In accordance with this Code, all residential rental properties are required to be licensed. No person shall operate a rental dwelling unless he holds a current unrevoked operating license. Please complete the following steps to apply for your Rental Dwelling License: 1. Complete and submit a Rental Dwelling License application and remit with the licensing payment to the Village of Palatine for the rental dwelling license. The application and license fee should be mailed to the Village of Palatine, Neighborhood Services Division, 200 E Wood St, Palatine, IL 60067 2. Contact our office at 847-359-9042 to schedule an inspection of the property. The inspection will not be scheduled until payment and application have been received. The owner or his/her designated agent must be present at the inspection. 24 hours' notice is required if an owner or agent must reschedule or cancel a scheduled inspection. If an owner or agent fails to appear or cancel 24 hours in advance, a cancellation fee of $67.00 will be assessed. 3. The Crime Free Multi-Housing Village Ordinance requires you to attend a seminar. You are also required to adopt and enforce a Crime Free Lease Addendum. Call Officer Gulisano of the Palatine Police Department at 847-359-9016, in order to reserve a seat at a seminar. 2018 Rental Licensing Annual Fees: Single unit (condo, townhome, single family home)..$78.00 Multi-dwelling building.. $22.00 per unit or $109.00, whichever is greater If you have any questions regarding the applicable fee due for your property, please contact our office at 847-359-9042 prior to submitting your payment and application. Payment must accompany application. We do not accept faxed or emailed applications. We do not accept payment by phone or online. RENTAL INSPECTION INFORMATION: Your rental property must be inspected and found in compliance with Village Code. It is necessary that you make arrangements with your tenants to gain access to the dwelling units for inspection. Owners should provide tenants with a minimum of 72 hours notice prior to any scheduled inspection. If the initial inspection fails, a reinspection is required. Reinspection fee(s) must be paid prior to scheduling a reinspection. The base license fee is $22.00 per unit or $109.00 per building, whichever is greater. A property owner can maintain this minimum inspection requirement and base license fee if they maintain an average of 3 or less violations based upon the number of units inspected. A common area shall be counted as a single unit in the calculation of the average number of violations per dwelling unit. Properties having an average of 4 to 5 violations will be charged $78.00 per unit for the license renewal the following year and 50% of the units will be inspected the following year. Properties having an average of 6 or more violations: 1.) will be required to have 100% of the units inspected within 60 days of the initial inspection in the same year. 2.) will pay $129.00 per unit for the license renewal the following year. 3.) 100% of the units will be inspected the following year. Single Family Dwellings (attached, detached and condos): The rental licensing fees for Single Family Dwellings will be based upon condition of the rental properties: Inspections of a single-family rental dwelling will be reduced from annually to every other year. The yearly base license fee is $78.00. A property owner can maintain this inspection requirement and the yearly base license fee if they maintain an average of 3 or less violations. Properties having 4 or more violations will be charged $145.00 for the license renewal the following year and will require an inspection that year. If you, the owner, are unable to be present for scheduled inspections, or attend a Crime Free Multi-Housing seminar, you must designate an agent for your property. If you live a significant distance from Palatine, it is recommended that you appoint an agent for your property. An agent does not necessarily mean you need to hire a property manager or real estate agent, you can ask a close friend or family member over the age of 18 who is willing to help. As your designated agent, your agent may: - Call our office to schedule the inspection of the rental property, and be present at that inspection. (You may designate your tenant as your agent for inspection purposes only.) - Register for and attend a Crime Free Multi-Housing Seminar, and adopt and enforce a Crime Free Lease Addendum. (You may not designate your tenant as your agent for Crime Free Multi-Housing purposes, as the program is not designed for tenant attendance.) You may designate more than one agent if you wish. All agent names and contact info must be listed and each agent must sign the application. If you designate a property management company as your agent, the management company s name and contact info must be provided, as well as the individual names, contact info and signatures of all employees of that company who will be present at inspections of your property, or completing Crime Free Multi-Housing requirements on your behalf. If owner or agent information changes, you are required to submit a new license application form. A newly submitted license application form replaces any applications currently on file. APPLICATIONS WITH PRINTED OR ELECTRONIC SIGNATURES ARE NOT ACCEPTED ALL CORRESPONDENCE RELATING TO THE RENTAL DWELLING LICENSE WILL BE MAILED TO THE OWNER. OUT OF STATE OWNERS If you live outside of Illinois, you are required by Village ordinance to designate a local agent. The agent represents an out of state owner in the event of an emergency at the rental property, and can schedule and be present for inspection appointments, and become certified in the Crime Free Multi-Housing