Operation & Maintenance Inspection Report Month of July 2015 Project: - Design, Engineering, Construction, Development, Finance, Operation and Maintenance of Jaipur to Deoli section of NH 12 (Km 18.700 to Km 165.000) in the state of Rajasthan under NHDP Phase III. National Highway Authority of India under NHDP Phase-III has awarded the project for the development of the Highway Project in Section of National Highway 12, which consist of four laning of Jaipur to Deoli from Km 18+700 to Km 165+000 on Design, Built, finance, Operate and Transfer basis to the Concessionaire, M/s IRB Jaipur Deoli Tollway Pvt. Ltd. The Concessionaire M/s IRB JAIPUR DEOLI TOLLWAY PVT LTD. is executing the above project in the name of EPC contractor M/s IRB Infrastructure Developer Ltd. The Concessionaire has appointed M/s Stup Consultants Pvt. Ltd. as the Project Management Consultant (PMC/Engineer) for the supervision of construction of the entire project. This project starts from Ch. 18.660 Km (Design Ch. 0+000) and ends at Ch. 165+000 km (design Ch 148+975) of NH-12. National Highway authority has appointed M/s Architect, Engineers & Consultants Pvt. Ltd. as Independent Engineer in accordance to the Concessionaire Agreement, Schedule P signed between the Authority and the Concessionaire. The term of reference for independent engineer is well defined in Schedule Q of CA. Project Highway has been declared provisionally completed -1 and put on commercial use since 27.08.2013 at 23.44 Pm after successful completion of length more than 75% i.e. 119.748 Km of total length i.e. 148.924 KM Project Highway has been declared provisionally completed -2 and put on commercial use since 20.05.2014 after successful completion of 25.216 Km of total length i.e. 148.924 KM Project Highway has been declared provisionally completed and put on commercial use since 20.05.2014 after successful completion of 144.964 Km of total length i.e. 148.924 KM
INDEPENDENT ENGINEER S MONTHLY INSPECTION FOR O & M For the Month of July 2015 Sl. No Description Remarks of Independent Engineer 1 2 Overview of the Status Quality and Safety Project Highway NH-12 from Km 18+700 (Design Ch. 0+000) to Km 165+000 (Design Ch. 148+924) has been developed by NHAI to four lane on DBFOT basis through Concessionaire. Project Highway has been provisionally commissioned for commercial use on 20 th May-2014 for the length of 144.964 Km from total length of 148.924 Km of Project Highway. Concessionaire have been authorized to collect the Toll for the completed length of highway i.e. 144.964 Km at Two Numbers of Toll Plaza located at (i) Km. 30+500 (Design Ch. 12.100) for the length of 78.460 Km and (ii) Km. 105+000 (Design Ch. 90.450) for the length of 66.500 Km as per approved Fee Notification of Authority in accordance to Schedule-R from the user of highway at the rate described for various categories. Concessionaire has to operate and maintain the completed length of the project highway in accordance to the Schedule-K along with completion of the balance length of Km 3.96. Quality During O&M During the Operation Period, the Concessionaire shall operate and maintain the Project Highway in accordance with this Agreement and obligations of the Concessionaire hereunder shall include: Permitting safe, smooth and uninterrupted flow of traffic on the Project Highway Collecting and appropriating the Fee Minimising disruption to traffic in the event of accidents or other incidents affecting the safety and use of the Project Highway by providing a rapid and effective response and maintaining liaison with emergency services of the State
The Concessionaire shall keep a copy of every FIR recorded by the Police with respect to any accident occurring on the Project Highway. In addition, the Concessionaire shall also collect data for all cases of accidents not recorded by the Police but where a vehicle rolled over or had to be towed away. The information so collected shall be summarised in the form prescribed by IRC/ MORTH for this purpose. The Concessionaire shall also record the exact location of each Carrying out periodic preventive maintenance of the Project Highway Undertaking routine maintenance including prompt repairs of potholes, cracks, joints, drains, embankments, structures, pavement markings, lighting, road signs and other traffic control devices Undertaking major maintenance such as resurfacing of pavements, repairs to structures, and repairs and refurbishment of tolling system and other equipment Preventing, with the assistance of concerned law enforcement agencies, any unauthorised use of the Project Highway Protection of the environment and provision of equipment and materials therefore Operation and maintenance of all communication, control and administrative systems necessary for the efficient operation of the Project Highway Maintaining a public relations unit to interface with and attend to suggestions from the Users, government agencies, media and other agencies. Safety During O&M The Concessionaire shall ensure safe conditions for the Users, and in the event of unsafe conditions, lane closures, diversions, vehicle breakdowns and accidents, it shall follow the relevant operating procedures including the setting up of temporary traffic cones and lights, and removal of obstruction and debris without delay The Concessionaire s responsibility for rescue operations on the Project Highway shall be limited to an initial response to any particular incident until such time that the competent authority takes charge and shall include prompt removal of vehicles or debris or any other obstruction, which may endanger or interrupt the smooth flow of traffic. The Concessionaire shall develop, implement and administer a surveillance and safety programme for Users, including correction of safety violations and deficiencies and all other actions necessary to provide a safe environment in accordance with this Agreement The Concessionaire shall establish a Highway Safety Management Unit (the HSMU ) to be functional on and after COD, and designate one of its officers to be in-charge of the HSMU. Such officer shall have specialist knowledge and training in road safety and traffic engineering by having attended a course conducted by a reputed organisation on the subject.

