A Qualified person s statements and certifications As the qualified person, I certify that: A phase one environmental site assessment of the record of site condition property, which includes the evaluation of the information gathered from a records review, site reconnaissance, interviews, a report and any updates required, has been conducted in accordance with the regulation by or under the supervision of a qualified person as required by the regulation. As of 2015/11/12, no phase two environmental site assessment is required by the regulation for the record of site condition property and based on the phase one environmental site assessment for the record site condition property, in my opinion, it is not necessary for any other reason to conduct a phase two environmental site assessment for the record of site condition property. As of 2015/11/12, in my opinion, based on the phase one environmental site assessment, there is no evidence of any contaminants in the soil, ground water or sediment on, in or under the record of site condition property that, if the record of site condition property were put to any of the types of property uses listed in subsection 1 (2) of the regulation, are likely to interfere with any of those types of property uses. I am a qualified person and have the qualifications required by section 5 of the regulation. I have in place an insurance policy that satisfies the requirements of section 7 of the regulation. I acknowledge that the record of site condition will be submitted for filing in the Environmental Site Registry, that records of site condition that are filed in the Registry are available for examination by the public and that the Registry contains a notice advising users of the Registry who have dealings with any property to consider conducting their own due diligence with respect to the environmental condition of the property, in addition to reviewing information in the Registry. Phase I Conceptual Site Model The Phase I property is located on the northeast corner of Belsize Drive and Thurloe Avenue, Toronto, Ontario. It has an area of approximately 1217 square metres (0.1217 hectares). At the time of the site reconnaissance, the site was vacant and excavated. Shoring had been erected adjacent to the north and east property boundaries. Following a review of the historical documentation, previous investigations and site reconnaissance during the Phase I ESA, it is possible to formulate an initial conceptual site model (CSM). The CSM is a simplification of reality, which aims to provide a description and assessment of any areas where potentially contaminating activity on or potentially affecting the Phase I property has occurred, and any contaminants of potential concern. A CSM was developed based on the findings of the Phase I investigation, completed in accordance with O.Reg. 153/04, as shown on Figure 4. Since no potentially contaminating activities (PCA) were identified for the Phase I property, no areas of potential environmental concern (APEC) were identified. Based on historical aerial photographs, chain of title information and the records review, it appears the Phase I property was first developed circa 1913, when a church was built on the property. An addition to the church was subsequently built in 1956. The church was demolished in 2013 and the Phase I property remained vacant until the construction that is being undertaken at the Phase I property began in August 2015. The land use within the immediate vicinity of the Phase I property appears to be all residential. Commercial/residential properties are located along Mount Pleasant Road to the east. Based on fire insurance plans, the surrounding residential properties were developed around the same time as the Phase I property in the 1910s. Light commercial properties along Mount Pleasant Road, which was constructed in the 1910s, were developed in the 1930s. At the time of the Phase I site reconnaissance, all fill materials had been removed from the Phase I property. There was no evidence that the former presence of fill adversely affected the native soil at the Phase I property. Therefore, it is the opinion of the Qualified Person that the former presence of fill does not constitute a PCA. Thus, there are no PCA originating from the Phase I property. Current and historical gasoline service stations, service garages, and dry cleaners were identified to be present within the Phase I study area. The current and historical gasoline service stations were located downgradient or cross gradient from the Phase I property. One gasoline service station had been converted from a gas station to a residential property and a Record of Site Condition had been filed. One current service garage is present within the Phase I study area, however it is downgradient of the Phase I property. Auto parts were historically sold from a property located cross-gradient from the Phase I property, however there is no evidence that a service garage operated at that property. All of the current and 1

historical dry cleaners were located downgradient or cross gradient from the Phase I property and no evidence could be found that dry cleaning services actually occurred at any of these properties. Therefore, it is the opinion of the Qualified Person that none of the current and historical gasoline service stations, service garages, and dry cleaners are contributors of PCA to the Phase I property. Several historical underground storage tanks used for storage of fuel oil or gasoline were found to be present within the Phase I study area. The fuel oil tanks would have had small volumes as they were installed to heat single buildings on each property. Also, fuel oil typically has a strong odour, and no odour or staining was identified in the test hole logs from a previous geotechnical investigation or during the current excavation work at the Phase I property. The closest underground tank was located immediately upgradient of the Phase I property at 4 De Savery Crescent. Since this property was redeveloped in 2013, the tank is no longer present. Therefore, it is the opinion of the Qualified Person that none of the historical underground storage tanks are contributors of PCA to the Phase I property. Leaks and spills identified in the Phase I study area include several natural gas leaks and two transformer oil spills. Due to the nature of natural gas, once a gas leak has occurred it would dissipate in the surrounding air and is not likely to adversely affect the Phase I property. The transformer oil spills were of small quantity (5 to 22 litres). One spill was located cross gradient and the location of the other is downgradient. Therefore, it is the opinion of the Qualified Person that none of the historical natural gas leaks and transformer oil spills are contributors of PCA to the Phase I property. Therefore, there are no PCA originating from any property in the Phase I study area. As such, there are no APEC on the Phase I property. The nearest surface water body is a tributary of the Don River, located approximately 1.2 kilometres southeast of the Phase I property. The tributary flows into Lake Ontario, approximately 6.5 kilometres south of the Phase I property. The Phase I property is not located on, or in close proximity to, an Area of Natural and Scientific Interest (ANSI), according to the Ministry of Natural Resources and Forestry Biodiversity Explorer website. There are no ANSI located within the Phase I study area. According to the MOECC well records, there are six well records within the Phase I study area. No wells were found on the Phase I property. Five records pertain to monitoring wells installed at 566 Mount Pleasant Road and the last record pertains to an abandoned monitoring well located at 54 Berwick Avenue. There are no drinking water wells located within the Phase I study area. The depth to the ground water table is approximately 3.3 to 5 mbgs, based on previous ground water measurements taken at the Phase I property. The site is located in the physiographic region known as the drumlinized Till Plains of the South Slope. The surface soils are predominantly composed of sandy silt. The underlying bedrock geology comprises shale, limestone, dolostone and siltstone of the Georgian 2

Bay/Blue Mountain/Billings formations of the Upper Ordovician period. The bedrock is approximately 65 mbgs. The location and depth of underground natural gas lines, water lines, sanitary and storm sewers, telephone lines and street light lines may affect contaminant transport. However, the Phase I property was excavated at the time of the site reconnaissance and no underground utilities were present. The investigation undertaken by exp with respect to this report and any conclusions or recommendations made in this report reflect exp s judgement based on the site conditions observed at the time of the site inspection on the date(s) set out in this report and on information available at the time of preparation of this report. Exp has confirmed neither the completeness nor the accuracy of the records that were provided by others; as such, this historical records review is identified as a potential source of uncertainty during the investigation. The conceptual site model is developed using multiple lines of evidence, searches and source information to make every reasonable attempt to ensure that findings of environmental significance are captured. Any uncertainty or absence of information in the records review, interviews, and site reconnaissance components of the Phase I investigation are not anticipated to materially affect the validity of the CSM of Phase I conclusions. 3
