Lease Addendum Moving In and Out Moving In Checklist 1) Signed lease by all occupants of apartment. 2) Completed application form, including SS#, home address, email and cell phone number. 3) Cosigner form signed and notarized. 4) Full payment of first, last and security deposit. Tenant may not take occupancy of the apartment until after all four items above are completed by each individual who will occupy the apartment. The keys will be handed out at the commencement of the lease unless the tenant makes special arrangements with the landlord. In the event the tenant is unable to pick up the keys on this date, the tenant agrees to pay a fee of $25 for mailing the keys to the tenant. If the apartment floors are not left in broom-clean condition and the bathroom and kitchen appliances are not cleaned by the prior tenants, the landlord will clean the apartment within 5 working days after you move in upon your request. This means the following: 1) Living room, hallways, and bedrooms will be broom swept; and 2) Kitchen and bathroom(s) will be mopped and all appliances thoroughly cleaned. The landlord will not take requests to clean the apartment after 5 days. The tenant will be responsible for cleaning the apartment after this time. Any additional work may take up to 15 days to complete. These items may include painting, holes in the walls, cracked tiles, broken windows, leaky sinks, etc. These items will be fixed once the landlord is given a written and reasonable list of the items that need to be addressed. The landlord will fix all reasonable requests in a timely manner, giving priority to certain emergency items like non-working appliances, plumbing and electric problems. I understand that my apartment may not be cleaned to my satisfaction on my move in date as the previous tenants moved out the previous day. Tenant signature:
Moving Out Checklist 1) Turn off utilities. A fee of $200 will be deducted from the security deposit in the event the tenant does not turn off the utilities after 10 days of the termination of the lease. 2) Clean apartment. 3) Leave all keys on the stove. Landlord will deduct time and materials to rekey locks or $25 per missing key. 4) No trash is to be left outside the house, except in the designated trash bins. 5) No trash or storage items are to be left in the hall or the basement. 6) All smoke and carbon monoxide detectors are to be left in working condition. 7) The apartment must be left in the same condition it was found in. 8) Tenant must replace all nonworking light bulbs (included recessed light bulbs for recessed lighting) before moving out. Otherwise landlord may charge for time and materials to replace the bulbs. 9) Provide landlord with forwarding address and notify the post office to forward your mail. Security Deposit Last Month s Rent Co-signer Form Security Deposit The security deposit is equal to one month s rent and is to be paid at the signing of the lease. You may not move into your apartment until the entire security deposit is paid. The deposit covers small damage done to the apartment and common areas by the tenant, late fees, and any fines incurred by the tenant during the term of the lease (i.e., improper garbage disposal fines, etc.). The security cannot be used in lieu of a rent payment. If you attempt to do so, you may be sued or face eviction by the landlord. You will also forfeit your entire security deposit. The security deposit (or balance thereof after itemized deductions are made) will be mailed in the form of a check per apartment designated by the tenant within 20 days of the lease termination. In the even the tenant does not provide a forwarding address, the security deposit letter (only) will be mailed to the tenant s former address. Co-Signer If the tenant makes an annual salary of less than $65,000 a co-signer is required. A guaranty form must be completed and received by the landlord 10 days before the commencement of the lease in order to move into the apartment. This form is available at www.provrents.com. The form must be signed, notarized and mailed to PO Box 83 Wayland, MA 01778. The co-signer may be anyone over 21 years of age who has a good credit rating and makes over $65,000 a year. Otherwise, the co-signer must be a parent. The co-signer is only a secondary means of financial collection. The tenant will still be held responsible if the rent is unpaid. Rent Payment Late Fee Trash Fines Rent Payment The rent is due for the apartment on the first day of every month. The rent is to be paid monthly at www.provrents.com. No other form of rent payment will be accepted by the landlord during the term of this lease without written permission from the landlord. Late Fee The tenant has a grace period of three additional days after the first of the month to pay rent. This means that rent is considered past due if paid after the fourth day of each month and a late fee of $50 will be automatically charged. The tenant agrees to pay this late fee with each late payment made. If the late fee is not paid by the tenant, the tenant agrees that the landlord may deduct double the late fee ($100 per incidence) from the security deposit. If the tenant fails to pay the rent by the 15 th of the month, a legal demand notice ( Rent in Arrears ) will be sent by the landlord. This will provide the tenant with five additional days to pay the rent, or else face possible eviction proceedings. In the event tenant does not pay rent on or before the due date, tenant agrees to notify the landlord immediately. Trash Fines Tenants are responsible for putting out the trash and recycling containers on the night before designated trash pickup day. The trash may not be put out before 5pm on the night before trash pickup takes place. Otherwise, the city of Providence may levy a fine that will be deducted from the tenant s security deposit. For most properties located in the Federal Hill, Amory District, and upper Oakland Avenue., the designated trash pickup day is Friday unless there is a holiday that week, in which case the designated trash pickup day will be the next day (i.e., Saturday) for most apartments. Other properties located in Elmhurst, Lower Oakland Avenue (below Smith Street) and Harborside areas have trash pickup days other than Friday. All trash barrels and recycling containers must be collected by 5pm on the trash pickup day. The tenant will be responsible for all fines for improper storage of containers by the city of Providence. Tenants are also responsible for all trash fines incurred during the term of their tenancy. A $75 processing fee will be added to all garbage fines paid by the landlord. The fee will be deducted from the security deposit. Unless a given apartment/individual is known to be responsible for a given fine, the fine and processing fee will be pro-rated among all tenants living in the building. This means that trash
collection and disposal is a shared responsibility. All tenants must cooperate to ensure proper disposal of trash takes place on a weekly basis. Failure to dispose of trash, including overflowing containers, may result in a large trash fine by the city of Providence. All trash fines will be deducted from the security deposit. The tenant is also responsible for keeping all common areas, including stairs, hallways, basements, sidewalks and backyards free from trash.
