Kings Creek Village Townhouse Association, Inc. APPLICATION FOR RENTALS Dear Owner and Prospective Tenant: Attached is an application and a Lease Rider. Please complete the forms answering as much information as possible. The application and rider, along with a copy of the lease and checks must be submitted to the Townhouse management office at least 15 days prior to occupancy. 1. The application must be filled out completely by the prospective tenant or this may result in a delay in the approval process. Husband and wife or parent/dependent child may submit only one application. Otherwise, a separate application must be submitted for each tenant. 2. A fully executed copy of the lease and the Association Lease Rider must accompany the application. 3. All assessments of the owner must be current. 4. The applicant must include a $60.00 processing fee made payable to Lynx Property Services for each applicant, other than husband/wife. This is a nonrefundable fee. Acceptance of processing fee does not in any way, constitute approval. Only Money order or cashier check will be accepted. 5. Occupancy before approval is strictly prohibited. 6. A check for the $300.00 Common Area Damage (CAD) deposit must be submitted by the townhouse owner with the application, made payable to the King Creek Village Townhouse Association. No CAD deposits will be accepted by KCVTA from the tenant. Any damage by the tenant, their children, guests or invitees shall be deducted from the Owners CAD deposit. This deposit will be kept in a noninterest bearing account. Within (15) fifteen days after the expiration date of the lease and the vacating of the premises by the tenant, KCVTA shall give written notice, to the owners, of its claim against the CAD, if any. 7. Applicant will receive a copy of the Kings Creek Village Association, Inc. (Master) Rules and the Townhouse Rules and Regulations. The applicant and owner must both sign the Lease Rider which acknowledges receipt of these items. 8. Vehicle information on the application must be completed for every car owned by the prospective tenant(s). This information will need to be updated if a new car is purchased. Commercial vehicles, boats, trailers, motor homes, campers, recreational vehicles, etc. are not allowed to be parked on the property, in the parking spaces or guest parking areas at any time. There are only two (2) assigned parking spaces available per townhouse. 9. The lease term shall be for one year only and is renewable on a yearly basis. Fifteen days prior to the end of the lease, the Owner must forward any renewal or extension of the lease to the management office. 10. The pet information on the application must be completed. Cats and dogs are not permitted to run free in the community. Dogs must be walked on a leash at all times when outdoors and picked up after. 11. Use of townhouse is for single family residence only. No multi-family occupancy is permitted. Resident regulations: 2 bedroom townhouse - no more than 4 residents 3 bedroom townhouse - no more than 5 residents 12. The home may not be used for business or commercial purposes. 13. Moving of furniture IN or OUT of a townhouse is only permitted from: 8:00 am to 8;00 pm daily.

Kings Creek Vi"age Townhouse Association, Inc. RIDER TO LEASEOF TOWNHOUSE UNITS 1. Lessee does hereby acknowledge receipt of a copy of the Kings Creek Village Association, Inc. Rules, the Kings Creek Village Townhouse Association, Inc. Rules and regulations, and the By-laws of Kings Creek Village Townhouse Association, Inc. (the Association) and agrees to be bound by the provisions contained therein. Lesseefurther agrees to protect the common areas of Kings Creek Village and not to permit or commit waste thereon or damage thereto or to use the common areas or his or her unit in a manner which would constitute a nuisance to other unit owners. Lessee acknowledges that any violation of the provisions of this rider or the provisions contained in the Florida Homeowners Association Statute (Sec. 617.301, et. Seq.Oas same may from time to time be amended, shall constitute a material default under the terms of this lease. 2. Lessor agrees that in the event of such default by Lessee, the Lessor shall immediately, at his own expense, take all steps necessary, including legal action to secure the removal of the Lesseefrom the premises. If the Lessorfails to act within ten (10) days after receiving written notice of default from the Association then, in such event, Kings Creek Village Townhouse, Inc. is hereby authorized to commence legal action for the removal of the Lesseefrom the premises. For purposes hereof, the Lessordoes irrevocably appoint, for the duration of this Lease,the Association as its Agent and Attorney-in-Fact, for the purpose of enforcing the provisions contained in this Rider, and in connection therewith does hereby authorize the Association to take any and all legal action which it may reasonably deem to be necessary, including an action for removal of the Lesseefrom the premises, in order to enforce the provisions contained herein. 3. In the event that legal action is commenced by the Association against either the Lessee or Lessor, provided that the Association prevails in said action, the Lessee and Lessor individually and collectively shall both be liable for the costs and attorney's fees incurred by the Association in connection with such action. The Lesse and Lessoreach agree to indemnify, save and hold harmless the Association from all costs and expenses and damages of any type whatsoever which the Association may reasonably incur as a result of or arising out of the actions of the Lessee in connection with this tenancy, and it is understood that both the Lessor and Lessee shall be liable for any damages caused to the Association property or common elements by the Lessee, his guests or invitees. It is, however, understood that notwithstanding the foregoing, the Lessor shall only be liable for actions of the Lessee,his guests and/or invitees to the same extent that the Lessorwould be if the Lessor personally undertook such action. 4. This lease is not assignable nor shall any sublease of the demised premises be permitted. 5. At the request of the Association, Lessee shall provide Association with credit references, employment references and such other information as may be reasonably necessaryto assure Lessee'sfinancial responsibility. 6. The Covenants and conditions herein contained and contained in the Declaration of Covenants and restrictions for Kings Creek, the By-laws, Articles of Incorporation and Rules and Regulations of Kings Creek Village Association, Inc. and Kings Creek Page1 of 2

Village Townhouse Association, Inc. shall apply to and bind the heirs, legal representatives and assigns of the parties hereto. 7. It is specifically understood that the said premises are leased to the Lesse and his family consisting of adults and children. 8. It is understood and agreed that any approval received from the Association in connection with this Lease shall only apply to the original Leaseterm and shall not be considered an approval of any option to renew the Lease, regardless of whether such option is contained in the Lease. Lessor or Lesse shall give Association written notice of their intent to renew or extend the Lease forty-five (45) days prior to its expiration. 9. In all cases hereinabove set forth wherein both the Lessorand Lesseeare liable, said liability shall be considered joint and several. 10. In the event that any of the terms of the Lease to which this Rider is appended are inconsistent with the terms contained herein, the terms of this Rider shall govern. 11. Lesseeacknowledges having read and understood the Leaseand this Rider.
