Interview of the applicant(s) is by the Admissions Committee and approval is by the Board of Directors. The Admissions Committee will not schedule an interview with the applicant(s) if any information is missing from the application. The Board of Directors and/or management reserve the right to request additional information at any time during the review and interview process. All proposed shareholders and occupants must be available to interview in person before the Board Admissions Committee. Maximum Financing: 50% Please complete the following Application Submission Requirements: 1) Purchase Application, completed and signed. (Please include names and ages of all proposed occupants) 2) Contract of Sale executed by all parties. 3) Lead Paint Disclosure form and Window Guard form must be executed by Seller, Purchaser, and Agent. 4) Credit authorization form and Contact Information Form to be completed by purchaser(s). 5) Applicant Releases for the Board and the managing agent to be executed by purchaser(s). 6) Purchaser(s) to submit letter from landlord or managing agent. 7) Pet Rider Form must be executed by purchaser(s). 8) Letter from employer(s) stating job function, salary and length of employment. 9) House Rules acknowledgement, Homeowners Insurance acknowledgement, tax abatement form and Contact Information Form must be executed by all parties. 10) If financing, commitment letter, appraisal and three (3) original executed Aztech Recognition agreements must be submitted with the package. 11) Applicant(s) must each submit four (4) personal letters of reference and three (3) business letters of reference. 12) Net Worth affidavit must be executed by purchaser(s). 13) Statement of Assets and Liabilities must be filled out by all purchaser(s). (Balance sheet, notes to balance sheet, annual income statement and expense). Packages must include verification of assets (i.e. letter from bank(s) stating type of account, amount on deposit, in dollars and age of account monthly brokerage statements, etc.) 14) Fully executed copies of the last two (2) years tax returns must be submitted with package. (3 Years of tax returns if this is an all cash purchase). OVER>

2 Special Estate Transfer Requirements: (Please submit a fee of $ payable to Maxwell- Kates, Inc for the required review by in-house legal counsel). 1) Certified copy of Death Certificate 2) Original testamentary letters (within the last (6) six months). 3) Copy of the Decedent s Will with attorney Certification, 4) Affidavit as to the payments of Debts and Domicile. 5) Internal Revenue Service Certificate discharging property subject to Federal Estate Lien. 6) New York State Certificate discharging property subject to New York State Tax Lien. Please be advised that a Power of Attorney will not be permitted for the Executor. The Executor must attend or have all documents pre-signed. If there is more than one executor, one must be present at closing and the other(s) must have all documents pre-signed. HOUSE RULES The public halls and stairways of the building shall not be obstructed or used for any purpose other than ingress to and egress from the apartments in the building, and the fire towers and fire escapes shall not be obstructed in any way. 2. Children shall not play in the public halls, stairways, fire towers or elevators, and shall not be permitted in the service elevator of the building. 3. No public hall above the ground floor of the building shall be decorated or furnished by any lessee in any manner without the prior consent of the lessor and of all of the lessees to whose apartments such hall serves as a means of ingress and egress. 4. No lessee shall make or permit any disturbing noises in the building or do or permit anything to be done therein, which will interfere with the rights, comfort or convenience of other lessees. No lessee shall play upon or suffer to be played upon any musical instrument or permit to be operated a phonograph or a radio or television loud speaker in such lessee's apartment between the hours of 1 1 p.m. and the following 9:00 a.m. if the same shall disturb or annoy other occupants of the building, and in no event shall any lessee practice or suffer to be practiced either vocal or instrumental music for more than two hours in any day or between the hours of 6:00 p.m. and the following 9:00 a.m. No lessee shall give vocal or instrumental instruction at any time. 5. Each lessee shall keep such lessee's apartment in a good state of preservation and cleanliness and shall not sweep or throw or permit to be swept or thrown there from, or from the doors, windows, terraces or balconies thereof, any dirt or other substance. 6. No article shall be placed in the halls or on the staircase landings, fire towers or fire escapes, nor shall anything be hung or shaken from the doors, windows, terraces or balconies or placed upon the windowsills of the building. 7. No shades, awnings, window guards, ventilators or air conditioning devices shall be used in or about the building except such as shall have been approved by the lessor

No sign, notice or advertisement shall be inscribed or exposed on or at any window or other part of the building, except such as shall have been approved in writing by the lessor; nor shall anything be projected out of any window of the building without similar approval. 9. The passenger and service elevators in the building shall be operated only by employees of the lessor and there shall be no interference whatever with the same by lessees or members of their families or their guests, employees or sub-tenants, except as approved by the lessor No velocipedes, bicycles, scooters or similar vehicles shall be allowed in the passenger elevators and no baby carriages or any of the above-mentioned vehicles shall be allowed to stand in the public halls, passageways, areas or courts of the building Servants, messengers and trades people shall use the service elevators in the building for ingress and egress and shall not use the passenger elevators for any purpose, except that nurses in the employ of lessees and nurses in the employ of guests or sub-tenants of lessees may use the passenger elevators when accompanying children of said lessees, guests or sub-tenants Kitchen supplies, market goods and packages of every kind are to be delivered only at the service entrance of the building and through the service elevators to the apartments Trunks and heavy baggage shall be taken in or out of the building through the service entrance. 14. Garbage and refuse from the apartments shall be deposited in the incinerators of the building only at such times and in such manner as the lessor may direct Water closets and other water apparatus in the building shall not be used for any purposes other than those for which they were constructed, nor shall any sweepings, rubbish, rags or any other article be thrown into the same. Any damage resulting from misuse of any water closets or other apparatus shall be paid for by the lessee in whose apartment it shall have been caused No lessee shall send any employee of the lessor out of the building on any private business of a lessee, including the parking of cars or the walking of dogs No animals, including dogs, cats and other usual household pets, or reptiles shall be kept or harbored in the building. No bird shall be kept or harbored in the building

