1 PH Admission Applicant fills out and submits application to public housing property management office or central office. Unique Identifyers Tcode Name SSN Public Housing Specialist sends applicant an appointment letter for office visit Birth Certificates (All members in household) Social Security cards (All members in household) Bank statements (All members in household) Proof of income (All members in household) Government issued I.D. (All adult members in household) During appointment, applicant completes the following documents: Personal Declaration Form Criminal Activity Certification Fraud Awareness Form Supplement to Application for Federally Assisted Housing Debts owed to Public Housing Agencies and Terminations Declaration of United States Citizenship Employment Verification Form Student Verification Form Utilities Form (Gas & Alabama Power) Authorization for Release of Public Record Authorization for Release of Information Child Support Verification Form Verification of TANF Assistance Public Housing Specialist process applicant s file to determine program eligibility No Ineligibility letter is sent to applicant if applicant does not meet requirements. Yes Eligibility letter is sent to applicant if applicants meet requirements Applicant is contacted when they reach the top of the public housing waiting list and a unit becomes available. At this time applicant must bring in updated proof of income and deposit
2 Housing Specialist sends applicant to property to view unit that is available. No Yes If tenant accepts unit they receive utility papers from property manager s office. The tenant has 24 hours to return utility papers. Manager contact housing specialist with move-in date. Housing specialist prepares tenant s file and tells tenant the deposit amount and prorated rent amount.
3 Move-In Process Property Manager moves tenant into system using yardi step-by-step instruction. Also tenant completes move-in packet. Unique Identifyers Tcode Name SSN After tenant completes move-in packet, property manager and tenant conducts move-in inspection of unit with Form 103. The HUD 103 form is to be completed and inserted in tenant file. After inspection tenant receive two keys and moves in. Property manager or clerk creates work order for maintenance to light the pilot and check for gas leaks in unit. 4 Interim Process Report a change in household composition, the tenant has to come in the property manager s office to report the change within 10 days. The tenant needs to bring into the office the following: Income Verification (Current Pay Check Stubs & Employer complete mailing address) TANF (Most recent print out) Unemployment Compensation (Statement if approved or denied) Child Support Award Letter or print out During the interim process the tenant must complete the following: Public Housing Application Tenant Reported Changes form Personal Declaration Employment Verification Job Termination Verification (If applicable) After verification forms have been verified and management has entered updated information into yardi the tenant is mailed a notice of rent adjustment. EIV should be printed and inserted in tenant file. A zero income checklist form if tenant is reporting no income in the household.
5 PH Reexamination Process Housekeeping notice should be mailed and inspection should be completed before re-exam Tenant is sent a letter to come into the property management office for their reexamination. The tenant is required to bring in the following documentation: 4 current pay checks stubs Tax return (prior year) Tenant is required to complete the following information: After verification forms have been verified and management has entered updated information into yardi the tenant is mailed a notice of rent adjustment and page 1 & 2 of alongwith Notice of Rent Adjustment..
