1 Preliminary Stage: Things to be considered 1) Support Provider will need to consider the following before contacting New Start: STAGE 1 New Start Supported Housing How can help? 1) Contact New Start Supported Housing with information regarding property requirements listed above:
2 2) New Start Supported Housing will forward our Accommodations Needs Assessment form (ANA) to the support provider to complete and return to us. ANA FORM COMPLETED BY SUPPORT PROVIDER 3) Support Provider will complete ANA and return to New Start with as much detail as possible to assist with our search ANA FORM COMPLETED BY SUPPORT PROVIDER 4) New Start Supported Housing will total the average housing cost of the property so that the Support Provider is able to factor the cost into the overall support package when presenting to commissioners The Breakdown will include: The actual rent and other housing management cost Deposit/ Holding Fee required to secure property and administration cost It will NOT include cost for: Utility Bills (which will be paid directly by the tenant) Council Tax (which we will apply for Discount/Exemption after HB has been awarded)
3 5) Support Provider must give NSSH confirmation in writing that funding of the placement has been agreed 6) New Start Supported Housing aim to secure a property within first 6-8 weeks of search, however this could go beyond the specified timeframe depending on location and any special requirements STAGE 2 Once Property is identified 1) It will be necessary for the Support Provider and Service User to be able to view the property without much delay as the properties are generally on the OPEN MARKET and we are competing with the general public!
4 Once Support Provider has decided on a particular property and would like NSSH to take up tenancy of the property. Formal notice in the form of an or letter will be necessary to instruct NSSH to secure the property from the open market 2) Support Provider must formally instruct NSSH in writing to take property off open market 3) All adaptation to property must be agreed by the landlord prior to securing property 3) New Start will send a Pre- Occupation Guide to Support Provider to complete The Pre-Occ will any specific adaptations to be made to the property prior to move in. PRE OCC WET ROOM DISABLED ACCESS FURNITURE 4) Support Provider will complete and return Pre- Occupation which acts a plan of action for NSSH and Support Provider to use to ensure all the peppertree work is agreed before tenancy PRE- OCC WET ROOM DISABLED ACCESS FURNITURE STAGE 3 1) New Start Supported Housing will finalise and agree Lease Agreement with Landlord 2) Move in Date will be agreed between NSSH, Support Provider and Landlord
5 STAGE 4 Moving in Process 1) Service User takes up new tenancy 2) Move is arranged between the Service user and the support provider 2) First day of tenancy New Start will issue: Tenancy Agreement- will be completed on first day of tenancy and a copy returned to NSSH Inventory- will be completed on first day and a copy returned to NSSH Welcome Pack Keys to the home will be handed over to the tenant *In some circumstances we will require the support worker to act on our behalf to ensure the tenancy agreement and inventory are signed and returned to NSSH. 3) Support Provider must be applied for Housing Benefit within the first 7 days of moving in.
6 4) Council Tax exemption/discount can ONLY be applied for once Tenant is in receipt of Housing Benefit End of Tenancy When someone is leaving New Start Supported Housing Tenant or Support Provider must give at least 4 weeks written notice to New Start Supported Housing Support provider will be responsible for notifying local council to cancel Housing Benefit and Council Tax. This is so as the council will only speak to individuals with authorisation which is normally the support provider. What will happen before the service user leaves A Check Out inventory- Support Provider and client to be present Deposit will be returned Payment of outstanding Arrears/Damages
