The information represents the site conditions at the sampling points at the time of sampling only and the conditions between and beyond the sampling points may vary. As of 2012/02/08, in my opinion, based on the phase one environmental site assessment and the phase two environmental site assessment, and any confirmatory sampling, there is no evidence of any contaminants in the soil, ground water or sediment on, in or under the RSC property that would interfere with the type of property use to which the RSC property will be put, as specified in the RSC. Ground water sampling has been conducted in accordance with the regulation by or under the supervision of a qualified person as required by the regulation. As of 2012/02/08, in my opinion, based on the phase one and phase two environmental site assessments and any confirmatory sampling, the RSC property meets the applicable full depth generic site condition standards prescribed by section 36 of the regulation for all contaminants prescribed by the regulation in relation to the type of property use for which this RSC is filed, except for those contaminants (if any) specified in this RSC at Table 2, Maximum Compared to Standards Specified in a Risk Assessment. As of 2012/02/08, the maximum known concentration of each contaminant in soil, sediment and ground water at the RSC property for which sampling and analysis has been performed is specified in this RSC at Table 1, Maximum Compared to Applicable Site Condition Standards. I am a qualified person and have the qualifications required by section 5 of the regulation. I have in place an insurance policy that satisfies the requirements of section 7 of the regulation. I acknowledge that the RSC will be submitted for filing in the Environmental Site Registry, that records of site condition that are filed in the Registry are available for examination by the public and that the Registry contains a notice advising users of the Registry who have dealings with any property to consider conducting their own due diligence with respect to the environmental condition of the property, in addition to reviewing information in the Registry. The upper floor was rented to private individuals and to some professional offices. Between 1926 and 1958, a Hat shop operated from #1049 Gerrard. Afterwards, a coin-operated Laundromat operated from the site until at least 1963 but there is no evidence this operated as a dry cleaners. Commercial/ residential

In 1915, a dentist s office operated from#1049 likely under a lease. In 1905, #1045 is not listed in the City Directory. 1889-1949 Joseph Simpson PIN 21050-0157 (LT), west section. Fencing Supplies Retailer. No information was found on this company. Commercial/ Residential upper floors. No historical land title information found for this parcel in Toronto Land Tiles Office prior to 1889. Land sub-divided in 1889, but not developed. Commercial/residential Commercial/ residential The FIP s for 1913 show three separate buildings at 1049, 1055 and 1057 Gerrard St; the FIP for 1939 shows 3 buildings in the same location and again in June 1945; Historical Title search; Fire Insurance Plans; aerial photos; The fence company may be the pre-cursor to Simpson Fence Ltd., a large supplier of fencing products in Ontario with outlets in London and Chatham but this has not been confirmed. There is no evidence that manufacturing of fencing was conducted at the subject site and it is highly unlikely given the very limited space available for this type og manufacturing.

PHASE II CONCEPTUAL SITE MODEL Figure 1 below shows the Phase I conceptual site model using a 2002 Google Earth TM Image identifying the subject site within the red dotted line and the 250m area around the site within the orange dotted line. Each zone within the area is identified as commercial, industrial, residential, government, parkland or agricultural. Properties with potentially contaminating activities threatening the subject site are identified with a green letter and are discussed below. A - The subject property is referenced on the Phase I Conceptual model because it used to operate as funeral home and has a record of pathological wastes being removed from the site. Ontario Regulation 347 Waste Generators Summaries often identify products that could threaten the environment if leaked or spilled. 1059 Gerrard St E, the neighbour immediately east of the site, also now operates in the funeral business and has been producing pathological wastes since 2002. Proper removal is evidence of proper management of wastes and therefore is not of environmental concern. B - Anderson's Storage Tank records 1018 Gerrard St E, on the northwest corner of Gerrard and Marjory Ave (120 m west) as a gasoline station registering a gasoline tank in 1926. It is unclear how long exactly this gasoline station operated. This is considered an environmental concern, though greatly lessened since the record is ~90 years old and the site is ~120 m to the west. At that range and time this activity is only of minor concern. C - Anderson's Waste Disposal Site records record that a former Marjory Dump was located in approximately the same area as the industrial zoned area ~150 m to the northwest of the site, centred in the ravine of the Leslie Creek. Landfills are potentially contaminating activities. The majority of the waste was deposited into the ravine further to the west, and the landfill was reported as last opened in the 1950 s. At this range and about 60 years ago this former landfill is not considered a significant threat to the subject site. Areas of Potential Environmental Concern, APEC s Former operations include the handling of pathological wastes but the business has been registered with the MOE Reg. 347 since 1988 and appears to have operated with care for housekeeping. The basement and main floor where operations are performed have concrete floors with appropriate drains indicating any type of potential contaminant release is unlikely. Soil and groundwater are not expected to be impacted by on-site operations but this was checked in the phase II investigation. Stratigraphy The three boreholes drilled reached a maximum depth of 5 m and the soils encountered were consistent in all three boreholes, being silty sand. Hydrogeology Three groundwater monitoring wells were installed to determine the direction of the groundwater flow and one of these was used to determine the groundwater quality. The groundwater flow through the site was determined to be in a south/southeasterly direction as shown in Figure 1d. Water Table The water table of the unconfined aquifer was at a depth of ~2.6 m below surface. Bedrock Depth The OGS map indicates the depth to bedrock in this area of Toronto is ~33 m. Subsurface Structures and Utilities The only known substructures are the basement under the existing building. This extends to a depth of 2.1 m below ground surface. The underground public utility lines include municipal water and sewer and natural gas which enter from Gerrard Street along the east wall of the building. These can form preferential migration pathways for contaminants.

Areas of Natural Significance The site is not in or close to an area of natural significance. Shallow Soil Property or Property Within 30 m of a Water Body The site is not is not a shallow soil property or within 30 m of a water body. Soil Brought on to Subject Site The site may have been leveled with imported fill during its construction but there was no physical evidence of this.
