Approval of the Final Plat subject to compliance with the condition of Preliminary Plat approval along with the following conditions prior to forwarding the plat to the City Council: 1) Submittal of an acceptable final subdivision agreement, if necessary, prior to forwarding the plat to the City Council, and 2) Approval of a Major Amendment to the Conditional Use Permit for College and university facilities in an R4 District. 6. C (D) C Holy Name Housing Corporation Preliminary and Final Plat of ADAMS PARK, a minor plat inside the city limits, with rezoning from R4(35), R5 and R5(35) to R5 and R6 South of 37th and Maple Street intersection Approval of the rezoning from R4(35), R5 and R5(35) to R5 and R6, subject to submittal of a final acceptable PUD (Lot 1) prior to forwarding the request to City Council for final action. Approval of the Preliminary P lat, subject to the following conditions: 1) Coordinate with the Public Works Department in regards to construction or providing a contribution to the storm sewer running east along Maple Street, 2) Verify that all utility

3 Page 3 easements not retained with the vacated rights-of-way have been removed or relocated, 3) Change the private right-of-way on Lot 1 to 37 th Plaza Circle, 4) Provide sidewalks as required by the Subdivision Ordinance, and 5) Include provisions in the subdivision agreement for compliance with all applicable stormwater management ordinances and policies. Approval of the Final Plat, subject to the conditions of Preliminary Plat approval and submittal of an acceptable final subdivision agreement prior to forwarding the plat to the City Council. 7. C (D) C City of Omaha Preliminary and Final Plat of NORTHEAST JOINT USE FACILITY REPLAT 1, a minor plat inside the city limits, with rezoning from R4(35) and GI to GI East of the 20th and Jaynes Street intersection Approval of the rezoning from R4(35) and GI to GI. Approval of the preliminary plat subject to: 1) Coordinating with Public Works and the Nebraska Department of Roads (NDOR) on the appropriate western boundary of the right-of-way vacation along 20th Street, 2) Adding the standard vacation language to the final plat, and 3) Compliance with all applicable stormwater management regulations. Approval of the final plat, subject to the conditions of preliminary plat approval and submittal of an acceptable subdivision agreement, if necessary, prior to forwarding to City Council. 8. C C Sixty Sorensen, LLC Revised Preliminary and Final Plat of SORENSEN PLACE, a subdivision inside the city limits, with a waiver of Section 53-8(4)(d) for Lot frontage, with approval of an MCC-Major Commercial Corridor Overlay District Northeast of 60th Street and Sorensen Parkway DISPOSITION: LAYOVER 7-0. Layover the plat for 30 days, with continuation of the public hearing, to allow the applicant time to address the sewer easement issue, submit an acceptable wetlands mitigation plan and to meet with City staff. 9. C (D) C Blondo 186, LLC Preliminary Plat of BLONDO 186, a subdivision outside the city limits, with a waiver of Section 53-8(2)(b) for Cul-de-sac length maximum from 600' to 715', along with rezoning from AG to DR and R4 Northeast of 189th and Blondo Streets DISPOSITION: LAYOVER C C Lanoha 135 Millard, LLC Preliminary Plat of LUMBERYARD DISTRICT, a subdivision inside the city limits, with rezoning from CC, GI and RR to CC and NBD An area generally bound by Millard Avenue on the north, Q Street on the south, 134 th Street on the east and 136 th Street on the west. DISPOSITION: APPROVAL Approval of the rezoning from CC, GI and RR to CC and NBD, subject to submittal of an acceptable final PUR prior to forwarding the request to the City Council. Approval of the Preliminary Plat, subject to the following being addressed with or prior to submittal of a Final Plat: 1) Dedicate a minimum of 12.5 back of curb for right-of-way for all frontages of Lot 7, 2) Place the following notes on the final plat: (a) There shall be no direct

