1 Allianz Worldwide Care Out-patient Direct Settlement Network - April 2015 In order to provide our insured membership with access to the best possible medical care and service, AWC has developed a network of medical practices, where insured members having an AWC International Plan membership card may receive medical treatment which will be settled by us directly with the clinic/medical centre. When visiting any of the clinics/medical centres within this network, please follow the processes outlined below: 1. Present your AWC membership card and proof of identification to the receptionist prior to the consultation. 2. Sign the Claim Form to acknowledge receipt of the treatment following the consultation. 3. Settle any co-payments, or deductible/excess payments with the clinic/medical centre prior to leaving. For example, if your policy with us has a 20% co-payment for out-patient treatment, you should settle 20% of the total bill prior to leaving the clinic/medical centre. In such a case, if your total bill is $300, we will pay $240 direct to the clinic/medical centre and you will pay $60 following your treatment and prior to leaving the clinic/medical centre. IMPORTANT NOTES i) The direct settlement network should not to be used to obtain any treatment, which falls outside the scope of your policy. ii) The direct settlement network is dependent upon the level of cover that you have purchased from Allianz Worldwide Care. iii) Allianz Worldwide Care will not provide cover for any membership fees that may be demanded by any medical provider. iv) If you use a Hospital outside of our Network, please ask the Hospital to contact us directly and we will arrange direct settlement. v) We also have access to an extensive network of providers through our third party administrators in the USA. Members with Worldwide Cover can contact the toll-free number as indicated on the reverse of the membership card for more information. Page 1 of 7
