FINAL PLAT SUBMITTAL CHECKLIST The submission requirements for a Final Plat Review are found beginning in Section 24-4-40 of the Weld County Code. The Weld County Code is available on line at www.co.weld.co.us (Quick Link-County Code) The Department of Planning Services may request additional information, application packets and/or fees that will aid in the review of the application. APPLICATION REQUIREMENTS: The applicant shall submit the final plat application fee and one packet of the required final plat application materials as follows: One original final plat application form One digital or electronic copy of completed application materials, if required One legible copy of all deeds or legal instruments identifying all fee owners of property under consideration. Signatures of all fee owners of property must sign this application or if an Authorized Agent signs, a letter of authorization from all fee owners must be included with the application. If a corporation is the fee owner, notarized evidence must be included showing the signatory has the legal authority to sign for the corporation. One original of the final plat map. The plat map shall be prepared in accordance with sections 24-4-40.D.6 and 24-4-40.D.7 of the Weld County Code, except it shall show only the property under consideration for the final plat. One copy of statement from the Weld County Treasurer showing no delinquent taxes exist for the final plat area One copy of a title commitment issued by a title insurance company or a title opinion by an attorney licensed to practice in the State. The commitment or opinion shall set forth the names of all owners of property. The commitment or opinion shall include a list of all mortgages, judgments, liens, easements, contracts and agreements of record in the County affecting the property described in the application. If the attorney's opinion or title commitment discloses additional holders or owners of such mortgages, judgments, liens, easements, contracts or agreements, not party to the application, the Board may require them to join in and approve it. The title commitment or opinion shall be completed within thirty days prior to the application submission date. One copy of affidavit and certified list of the names, addresses and the corresponding Parcel Identification Number assigned by the Weld County Assessor of the owners of property (the surface estate) within 500 feet of property being considered. This list shall be compiled from the records of the Weld County Assessor, the Weld County Website, www.co.weld.co.us or a person qualified to do the task, and shall be current as of a date no more than thirty days prior to the date the application is submitted to the Department of Planning Services. REQUIRED SUPPLEMENTAL INFORMATION The applicant shall respond in writing to the following. Any information not included may be required upon review of the application by the Department of Planning Services and referral agencies. Description of the types of uses proposed for the subdivision Summary of any concerns identified during the preliminary plan application process with an explanation of how the concerns will be addressed or resolved Total number of lots proposed Description of the subdivision circulation system, including sidewalk width, road width, type and depth of road surface, curb and gutter, valley pan, width and depth of borrow ditches and vehicle parking arrangement Statement indicating if on street parking will be permitted within the proposed subdivision Statement describing the ownership, function and maintenance of any school site, open space or park within the proposed subdivision If the applicant is to dedicate land for schools, roads, parks or other public purposes, a letter of intent from the appropriate public agency stating that it will accept the lands to be dedicated Description of the proposed water system accompanied by an estimate of the total gallons per day required to serve the subdivision One copy of a water supply resource report. The report will contain written evidence that a water supply of sufficient quality, quantity and dependability will be available to serve the proposed subdivision. Such evidence may include, but shall not be limited to, the following: evidence of ownership or use of existing water rights; historic use and estimated yield of claimed water rights; amenability of existing rights to a change in use; and evidence that a public or private water supply is available. The amount of water available for use within the subdivision, feasibility of extending services and evidence concerning potability of the water supply for the proposed subdivision shall be identified. Copy of a contract or some tangible guarantee providing for a common water supply if water is required to be supplied by a water district, municipality or other agency Description of the proposed sewer system. The description shall include an estimate of the total number of gallons per day of sewage to be treated by public sewer or the suitability of another means of disposal if public sewer is not required. Copy of a contract or other tangible guarantee providing for adequate sewage treatment by a public sewage treatment agency if public sewage treatment is required Statement explaining how recommendations of the Colorado Geological Survey will be met List of any covenants, grants of easement and restrictions imposed upon any land, buildings and structures within the proposed subdivision Copy of a Colorado Department of Transportation Access Permit if a new street intersects with a State highway If applicable, a copy of an agreement signed by the applicant and representative of the irrigation ditch company. The agreement shall specify an agreed upon treatment of the ditch in accordance with Section 24-6-40 of the Weld County Code. Proof of an existing easement or dedicated right-of-way when it is contiguous to an easement or right-ofway of the proposed subdivision One copy of a proposed subdivision improvements agreement executed by the applicant. The agreement forms are available at www.co.weld.co.us. The agreement shall be made in accordance with the County policy on collateral for improvements.

