Marie Social Housing Application Centre to ensure that people have equal opportunity and ease of access to the waiting lists of all members of Sault Ste. Marie Social Housing Application Centre. Members consist of Local Non-Profits, Co-Ops and the Sault Ste. Marie Housing Corporation. Complete all sections of the attached application form. Please print all information in ink. ELIGIBILITY REQUIREMENTS FOR RENT-GEARED-TO-INCOME (RGI): [O. Reg. 367/11 s. 24 - s. 32] At least one member of the household is 16 years of age or older and is able to live independently with or without support services. Each member of the household must be a Canadian Citizen, has made application for status as a permanent resident or has made a claim for refugee protection under the Immigration and Refugee Protection Act (Canada). No removal order has become enforceable against any member of the household.. No member of the household owes arrears of rent or money owed as a result of damage they caused with respect to any previous tenancy in any housing project under any transferred housing program. The Sault Ste. Marie Social Housing Application Centre may not require payment if it is satisfied that there are extenuating circumstances or a suitable agreement to repay the housing provider has been arranged. No member of the household has been convicted of an offence under Section 36 of the Housing Services Act, 2011 or a crime under the Criminal Code (Canada) in relation to the receipt of rent-geared-to-income assistance, or found by the Landlord and Tenant Board (formerly known as the Ontario Rental Housing Tribunal) or convicted of an offence of misrepresentation of income for the purpose of Rent-Geared-To- Income. All RGI tenants will be required to pursue all possible sources of income, that they may be eligible for, including Ontario Works, Child Support, Employment Insurance and Immigration Sponsorship Support. A household that owns residential property that is suitable for year-round occupancy, whether the property is in or outside of Ontario, must sell the property within 180 days after receipt of rent-geared-to-income subsidy. (May be subject to review on a case-by-case basis). The household s Gross Household Income is less than or equal to the Service Manager s Household Income Limit amount applicable to the size and type of unit. If your income is greater than the Service Manager s Household Income Limit amount, you can apply directly to the Non-Profits and Co-operatives for market rent units. Applicants and Tenants who knowingly misrepresent their income will be ineligible for RGI subsidy, and will be subject to fines or imprisonment. DEFINITION: SPECIAL PRIORITY HOUSEHOLD CATEGORY A household that the Service Manager has determined eligible to be included in the Special Priority category on the waiting list is a household where: [O. Instructions for Making an Application to the SSM Social Housing Application Centre If you are applying for Special Needs/Modified Housing, you must complete the entire application including Section 7. Verification of all income and assets will be required before an application can be processed including realtor appraisals of unsold properties; proof of reasonable efforts to sell property will be required upon acceptance for rent-geared-to-income assistance. If you are receiving Ontario Works or Ontario Disability Support Program, a copy of your drug or dental card and cheque stub will also be required. For Part-time custody, legal verification or a Notarized/Commissioned document is required verifying you having overnight custody. A Co-Applicant is any household member 16 years of age or older. When your application is received, it will be reviewed to ensure it is complete. You will receive written notification stating whether or not your application is complete; reasons will be listed if incomplete. You will be given 10 days to respond to an incomplete notice, failing which your application will be cancelled & shredded. It is very important that you can be contacted at the phone number(s) you have provided. Any changes to your application ( income, address, telephone number, etc.) must be reported to the Social Housing Application Centre within 30 days after the change. [O. Reg. 367/11 s.28. (1-7)]. Failure to notify the Social Housing Application Centre may result in the cancellation of your application. You must state all sources of income of those in your household and provide proof of each, including last year s income tax return (if previous years taxes have not been filed, verification is required from the Canada Revenue Agency). DEFINITION OF INCOME Income means the total amount of all payments of any nature paid to or on behalf of or for the benefit of the member, subject to exceptions. O. Marie Social Housing Application Centre, (Application Centre) knowing that they will be relied upon by the Application Centre and its member social Housing providers, to assess my qualifications for rental accommodation and to establish rent: 1. I/We understand the requirements for reporting all household income and assets and I/we agree to comply. I/We have reported all income received and all assets currently owned and any assets transferred within the last three years by every member of the household. 