REQUEST FOR PROPOSALS CONSTRUCTION MANAGEMENT SERVICES The Parish of Livingston is requesting Proposals for construction management services for the Livingston Parish Live Oak Retrofit project in Livingston Parish. The project consists of retrofitting the Live Oak Junior High School Auxiliary Gym and Main Gym, including the entire building envelope, roof, windows, and doors. The project also includes the installation of a built-in safe room in the corridor of an existing classroom building which will support 100 occupants. Improvements will mitigate wind hazards to protect against 110 mph winds. Request for Qualifications Livingston Parish, Louisiana PUBLIC NOTICE Livingston Parish is hereby requesting written submittals of a Statement of Qualifications from firms interested in providing Professional Engineering services to be performed at the direction of the Livingston Parish Government Office of Homeland Security & Emergency Preparedness (LOHSEP) for Improvements to the Haynes Settlement Water System. Description: Services required for this Project include those basic and supplemental services necessary to make improvements and extensions to the residential water system totaling approximately 2,900 linear feet (LF) within the established roadside ditch right-of-way. This project includes all valves, hydrants, meters, and piping to complete the project. Existing residences in the area are operating under individual private well supply systems. This project will establish water service tie-ins to these areas. Firms submitting a statement of qualifications must identify all firms that they expect to use to provide supplemental services. The facility is located at 30140 Eden Church Road, in Denham Springs, Livingston Parish. Upon the effective date of the final permit, the LPDES permit shall replace the previously issued LPDES permit. The principal discharge from this proposed source is made into Grays Creek via an unnamed ditch and Dixon Canal, waters of the state classified for primary contact recreation, secondary contact recreation, and propagation of fish and wildlife. Under the SIC Code 2899, the applicant proposes to discharge low contamination potential stormwater runoff from an existing chemical manufacturing facility. During the preparation of this permit, it has been determined that the discharge will have no adverse impact on the existing uses of the receiving waterbody. As with any discharge, however, some change in existing water quality may occur. This permit is being modified to change the wastewater description for Outfall 007 to include the discharge low contamination potential stormwater runoff from the new property including the railcar loading area and to change the STORET Code for Oil & Grease from 03582 to 00556 for Outfalls 002, 005, 006, and 007. In accordance with LAC 33:IX.3105.B.2, only those permit limitation and conditions pertaining to the draft modifications are open to public comment.
