GENERAL INFORMATION 1.1 Scope of the Project JKL Bahweting School PSA is seeking to vendor to purchase and install new carpet and cove base within 10 classrooms and 1 hallway. 1.2 Bid Submission Bid closing is September 24, 2018 at 2:00 PM EST at which time all received bids will be publically opened. All bids received after closing will automatically be rejected. Bids must be sealed and clearly marked RFP #20180910.1 and addressed to the attention of the Director of Business and Finance. 2.0 NEW CARPET JKL Bahweting School PSA is seeking to vendor to purchase and install new carpet and cove base within 10 classrooms and 1 hallway. 2.1 Carpet and Cove Base Carpet squares will be installed in 10 classrooms and adjoining hallway within the upper elementary wing. Both classrooms and hallway will have matching carpet. Hallways will have YELLOW cove base. Classrooms will have BLACK cove base. 2.2 Site Visit Dimensions and lengths of required materials will not be provided. An on-site visit is REQUIRED. Responding vendors will be responsible for determining appropriate amounts of materials and supplies to complete the project. A sample of the required carpet squares will be available during the on-site visit to determine the manufacturer and style that is required. 2.3 Installation Responding vendors will be responsible for providing labor to first remove the existing carpet and cove base; including moving furniture and items within the classrooms. Installation of the new carpet squares and cove base is required including moving furniture back to their original locations. Schedule for installation will be determined at a later date. However, it will occur during school breaks or times when school is not in session on a room by room basis if necessary. 3.0 BID REQUIREMENTS 3.1 Additional Bid Requirements All alternates, variations, and exceptions to any requirement must be clearly stated. The school reserves the right to consider proposals based on their relative merit, risk, and value to the school. The school reserves the right to cancel all or parts of this request for bid if it is in the best interest of the school. This bid request does not commit us to award a contract, to pay any costs associated with the preparation of a response, to procure or contract for services or equipment. Page 4
3.2 Rejection of Bids JKL Bahweting School reserves the right to accept or reject any or all bids, and to waive any minor discrepancies or technicalities in the bid or specifications when deemed to be in the best interest of the school. 3.3 Required Information in Bid Responses Vendor responses may be rejected for failing to in provide all required information/documentation! 3.3.1 Detailed Quotation A detailed quotation must be provided. The quotation should include all required costs. This must include pricing for materials and supplies and related labor costs for removal and installation. The school is a tax-exempt organization and documentation will be provided at time of purchase/ordering. 3.3.2 Contact Information Appendix Form A 3.3.3 Iran Economic Sanction Act Appendix Form B In compliance with Michigan s Iran Economic Sanctions Act (P.A. 517 of 2012), vendor is required to provide notarized documentation certifying that it is not an Iran linked business. Vendor submissions that do not include this documentation will be rejected without further review or consideration. 3.3.4 Relationship Disclosure Appendix Form C Vendor must submit a sworn, notarized statement disclosing any familial relationship that exists between the owner or any employee of the bidder and that of any member of the JKL Board of Education or the School Administrator. APPENDIX: FORM B AFFIDAVIT OF COMPLIANCE IRAN ECONIMIC SANCTIONS ACT The undersigned, the owner or authorized officer of the below named contractor (the Contractor ), pursuant to the compliance certification requirement provided in the JKL Bahweting School s (the School District ) Request for Proposals for JKL RFP #20180820.1 (the RFP ), hereby certifies, represents and warrants that the Contractor (including its officers, directors, and employees) is not an Iran linked business within the meaning of the Iran Economics Sanction Act, Michigan Public Act No. 517 of 2012 (the Act ), and that in the event Contractor is awarded a contract as a result of the aforementioned RFP, the Contractor will not become an Iran linked business at any time during the course of performing the Work or any services under the contract. The Contractor further acknowledges that any person who is found to have submitted a false certification is responsible for all civil penalty of not more than $250,000.00 or 2 times the amount of the contract or proposed contract for which the false certification was made, whichever is greater, the cost of the School District s investigation, and reasonable attorney fees, in addition to the fine. Moreover, any person who submitted a false certification shall be ineligible to bid on a request for proposal for three (3) years from the date that is determined that the person submitted the false certification.
