ASBESTOS SURVEY REPORT: Introduction: Jacobs have been instructed by the client to carry out a Type 3 Asbestos Full Access and Identification Survey (pre-demolition/ major refurbishment survey) in accordance with Health & Safety Executives Guidance Note MDHS100 (July 2001). The purpose is to locate and describe as far as is reasonably practicable all Asbestos Containing Materials (ACMs) in the building. Methodology: Type 3 Asbestos Full Access and Identification Surveys are likely to involve destructive inspection in order to gain access to all areas where the presence of asbestos is suspected. A full representative sampling programme is undertaken and analysis of ACMs is carried out in accordance with the laboratories UKAS Accredited Documented In - House Methods contained in the Jacobs Divisional Quality Manuals Volumes 3 and 4. The results of the sample analysis are recorded in the reports. Plans indicating the product types, extent, locations and positions of samples are provided. Photographs are included as directed by the client to illustrate the nature of the ACMs in situ. Recommendations: The purpose of the factual report is to allow the client to use the information as a basis for tendering the removal of ACMs prior to demolition or refurbishment. The report does not assess the condition of ACMs other than noting damage or debris present. Further interpretation and recommendations are only made at the request of the client. Health & Safety: Jacobs carries out work in accordance with the provisions of current Health & Safety Legislation & Guidance. Special reference is made to our Documented In-House Methods contained in the Jacobs Divisional Quality Manuals Volumes 3 and 4. Jacobs will not carry out destructive sampling where evidence indicates the presence of live services or where other serious Health and Safety issues arise. The client will be contacted in this event and where no satisfactory resolution can be made the materials will be presumed to contain asbestos in accordance with the provisions of MDHS 100. PAGE 2

ASBESTOS SURVEY REPORT: TYPE 3 JOB REF: LAB REF: DATE OF SURVEY: COMMENTS: This survey does not constitute a full type 3 FULL ACCESS SAMPLING AND IDENTIFICATION SURVEY, as the survey was restricted to the areas specified b the client prior to the survey. The qualified areas specified by the client for type 3 survey are as follows: The panels to the rear of the balcony wall at Dress Circle level. The timber cladding to the walls at Grand Circle level. After inspection it was apparent that the were no suspect asbestos containing materials in the above areas at the time of the survey, as a result of this no samples war taken at this time.