4 Page 4 access to Q Street from Lot 7., (b) There shall be no direct access to Millard Avenue from Lot 1., 3) Provide for all improvements identified in the final approved traffic study, 4) Work with Public Works for the review and approval process of the roundabout and on-street parking improvements and dedicate for right-of-way to 1 foot beyond the back of the adjacent sidewalk, 5) Provide a pedestrian connection or work with the Planning Department on a possible site for a pedestrian connection across the drainageway at 134th Street to the subject site, 6) Work with the Parks Department to determine whether a 30 foot wide trail easement is needed along the southern portion of Lot 7, 7) Provide sidewalks as required by the Subdivision Ordinance, and 8) Include provisions in the subdivision agreement for compliance with all applicable stormwater management ordinances and policies. Overlay Districts 11. C (D) Holy Name Housing Corporation Approval of a PUD-Planned Unit Development Overlay District South of 37th and Maple Street intersection Approval of the PUD-Planned Unit Development Overlay District, subject to working with city staff on landscape plantings, receiving a waiver from the ZBA for the 5-unit townhouse structure, submittal of revised architectural renderings and a development/construction schedule prior to forwarding the request to the City Council. 12. C Lanoha 135 Millard, LLC Approval of a PUR-Planned Unit Redevelopment Overlay District An area generally bound by Millard Avenue on the north, Q Street on the south, 134 th Street on the east and 136 th Street on the west. Special Use Permits 17. C C C C Sixty Sorensen, LLC Approval of a Major Amendment to a Special Use Permit to allow Convenience storage, approval of a Major Amendment to a Large Project Special Use Permit, approval of a Major Amendment to a Conditional Use Permit to allow Multiplefamily residential and approval of a Conditional Use Permit to allow Warehousing and distribution (limited) in a CC District Northeast of 60th Street and Sorensen Parkway DISPOSITION: LAYOVER 7-0. Layover the Major Amendment to a Large Project Special Use Permit, Major Amendment to a Special Use Permit to allow Convenience storage, Major Amendment to a Conditional Use Permit to allow Multiple-family residential and Conditional Use Permit to allow Warehousing and distribution (limited) in a CC District for 30 days, with continuation of the public hearing, to allow the applicant time to address the following: 1) Submit a full landscape plan for the entire site, 2) Submit a landscape plan for each lot, 3) Submit larger (min. 18x24 ) to-scale site plans for each lot, 4) Submit a building elevation for the proposed Warehousing and distribution (limited) building on Lot 3, 5) Revise the building elevations for the proposed Convenience storage buildings to indicate building material and height, and 6) Submit larger building elevations for the proposed Multiple-family residential structures. 18. Conditional Use Permits 19. C (D) 3101 South 24th Street LLC Approval of a Conditional Use Permit to allow Multiple-family residential in a CC District (property also located within an existing ACI-1(PL) Overlay District) 3101 South 24th Street Approval of the Conditional Use Permit to allow Multiple family residential in a CC-ACI-1(50) District subject to: 1) Compliance with the proposed site plan, 2) Compliance with the proposed operating statement, 3) Compliance with the previously approved building elevations, and 4) Compliance with all other applicable regulations. 21. C (D) C Quality Living, Inc. Approval of a Major Amendment to a Conditional Use Permit to allow Convalescent services in a R7 District 6404 North 70th Plaza Approval of the Major Amendment to the Conditional Use Permit subject to: 1) Compliance with the revised site plan and elevations submitted for the proposed new building and building renovation/addition, 2) Providing 5 additional trees of acceptable size and variety in the street yard to the west of the Life Center building, 3) Redesigning the parking lot for the Life Center to either provide adequate stall overhang or 18 deep stalls and an access aisle for the sidewalk to the east of the building addition, 4) No net increase in stormwater runoff and must treat the first ½ of stormwater for water quality, and 5) Compliance with all other previous conditions of approval. 22. C Lanoha 135 Millard, LLC Approval of a Conditional Use Permit to allow Surface Parking in a NBD District East of the 136 th and Potwin Street intersection DISPOSITION: APPROVAL Approval of a Conditional Use Permit to allow Surface parking in a NBD District subject to: 1) Submittal of a larger to-scale site and landscape plan, 2) Compliance with the revised site and landscape plan, and 3) Compliance with all other applicable regulations.