The Concessionaire shall submit to the Authority before the 31st (thirty first) May of each year, an annual report (in ten copies) containing, without limitation, a detailed listing and analysis of all accidents of the preceding Accounting Year and the measures taken by the Concessionaire pursuant to the provisions of Paragraph 6.1 of this Schedule-L for averting or minimising such accidents in future. 3 Conformity with Maintenance and Safety Requirements In addition of the above, the Concessionaire has to be complete All items in the Punch List as Annex-A shall be completed by the Concessionaire within 90 (ninety) days of the date of issue of the Provisional Certificate. In Confirmation of the above, Independent Engineer has inspected the Project Highway on 07.08.2015 and observed followings in respect of maintenance and safety Observation of Maintenance The traffic flow is smooth and uninterrupted in 144.964 Km and Concessionaire has provided proper diversions and maintenance of existing highway for the balance section of highway. Concessionaire is collecting appropriate fee at Toll Plazas and submitting monthly fee statement to the Authority and not submitted to IE as informed by the concessionaire. Concessionaire has mobilizes 2 nos. of Cranes to remove disruption to the traffic in the event of the accident for rapid response, and provided Toll Free Helpline Number 18001806782 for the emergency services. Concessionaire has to start periodic preventive maintenance of the project highway in accordance to the Maintenance Manual. Concessionaire is doing routine maintenance for cleaning of the highway, restoration of rain cuts in shoulder and painting of Kerbs. Concessionaire has prevented unauthorized and overloaded vehicles to use of the project highway with assistance of concern law enforcement agencies. Unauthorized median opening is being closed by Concessionaire at ch. 126+800, Ch. 134+600, Ch. 135+450, Chg. 142+450, Ch. 145+700 & Ch. 148+520 work is under progress. Plantation work founded in progress but very slow. Instructed to concessionaire to provide more team for plantation to speed up the work. Rain cut were found in shoulder portion (MCW & Service road) at several locations and it was being maintained by Concessionaire. Road marking were observed faded at some stretches specially at overlay portion which has to be done. Concessionaire has done avenue plantation of 34567 along the highway to the protection of the environment and need to be maintain and watering to grow the trees. Concessionaire has provided Toll Free Helpline Number as above for
4 5 Lapses during O & M Defects during O & M operation of the project highway. Concessionaire has established public relation unit and not informed to the Independent Engineer. Safety During O&M Concessionaire is enforcing safety measure during operation by providing traffic cone in the event of accident. Concessionaire has to submit diversion plan to lane closures during maintenance. Concessionaire is doing rescue operation for the vehicles, which may endanger of interrupt the smooth flow of the traffic. Concessionaire has developed and implemented surveillance and safety programme for users and informed to the Independent Engineer. Concessionaire has established Highway Safety Management Unit (HSMU). Concessionaire has submitted weekly accident report and the information submitted to the Authority. Concessionaire has completed work of the Punch List within 90 days of the date of issue of Provisional Certificate. Concessionaire has to maintain the project highway in accordance to the Schedule-K of the Concession Agreement and IE is monitoring lapses and informed to Concessionaire. Concessionaire has to maintain the project highway for provisionally completed section and the balance section to be completed and all the defects during the O&M has to be rectified in accordance to the CA and Maintenance Manual. 6 Deficiencies during O & M It is being observed during inspection that concessionaire is maintaining median tree plantation, Concessionaire has enforced safe guard to the protection of the ROW and encroachment removal with association of local administration is in progress., 7 8 Final Remarks: Pending Submittal: Concessionaire has obtained CRS for construction of ROB. Concessionaire has completed ROB-1 at Shivdaspura both side and ROB-2 at Newai both side and opened to traffic. 1) As Built Drawing of Highway. Concessionaire has submitted As Built Drawing of Structures and reviewed and signed by IE.As built drawing has been submitted to NHAI.