Program. Inspections are conducted Monday through Friday, from 8:30 a.m. to 3:30 p.m. We will schedule a specific date and time for your inspection. We meet only with the owner or their designated agent for the inspection. The inspection can only be scheduled after the application and payment have been received in our office. The owner or designated agent should call our office at 847-359-9042 to schedule the inspection appointment. The name of the owner or agent who will be present will be listed in the appointment. Our office must be notified prior to the inspection if there is a change in attendance. If an unauthorized individual appears for the inspection in the place of owner or agent, the appointment will be cancelled and a $67.00 cancellation fee will be assessed. The Inspector will meet owner/agent at the front entrance of the building. Do not wait in the rental unit for the Inspector; meet him at the front door of the building. Owner or agent must have key or code necessary to gain entrance to the building/rental unit. Failure to gain access will result in cancellation of the appointment and a $67.00 cancellation fee will be assessed. Failure to appear for a scheduled appointment without providing 24 hours notice (for an a.m. appointment) or by 11:00 a.m. (for a p.m. appointment) will result in a cancellation fee of $67.00. At the completion of the inspection, the Inspector will provide you with a copy of the Inspection Report: If the inspection is approved, no reinspection will be necessary. If the inspection fails, you are required to pay the assessed reinspection fee, correct any code violations noted on the report, and schedule the reinspection of the unit. To schedule a reinspection appointment, you are required to call our office at 847-359-9042 to be completed within the time frame given by the Inspector. RENTAL DWELLING INSPECTION CHECKLIST Interior Neighborhood Services Division 200 E Wood Street, Palatine, IL 60067 (847) 202-6673 Smoke Detector/ CO Detector Functioning smoke detector must be present and functioning on every floor level within the dwelling Functioning smoke detector must be installed on the ceiling or wall outside each separate sleeping area in the immediate vicinity of bedrooms Functioning smoke detector must be installed in every bedroom Functioning carbon monoxide (CO) installed within 15ft of all bedrooms Hardwired smoke detectors must be maintained as such Doors /Windows /Common Areas Doors leading into a garage must selfclose and latch Apartment doors leading into a common corridor must self-close and latch Exterior door locks (interior keyed deadbolts are prohibited) Windows and doors must be easily operable, in good condition and aligned to the frame Screens on all windows must be installed between April 15th to October 15th Balconies and joists are in good condition free from rotted material and capable of supporting the imposed dead and live loads. Electrical No electrical hazards should be present such as inadequate service, deterioration or damage All Ground Fault Circuit Interrupters GFCI) are functioning Pull chain light fixtures are secure and string properly attached Breaker box is labeled to its contents No open slots or loose breakers in breaker box Garbage disposal is functioning and no loose wiring Plumbing Water heating facilities shall be properly installed, maintained and capable of providing adequate amount of water at every faucet (hot water temperature must be maintained not less than 110F All plumbing fixtures must be properly installed and functioning No visible water hazards should be present such as back-siphoning or cross connections Interior Surfaces Walls and surfaces shall be maintained in good, clean and sanitary condition Walls and ceilings are intact and free from holes or water damage Kitchen and bathroom cabinetry shall be maintained in good, clean and sanitary condition Floors throughout living spaces are free from cracks and in good repair Stairs and walking surfaces shall be maintained in sound condition and good repair Handrails must be properly installed and secured to walls Washer and Dryers (if applicable) Dryer must vent to the outside of the dwelling and be free of obstruction Area behind dryer and washer is clean, free from dust and other debris Furnace, Water Heaters and Boilers (if applicable) All installations comply with the State Plumbing Code. Properly vented to the outside of the dwelling Vent pipes are properly attached Gas and water lines are installed correctly Metallic pipe installed off the water heater relief valve (pipe extended to within 6 inches of the floor) No flammable storage located within 3 ft Free from rust or any defects Air Conditioning Units (wall mounted) Units are in good working condition Unit cover is in good condition and properly installed No water leaks observed surrounding unit Electrical wiring is in good condition Pest Control General NOTE: No infestation of cockroaches or other pests No infestation of mice or other vermin No infestation of bedbugs Surfaces are free from food or other debris that can cause infestations Units are free from rubbish and garbage All installed appliances (i.e., stoves, refrigerators etc) must be in good working condition Basements used as a sleeping space must meet existing code requirements. Heat must be maintained at 68 F from September 15th to May 1st Living areas should be maintained in a clean, safe and sanitary condition Storage throughout living areas should be limited to allow a 3 ft means of egress to each exit and to each room This list includes most items covered in an inspection, but is not all inclusive. The property owner or agent is responsible for notifying tenant at least 24 hours before an inspection.

