The District Agenda is comprised of five different sections: The meeting will begin promptly at 11:00 AM with the first section, which is called Audience Comments. The Audience Comments portion of the agenda is when individuals may comment on matters that concern the District. Each individual is limited to three (3) minutes for such comment. The Board of Supervisors or Staff is not obligated to provide a response until sufficient time for research or action is warranted. IF THE COMMENT IS MAINTENANCE RELATED ITEM, THESE ITEMS WILL NEED TO BE ADDRESSED BY THE DISTRICT MANAGER OUTSIDE THE CONTEXT OF THIS MEETING. The second section is called Business Administration. The Business Administration section contains items that require the review and approval of the District Board of Supervisors as a normal course of business. The third section is called Business Items. The business items section contains items for approval by the District Board of Supervisors that may require discussion, motion and votes on an item-by-item basis. Occasionally, certain items for decision within this section are required by Florida Statute to be held as a Public Hearing. During the Public Hearing portion of the agenda item, the public will be permitted to provide one comment on the issue, prior to the Board of Supervisors discussion, motion and vote. Agendas can be reviewed by contacting the Manager s office at (813) 533-2950 at least seven (7) days in advance of the scheduled meeting. Requests to address items that are not on this agenda must be submitted in writing with an explanation to the District Manager at least fourteen (14) days prior to the date of the meeting and will be heard under Public Comments. The fourth section is called Staff Reports. This section allows the District Manager, Engineer, and Attorney to update the Board of Supervisors on any pending issues that are being researched for Board action. The final section is called Supervisor Requests. This is the section in which the Supervisors may request Staff to prepare certain items in an effort to meet residential needs. Public workshop sessions may be advertised and held in an effort to provide informational services. These sessions allow staff or consultants to discuss a policy or business matter in a more informal manner and allow for lengthy presentations prior to scheduling the item for approval. Typically no motions or votes are made during these sessions. Pursuant to provisions of the Americans with Disabilities Act, any person requiring special accommodations to participate in this meeting is asked to advise the District Office at (813) 533-2950, at least forty-eight (48) hours before the meeting. If you are hearing or speech impaired, please contact the Florida Relay Service at 7-1-1, who can aid you in contacting the District Office. Any person who decides to appeal any decision made by the Board with respect to any matter considered at the meeting is advised that this same person will need a record of the proceedings and that accordingly, the person may need to ensure that a verbatim record of the proceedings is made, including the testimony and evidence upon which the appeal is to be based.

District Counsel Ms. Warren announced that a Sunshine Laws and Public Records update will be given at the next meeting. B. District Engineer Ms. Stewart gave an update regarding sump #99 and discussed underdrain connections and the sump on the corner of Tahoe. She also spoke about District sump management. C. District Manager Mr. Phillips announced that the next regularly scheduled meeting would be held on

FOURTH ORDER OF BUSINESS Ratification of Fiscal Year 2017/2018 Insurance Renewal Mr. Phillips presented the executed application for Fiscal Year 2017/2018 insurance renewal to the Board for ratification. On a motion by Mr. Temple, seconded by Ms. Holley, with all in favor, the Board of Supervisors ratified the application for Fiscal Year 2017/2018 insurance renewal for the Bridgewater Community Development District. FIFTH ORDER OF BUSINESS Presentation of 2016/2017 Fiscal Year- End Unaudited Financial Statements Mr. Phillips presented the 2016/2017 Fiscal Year-End Unaudited Financial Statements to the Board. Discussion ensued. SIXTH ORDER OF BUSINESS Consideration of Minutes of the Board of Supervisors Regular Meeting Held on July 13, 2017 On a motion by Mr. Temple, seconded by Mr. Warren, with all in favor, the Board of Supervisors approved the Minutes of the Board of Supervisors Regular Meeting held on July 13, 2017 as presented for the Bridgewater Community Development District. SEVENTH ORDER OF BUSINESS Consideration of Operations & Maintenance Expenditures for July, August, and September 2017 On a motion by Mr. Rooney, seconded by Mr. Warren, with all in favor, the Board of Supervisors approved the Operations & Maintenance expenditures for July 2017 ($11,405.38), August 2017 ($15,933.47) and September 2017 ($22,427.64) for the Bridgewater Community Development District. EIGHTH ORDER OF BUSINESS Consideration of Resolution 2018-01, Adopting Fiscal Year 2017/2018 Meeting Schedule Mr. Phillips presented Resolution 2018-01 to the Board for consideration. On Motion by Mr. Temple, seconded by Mr. Rooney, with all in favor, the Board of Supervisors adopted Resolution 2018-01, Adopting the Fiscal Year 2017/2018 Meeting Schedule, for the Bridgewater Community Development District.