If applicable, one copy of an off-site road improvements agreement executed by the applicant. The agreement shall be in accordance with Section 24-9-20 of the Weld County Code. One copy of a drainage report prepared in compliance with the requirements of Sections 24-7-120 and 24-7-130 of the Weld County Code One copy of an erosion control report if required by the Department of Public Works Subdivision road plans prepared by a professional engineer licensed to work in the State. The road plans shall be dated and bear the signature and seal of the engineer. The road plans shall include the following minimum data: 1) Plans and profiles of all roads to be improved 2) A typical cross-section of applicable roads, culverts and bridges 3) Typical road section, including pavement design supported by soil reports, test results and computations 4) Typical or specific details of road intersections and cul-de-sacs 5) A complete estimate of costs 6) Any additional information required by the Department of Public Works Title commitment or a title opinion covering all public dedications Warranty deed, if required, deeding to the appropriate entity any lands to be used for the benefit of the public or owners and future owners of the subdivision Geologic Maps and Investigation Reports regarding area suitability for the proposed development (application requirements attached) Colorado Geological Survey fee Investigation fee, if required The applicant shall submit 1 packet containing one original of all of the above for a 7 day completeness review. The packet shall be bound with either paper clips or binder clips (no substitutes). Applications bound in any other fashion may be returned to the applicant without review.

Fees are set by the Board of County Commissioners by Resolution. County Staff is not authorized to negotiate fees and cannot, under any circumstance, change, reduce, or amend the fees set forth in the Weld County Planning Fee Schedule. PURPOSE * The application shall be referred to any municipality or County governing body within a three mile radius of the parcel under consideration for a PUD. * Colorado Geological Survey Application Fees: All applications for resubdivision for redesign, addition or vacation will be submitted to the Colorado Geological Survey for review along with the required geologic maps and investigation reports. The following fee shall be collected by the Department of Planning Services at the time of application. As the fee will be forwarded, please make checks payable to the Colorado Geological Survey. Major replats of existing approved subdivisions: Cost = $485.00 Although most reviews will fall within the estimated time and costs built into the listed standard charges, it is necessary, in fairness to all users, to provide for those cases that will incur excess review costs. The Colorado Geological Survey will contact the local planner if it is evident that additional review costs will be requested. The applicant is responsible for all fees associated with the geological review. Excess time or travel charges will be made for those expenditures in excess of the normal range of review time (maximum 6.75 hours), or for extensive travel related to a particular case. * An additional investigation fee shall be added to the cost of the permit application when specific land, uses, buildings, manufactured homes, mobile homes, and structures that require a permit by the Weld County Code are located, moved, operated, or constructed prior to obtaining a permit. The investigation fee shall be fifty percent of the fee, established by separate action by the Board of County Commissioners for landuse applications. The payment of such investigation fee shall not relieve any persons from fully complying with the requirements of the Weld County Code, nor from any other penalties. * Site Plan Review applications may also be required for commercial, industrial and/or residential R-2, R-3 and R-4 districts following completion of the PUD process.

DRAINAGE REPORT REVIEW CHECKLIST Project Name: The purpose of this checklist is to provide the applicant s Engineer a basic list of items that County Staff will review in regards to a drainage report. The drainage design shall meet the requirements of the Weld County Code and commonly accepted engineering practices and methodologies. A detention pond design (or other stormwater mitigation design) is appropriate for projects which have a potential to adversely affect downstream neighbors and public rights-of-way from changes in stormwater runoff as a result of the development project. The design engineer s role is to ensure adjacent property owners are not adversely affected by stormwater runoff created by development of the applicant s property.
