistant r Deputy Directrs) can hst events at the Space free frm charge f Space rental fees assuming that their event is free, pen t the public, educatin-related (as determined by Urban Arts Space staff) and held within nrmal gallery hurs.