On Motion by Ms. Holley, seconded by Mr. Warren, with all in favor, the Board of Supervisors adopted Resolution 2018-02, Designating Primary Administrative Office and Headquarters of the District, for the Bridgewater Community Development District. TENTH ORDER OF BUSINESS Supervisor Requests Ms. Holley inquired about the distribution list and requested that her contact information be revised. ELEVENTH ORDER OF BUSINESS Adjournment On a motion by Mr. Temple, seconded by Mr. Warren, with all in favor, the Board of Supervisors adjourned the meeting at 11:42 AM for the Bridgewater Community Development District. CONTRACT AGREEMENT This Agreement made and entered into on Wednesday, December 06, 2017 by and between the Bridgewater Community Development District, a local unit of special purpose government of the State of Florida hereinafter referred to as the 'Special District', and Marsha M. Faux, Polk County Property Appraiser, a Constitutional Officer of the State of Florida, whose address is 255 North Wilson Ave., Bartow, FL 33830, hereinafter referred to as the 'Property Appraiser'. 1. Section 197.3632 Florida Statutes, provides that special assessments of non-ad valorem taxes levied by the Special District may be included in the assessment rolls of the County and collected in conjunction with ad valorem taxes as assessed by the Property Appraiser. Pursuant to that option, the Property Appraiser and the Special District shall enter into an agreement providing for reimbursement to the Property Appraiser of administrative costs, including costs of inception and maintenance, incurred as a result of such inclusion. 2. The parties herein agree that, for the 2018 tax year assessment roll, the Property Appraiser will include on the assessment rolls such special assessments as are certified to her by the Bridgewater Community Development District. 3. The term of this Agreement shall commence on January 1, 2018 and shall run until December 31, 2018, the date of signature by the parties notwithstanding. This Agreement shall not automatically renew. 4. The Special District shall meet all relevant requirements of Section 197.3632 & 190.021 Florida Statutes. 5. The Special District shall furnish the Property Appraiser with up-to-date data concerning its boundaries and proposed assessments, and other information as requested by the Property Appraiser to facilitate in administering the non-ad valorem assessment in question. Specifically, the Special District shall provide proposed assessments no later than Friday, July 20, 2018, for inclusion on the 2018 TRIM notice which is statutorily mailed within 55 days of July 1. The Special District s assessments shall, as far as practicable, be uniform (e.g. one uniform assessment for maintenance, etc.) to facilitate the making of the assessments by the mass data techniques utilized by the Property Appraiser. 6. The Special District shall certify to the Property Appraiser the Special District s annual installment and levy no later than Friday, September 14, 2018. The Property Appraiser shall, using the information provided by the Special District, place the Special District s non ad-valorem special assessments on properties within the district for inclusion on the 2018 tax roll. 7. The Property Appraiser shall be compensated by the Special District for the administrative costs incurred in carrying out this Agreement at the rate of 1% of the amount levied on the TRIM Notice. The Property Appraiser will require payment on or before Friday, September 14, 2018 for processing within the Property Appraiser budget year (October 1st September 30th). 8. If the actual costs of performing the services under this agreement exceed the compensation provided for in Paragraph 7, the amount of compensation shall be the actual costs of performing the services under this agreement. 9. If tax roll corrections are requested by the Special District, the Property Appraiser shall be compensated by the Special District for the administrative costs incurred at the rate of $5.00 for each tax roll correction exceeding ten (10) corrections per tax year. The Special District shall indemnify and hold harmless, to the extent permitted by Florida law and without waiving its right of any applicable sovereign immunity, the Property Appraiser and all respective officers, employees, agents and instrumentalities from any and all liability, losses or damages, including attorneys' fees and costs of defense, which the Property Appraiser and all respective officers, employees, agents or instrumentalities may incur as a result of claims, demands, suits, causes of actions or proceedings of any kind or nature arising out of, relating to or resulting from the negligent or intentional acts or omissions of the Special District or its employees, agents, servants, partners, principals, or subcontractors arising out of, relating to, or resulting from the performance of the Agreement. The Special District shall pay all claims and losses in connection therewith and shall investigate and defend all claims, suits or actions of any kind or nature in the name of the Property Appraiser where applicable, including appellate proceedings, and shall pay all costs, judgments, and attorneys' fees which may issue thereon.
