Owner Information RSC Property Information Environmental Condition ESA Information QP Profile Information Certification Statement QP Certification Statement Owner Certification Statement Supporting Documents QP Certification Statement A As the qualified person, I certify that: * A phase one environmental site assessment of the RSC property, which includes the evaluation of the information gathered from a records review, site reconnaissance, interviews, a report and any updates required, has been conducted in accordance with the regulation by or under the supervision of a qualified person as required by the regulation. As of 2011/05/25, no phase two environmental site assessment is required by the regulation for the RSC property and based on the phase one environmental site assessment for the RSC property, in my opinion, it is not necessary for any other reason to conduct a phase two environmental site assessment for the RSC property. As of 2011/05/25, in my opinion, based on the phase one environmental site assessment, there is no evidence of any contaminants in the soil, ground water or sediment on, in or under the RSC property that, if the RSC property were put to any of the types of property uses listed in subsection 1 (2) of the regulation, are likely to interfere with any of those types of property uses. I am a qualified person and have the qualifications required by section 5 of the regulation. I have in place an insurance policy that satisfies the requirements of section 7 of the regulation. I acknowledge that the RSC will be submitted for filing in the Environmental Site Registry, that records of site condition that are filed in the Registry are available for examination by the public and that the Registry contains a notice advising users of the Registry who have dealings with any property to consider conducting their own due diligence with respect to the environmental condition of the property, in addition to reviewing information in the Registry. The opinions expressed in this RSC are engineering or scientific opinions made in accordance with generally accepted principles and practices as recognized by members of the environmental engineering or science profession or discipline practising at the same time and in the same or similar location. Description and Assessment of Phase One Study Area I. There are no areas of potentially contaminating activity on or potentially affecting the phase one property that has occurred. II. III. Areas of potential environmental concern (APEC) are labelled in the drawing to the right, and are at least 100 m away from the RSC property. They consists of a Dental Office, Sutton s Garden Supplies and The Animal Cat Hospital is a registered waste generator with the MOE. All three of these areas pose a very low risk to the phase one property because of the relatively impermeable silt, clay soils, as is described in the site geology and found in Bore-1 soil descriptions from the Ecolog-ERIS report which is included in our Phase I report. No potential for underground utilities were found that could potentially affect contaminant distribution and transport. There are no main utility passing through the RSC property. Hydro main service lines and the feeder line to the property run overhead. Gas and water service connections do not normally have gravel base and would likely have been backfilled with the relatively impermeable soils native to the RSC property. Sewer lines will likely have a gravel base but these will be draining downwards away from the RSC property and therefore it is very unlikely that any potential contaminants from the main sewer lines which runs under Finch Avenue East will travel up to the RSC property. IV. The surficial geology at the phase one property, as published in 1955 in Map 1955-6, Township of North York showing Pleistocene Geology, consists of: Till: The uppermost till is overlain in most places by a thin layer of modified till consisting chiefly of varved clay containing pebbles, stones, and irregular layers of till. Bore-1 which was located at the Cat Hospital, at 119 Finch Avenue and is about 100 m west of the phase one sites and is described in the Ecolog-ERIS report which is included in our Phase I report. The soils found in bore-1 consist of silt and sand fill in the upper 400 mm and silt, clay lacustrine, wet age post glacial soils below this to a depth of 2.1 m. V. Ground water is not known for the study area, but is expected to exist below the impermeable soils at the site. The groundwater in the general area of the phase one study area is expected to flow in a easterly direction toward the tributary of the Don River, but there are no PCA east of the phase one property. Surface water is flowing from the south to the north toward Finch Avenue East and there are no PCA south of the phase one property. Thus both ground water and surface water flow up stream of the property are through other residential properties and are not of concern. The conceptual site model is based on the information collected through the completion of the Phase I ESA and is subject to the limitations stated in the report.
