The owner should provide you with a transmitter for the vehicle gate entry. If you need an additional transmitter, the cost of the transmitter is $50.00. Only two (2) transmitters per unit are allowed. Please make checks or money order out to Bella Vista on Lake Seminole. 2. Your home phone number or cell phone number must BE programmed into the gate system in order for your visitors to notify you when they arrive at the gated entry. The gate access system will not display the pone number of the residents. Your guest will press the call button which will automatically call your telephone number or cell number at which time you may open the gate by pressing the number six (6) if you live in buildings A and B or you may press nine (9) for buildings C through P on your telephone or cell phone. (Hold down for at least four seconds and hang up). Please provide your home or cell telephone number to the Management Company at your earliest convenience. 3. Keep clear of the gates at all times. The automatic gate is not for pedestrian use. Serious injury or death may result from entrapment in any moving parts of the gate or gate opening system. Management is not responsible if the gate hits your vehicle because you rushed through the gates. 4. Do not allow children near the gates. Serious injury or death may result if children are allowed to play near the gates. 5. Do not operate the gates unless you can see them and pedestrians and obstructions are clear of the path of the gate. 6. A qualified technician should perform gate operating system repair and service. Each condo unit may bring up to two (2) guests for use of the pool area, but must be accompanied by an adult at all times. 3. Lifeguards will not be present. All persons using the pool will do so at their own risk. 4. An adult must accompany children under the age of sixteen (16) at all times. 5. All persons using the pool shall obey the capacity requirements as defined by Pinellas Co0unty. 6. Proper swimming attire shall be worn while using the pool (bathing suits only). 7. Showering is mandatory before entering the pool. 8. Diving is not permitted. 9. Water sports ( pool balls or tag games) are not permitted. 10. Flotation devices are not permitted in the pool, except for water wings used by small children. 11. Fighting or rough play is not permitted in the pool. 12. Running is not permitted in the pool area. 13. Food or drinks are only allowed at the provided tables and in the grilling area. 14. Alcohol is not permitted at the pool. 15. Glass containers are not permitted at the pool. 16. Audio equipment brought to the pool shall be utilized solely with earphones. 17. Pool furniture shall not be moved from their original placement or may not be stood upon. 18. Bella Vista on Lake Seminole is not responsible for personal belongings left at the pool area.

Box 1624 727-785-2183 Palm Harbor, Fl. 34682 Fax 727-773-0263 EXCLUSIVE USE OF CLUBHOUSE FOR A PLANNED PARTY RULES AND REGULATIONS The reserved use of the CLUBHOUSE is $75.00 for 4 hours or less 6:00 PM to 10:00 PM With a refundable deposit of $200.00 Area will be inspected the next day and deposit check will be returned via mail only. EVENT DATE: EVENT TIME: The deposit is refundable as long as there is no damage to furnishings, flooring, fixtures and or the equipment, and the entire space is left in a clean condition, with all trash bagged and removed from the area entirely. Therefore two checks must be received by the Association s Management, one for the usage fee, which will be deposited prior to the event and one held until after an inspection is conducted regarding the conditions which were left after the space rental. Both checks will be made out to Bella Vista on Lake Seminole. IMPORTANT: Your checks must be received one week in advance prior to your booked date or your reservation will be cancelled without notice from the Association. All cancellations must be received within in 3 days of the event date or you will be charged $30.00 cancellation fee, This rule is to prevent last minute cancellations so everyone will have a fair chance of using the clubhouse. Person arranging the event must be 21 years of age and present at the event. Occupancy is limited to 10 people at the event. The pool area will not be closed to other residents. Children age 12 and under will be supervised while in the pool. No alcohol will be permitted at the event. No glass containers will be permitted at the event. All table and chairs will be cleaned and put back in place. The fitness room will be off limits to the guests. Any additional cleaning deemed necessary by Management will be ducted from the deposit. I agree to take full responsibility for any and all damages, which may occur during the use of the clubhouse, during my pre-arranged event. I agree to abide by the rules and regulations stated above.