19 which constitutes a nuisance in the opinion f the Board of Directors of the lessor. No animals being kept or harbored in the building prior to the adoption of this amended rule number seventeen shall be permitted on passenger elevators. Further any of the said animals must be carried or on leash in any of the public portions of the building No radio or television aerial shall be attached to or hung from the exterior of the building without the written approval of the lessor. 19. The agents of the lessor, and any contractor or workman authorized by the lessor, may enter any apartment at any reasonable hour of the day for the purpose of inspecting such apartment to ascertain whether measures are necessary or desirable to control or exterminate any vermin, insects or other pests and for the purpose of taking such measures as may be necessary to control or exterminate an such vermin, insects or other pests. 20. No vehicle belonging to a lessee or to a member of the family or guest, sub-tenant or employee of a lessee shall be parked in such manner as to impede or prevent ready access to the entrance of the building by another vehicle. 21. Except where the lessee has been granted the exclusive use of a laundry, the lessee shall use the laundry facilities available only upon such days and during such hours as may be designated by the lessor 22. The lessor shall have the right from time to time to curtail or re-locate any space devoted to storage or laundry purposes. 23. The lessor may retain a passkey to each apartment. No lessee shall alter any lock or install a new lock on any door leading into the apartment of such lessee without the prior written consent of the lessor. If such consent is given, the lessee shall provide the lessor with a key for lessor's use. 24. Complaints regarding the service of the building shall be made in writing to the managing agent of the lessor. 25. Any consent or approval given under these house rules by the lessor shall be revocable at any time. 26. Subleasing of apartments is not permitted for periods longer than one (1) year and then only once in any five-year period, Sub-tenants are subject to the same scrutiny by the Admissions Committee as prospective shareholders and prior to any interview with the Committee must have delivered to the cooperative's Managing

20 Agents all application forms and questionnaires duly filled out and signed. All subtenants must agree in writing to be governed by the provisions of the sub-leasing shareholder's proprietary lease to the extent that any provisions therein may be applicable to a party in occupancy other than a shareholder and by the House Rules and bylaws of the Corporation. Shareholders subleasing apartments must carry homeowners insurance and must require that subtenants carry homeowners/renters insurance. 27. These House Rules may be added to, amended or repealed at any time by resolution of the Board of Directors of the lessor.

21 Seventeen: No bird, reptile, or animal shall be permitted, raised, bred, kept, or harbored in, on or about the Building unless, in each instance, the same shall have been expressly permitted in writing by the Boarde Any such consent, if given, shall be revocable at any time by the Board in its sole discretion. Notwithstanding the foregoing, a Lessee (and not a subtenant) may request that the Board, or a committee of the Board, interview a dog, at which time Board members will determine, among other things, how the dog behaves and interacts with others. The Board may elect to have another dog(s) present at the interview. Provided the Board, after the interview, permits the dog to be maintained by the Lessee, Lessee shall be permitted to maintain the dog in the Apartment, provided the following rules are strictly complied with: a. Dogs can weigh no more than 25 pounds at full growth. b. Dogs must be carried or on a leash no more than 2 feet from the one in control of the leash when in the common areas of the Building. c. Dogs may not be transported in the main elevators and are restricted to the service elevator only. d. No "visiting" or "guest" pets shall be permitted in the Building. e. All dogs must be registered with the Board. f. Dog owners shall be responsible for their dog's behavior and shall not permit the dog to cause conditions which are a nuisance to residents, their children, employees and guests, including, but not limited to defecation or urination in any common area of the Building or the entrances; causing damage to property; aggressive behavior towards people and/or other pets; and, continuous barking. Pet owners must be respectful of those residents and guests who may be uncomfortable with dogs. g. It is the dog owner's responsibility to clean up after their pet in the event of an "accident" and to advise the Building staff immediately to prevent odors and minimize damage to the Building. A dog owner may be requested to muzzle their dog when in public spaces or common areas. A dog owner may be required, in the Board's sole discretion, to hire a trainer or animal behaviorist in the event the dog behaves aggressively, is noisy, or disturbs other residents of the Building. All costs associated with the dog, including the cost of an animal behaviorist, if required, shall be borne by the Lessee which harbors the dog. h. Rottweilers, Dobermans and Pit Bulls, or similar dogs, are specifically prohibited. i. Lessees who harbor a dog must make sure that actions by the dog are covered under the insurance policy Lessee maintains pursuant to Article Il, paragraph Eighteenth of the Lease or such other insurance policy with similar liability limits. [PLEASE CONFIRM THAT THIS IS FEASIBLE WITH YOUR INSURANCE PROFESSIONAL] j. All dogs must be licensed in accordance with applicable law and Lessees must submit proof of vaccination to the Board upon demand. k. The Board may demand, in its sole discretion, that the dog be removed from the Building. Failure to register a dog with the Board or any infraction of this Rule shall be construed as a violation of the Rules and Regulations and shall result in a $100 fine per infraction or per day until such violation is cured.