2. I/We understand that if rental accommodation is provided to me/us, that accommodation is to be occupied only by myself, the co-applicant(s) and those persons listed in Section 3 Other Members subject to approval. 4. I/We will notify the Application Centre within 30 days of any changes in my/our circumstances/application while I/we are on the waiting list. 5. I/We will notify the appropriate social housing provider within 30 days of any changes in my/our circumstances once I/we are placed in a housing unit. 6. I/We declare that I/we am/are in Canada legally. 7. I/We understand that it is an offence, under the Housing Services Act, 2011, for an applicant or any individual to knowingly obtain or assist a household member to obtain rent geared to income assistance for which they are not entitled. Such an offence carries up to a $5,000 fine and/or up to 6 months imprisonment as well as a prohibition from re-applying for assistance for a period of two years. If something on this application is missing, incorrect or false, the Application Centre, or the housing providers I/we have applied to may request additional information or may cancel my/our application. CONSENT TO DISCLOSE AND VERIFY INFORMATION 1. The disclosure of information contained in this application and associated documents and verification is done for the purpose of processing the application including, but not limited to: determining the eligibility of the household for subsidized housing, determining the size and type of unit in respect of which the household is eligible to receive subsidized housing, determining the placement of the household on waiting lists and determining the amount of gearedto-income rent/housing charge payable by the household. Any information contained on this form, or in attachments, is collected by the Application Centre and associated housing providers, pursuant to the Housing Services Act, 2011). This information will be used to determine the eligibility of housing applied for, the continuation of eligibility for geared-to-income housing, and may be used to determine the appropriate geared-toincome rent/housing charge and other purposes allowed by law. 2. I/We consent to the release of any information to the Application Centre about any bank account, safety deposit box, assets of any nature or kind held by me/us, or on my/our behalf, or by or on behalf of any of my/our dependants or children temporarily in my/our care, alone or jointly with any other person in any financial institution. 5. I/We further consent to the exchange of information with any social housing provider associated with the Application Centre, an Ontario Works delivery agent, a credit bureau, the Government of Canada, the government of any other province or territory, the Government of Ontario, or any agency, Ministry or department of any of the foregoing, or any party in order to verify information for the purposes of determining or verifying initial or continued eligibility for and administration of my/our rent-geared-to-income-assistance. Any arrears information will be shared with Access Centres across the province once the lease or occupancy agreement is terminated. 6. I/We understand that this consent will apply to inquiries made relating to my/our initial eligibility for, as well as my/our past and present receipt of rent-geared-to-income assistance. I/we further understand that the inquiries may take the form of electronic data exchanges. 13

I/We, undersigned, understand that any member of the household may make a request for an internal review of certain decisions made, with which I/we do not agree, regarding this Application for housing and, if housed, regarding the subsidized tenancy. 8. I/We hereby release the Application Centre, all associated housing providers, any employee, officer, agent or contractor from any liability or claim arising from the collection, storage, use or dissemination of any information received or collected pursuant to this Declaration, Release and Consent to Information. In the event that I/we am/are provided with rental accommodation as a result of my/our application, I/we acknowledge that my/our eligibility shall be reviewed at least every twelve (12) months and that I/we have the same obligation to provide information required by the review. In the event that I/we am/are provided with rental accommodation, this Declaration, Release and Consent to Information shall remain in force and be enforceable against me/us by the Application Centre and my/our housing provider, in addition to any other obligations with respect to the Declaration, Release and Consent to Information which may be imposed upon or agreed to by me/us. 9. I/We understand that any information on this form and any attachment given by the Application Centre to a body listed above is confidential and will only be given in accordance with the Housing Services Act, 2011.