Roof drainage shall be adequate to prevent dampness or deterioration in the walls or interior portion of the structure Buildings shall have approved address numbers placed in a position to be plainly legible and visible from the street or road Screens on all windows must be installed between April 15th to October 15th Walls shall be free from holes, breaks, and loose or rotting material; and maintained weatherproof and properly surface coated where required to prevent deterioration Foundation walls shall be maintained plumb and free from open cracks or breaks No graffiti or other vandalism Decks/Sidewalks Stairs and walking surfaces shall be maintained in sound condition and good repair Handrails and guards shall be firmly fastened and capable of supporting normally imposed loads Sidewalks, walkways shall be kept in a state of proper repair Balconies/ Patios Storage on balconies and patios shall be limited to outdoor lawn furniture, potted plants or approved barbeque grills Balconies and joists are in good condition free from rotted material and capable of supporting the imposed dead and live loads Patios must be free from cracks or defects Driveways and Parking Driveways must be in good condition and free from cracks or defects Parking spaces must have clear visible markings Motor vehicles must be parked on an approved surface Accessory Structures All accessory structures including detached garages, fences and walls shall be maintained structurally sound and in good repair Garbage/ Refuse Storage General NOTE: Dumpster enclosures must be maintained, structurally sound and in good condition Containers must be free from garbage overflow and lids covered Exterior grounds must be free from accumulation of rubbish or garbage No Rodent harborage All premises shall be graded and maintained to prevent the erosion of soil and to prevent the accumulation of stagnant water thereon This list includes most items covered in an inspection, but is not all inclusive. The checklist will help you navigate not only the Village Ordinance requirements but also assist in reminding you about the Crime Free Program, the mandatory addendum and the importance of having a screening standard in place when selecting future Palatine residents for your property. Additionally, the Police Department is now offering training to your renters to help ensure they know their rights as a tenant, their responsibilities to the upkeep of your rental and the enforcement of the Village Ordinance as it relates to the Crime Free Program. An invitation to this training is attached for you to provide to your renter. Please feel free to utilize these documents annually to make sure you are meeting all village requirements as well as help give you the confidence that you are selecting good renters and notifying them appropriately about their community responsibilities. Landlord Rental Checklist This checklist is intented as a guide for landlords to ensure they are completing all requirements for the village ordinance as well as a safeguard for landlords when signing the lease. As you complete each task, mark or initial its respective box until you have completed them. It is recommended to complete this checklist annually, when leases expire or are renewed or you renew your license with the village. I have applied and paid for my new license or paid to renew an existing license. I have contacted Neighborhood Services at 847-359-9042 to schedule my rental inspection. I have reviewed the rental dwelling checklist that was provided to me for life safety/bldg maintenance violations I have registered to attend a Crime Free Seminar if I have not attended one in the past*. I have adopted the mandatory Crime Free Addendum and have had my renters sign it along with the lease. I have put a screening standard in place to ensure I follow all Federal Fair Housing Laws and select a renter that enhances the community. I have made my renters aware, through the signing of the Crime Free Addendum that their behavior and the behavior of their guests is directly associated with their ability to retain rental housing. I have made sure my renters have a way to contact me or my agent should they need assistance at the property. I have made my renters aware that the Palatine Police Department offers training throughout the year to renters so they may learn about the Crime Free Program as well as landlord/tenant rights. *A Crime Free Seminar is only required if you are a new landlord. If you have taken a seminar in the past or in another town, you must contact 847-359-9016 to determine if your attendance status is accepted. * If you have purchased a new property in Palatine you must provide a signed copy of your Crime Free Addendum to Officer Gulisano.

CRIME FREE LEASE ADDENDUM As part of the consideration for lease of the dwelling unit identified in the lease, Resident agrees as follows: 1. Resident and Residents Occupants whether on or off of the property; and Residents and Residents Occupants guests and invitees, are prohibited from: a. Engaging in any criminal activity, including drug-related criminal activity, on or off the said premises. Drug related criminal activity shall mean the illegal manufacture, sale, distribution, use, possession and possession with intent to manufacture, sell, distribute, or use an illegal or controlled substance (also as defined in Section 102 of the Controlled Substance Act [21 U.S.C. 802]). b. Engaging in any act intended to facilitate criminal activity or permitting the dwelling unit to be used for criminal activity. c. Engaging in the unlawful manufacturing, selling, using, storing, keeping or giving of an illegal or controlled substance as defined in Illinois Compiled statutes, at any locations, whether on or near the dwelling unit premises. d. Engaging in any illegal activity, including, but not limited to prostitution, criminal street gang activity, threatening or intimidating as prohibited in, assault as prohibited in, including but not limited to the unlawful discharge of a weapon, on or near the dwelling unit premises, or any breach of the lease agreement that otherwise jeopardizes the health, safety and welfare of the landlord, his agent, or other tenant, or involving imminent or actual serious property damage,. 2. VIOLATION OF ANY ABOVE PROVISIONS SHALL BE A MATERIAL AND IRREPARABLE VIOLATION OF THE LEASE AND GOOD CAUSE FOR IMMEDIATE TERMINATION OF TENANCY. A single violation of any of the provisions of this addendum shall be deemed a serious, material and irreparable non-compliance. It is understood that a single violation shall be good cause for immediate termination of the lease. Proof of such a violation shall not require a criminal conviction, but shall only require a preponderance of the evidence. 3. Resident hereby authorizes property management/owner to use police generated reports against Resident for any such violation as reliable direct evidence, and/or as business records as a hearsay exemption, in all eviction hearings. 4. In case of conflict between the provisions of this addendum and any provisions of the lease, the provisions of this addendum shall govern. 5. Resident also agrees to be responsible for the actions of Residents occupants, Residents guests and invitees, and Residents occupants guests and invitees, regardless of whether Resident knew or should have known about any such actions. A guest or invitee shall be anyone who Resident or Residents occupant gives access to or allows on the premises or in the rental unit. 6. This Lease Addendum is incorporated into the lease or renewal thereof, executed or renewed at any time between Landlord/Manager and Resident/Lessee.
