I am a Senior Paralegal with Porzio, Bromberg & Newman, P.C. ("Porzio"), counsel to Allen D. Wilen, as assignee (the "Assignee") for the benefit of creditors herein. Porzio is located at 100 Southgate Parkway, Morristown, NJ 07962-1997. I am over eighteen years of age and not a party to the above-captioned action. Unless otherwise noted, I have personal knowledge of the facts set forth herein. 2. I submit this certification in accordance with N.J.S.A. 2A:19-1 et seq., (the "ABC Statute"), in order to provide the Court with a true list, under oath, of all the creditors of the Assignor who have provided claims to the Assignee. 3118248

This Court authorized Porzio's retention as counsel to the Assignee on June 15, 2015, nunc pro dunc to March 9, 2015 (the "Assignment Date"), the date in which the Assignee executed a Deed of Assignment for the Benefit of Creditors (the "Assignment"). NOTICE OF THE ASSIGNMENT AND SERVICE OF THE PROOF OF CLAIM 4. On March 26, 2015, I served a copy of the Order To Show Cause, entered on March 24, 2015, along with additional pleadings filed with this Court on March 13, 2015, to all known creditors of the Assignor. Included in that service was a Notice of Assignment, dated March 13, 2015, and a blank proof of claim form. A copy of the corresponding Certificate of Service for that mailing, filed with this Court on April 2, 2015, is annexed hereto as Exhibit A. 5. In addition to the March 26, 2015 service, publication of the Assignment was also made in the New Jersey Law Journal ("NJLJ"), once a week for four consecutive weeks. Annexed hereto as Exhibit B is a copy of the Affidavit of Publication stating that the notice was published on April 27, 2015, May 4, 2015, May 11, 2015 and May 18, 2015. 6. Pursuant to the ABC Statute, creditors of the Assignor have three (3) months from the Date of Assignment to file proof of claim forms with the Assignee. That deadline date was June 13, 2015. 7. During those ninety (90) days, I received, reviewed and logged all proof of claim forms. Each proof of claim submitted to Porzio was date-stamped and entered into a log sheet to track pertinent information from each claim form. 8. A complete list of all claims received by Porzio to date is attached hereto as Exhibit C. There were no proof of claim forms received that were excluded from the log. 9. For the avoidance of doubt, the claims listed on Exhibit C represent what was filed with Porzio, but the Assignee has not yet agreed to the validity of any of the claims and 3118248 2
