2 SEMINOLE OWNERS CORPORATION & TH STREET FOREST HILLS, NY HOUSE RULES The Seminole Owners Corp. House Rules are established pursuant to the Proprietary Lease. These rules may be amended or repealed at any time by the Board of Directors. The Board of Directors has directed and authorized its managing agent to enforce the house rules. A shareholder in violation of a house rule will be notified of the violation by the managing agent and given an opportunity to remedy the situation. An uncorrected violation will result in a fine. Unpaid fines on uncorrected violations will be referred to the cooperative s attorney for further action. A shareholder may appeal in writing the managing agent s decision regarding a house rule violation to the Board of Directors at: Board of Directors Seminole Owners Corp th Street Forest Hills, NY The shareholder will be notified in writing after the Board has reached a decision on the appeal. FAILURE BY THE COOPERATIVE TO ENFORCE ANY OR THE HOUSE RULES SHALL NOT BE CONSIDERED A WAIVER. THE COOPERATIVE SHALL NOT BE LIABLE TO THE SHAREHOLDERS AND LESSEES FOR FAILURE TO ENFORCE ANY RULES. IN ADDITION TO THE HOUSE RULES, ALL SHAREHOLDERS ARE RESPONSIBLE FOR COMPLYING WITH THE TERMS OF THE PROPRIETARY LEASE.

4 PUBLIC AREAS 1. The public halls and stairways of the building shall not be obstructed or used for any purpose other than entering or leaving the apartment or the building. Specifically, they shall not be used for recreational purposes. 2. Bicycles, scooters, strollers, carriages or vehicles are not allowed to stand in the lobby, public halls, garage and other public areas of the building. Bicycles may not be chained to any fence on the building property. Bicycles must be kept inside a resident s apartment. Residents are responsible for bicycles or other vehicles brought in by their guest. Pending space availability, residents may use the bicycle room located in the building for a monthly fee. Bicycles are stored here at the owner s risk. 3. The roof doors shall be kept locked at all times. The roof shall not be used for any purpose whatsoever. 4. Public halls and public doors shall not be decorated by any resident in any manner without the prior consent of the Board of Directors. Any defacement of walls, doors or the elevators by a resident, guest or employee of a resident will be repaired by the corporation at the expense of the unit owner. 5. Signs, notices or advertisements shall not be put in or on any window, in the elevators, in the lobby or any part of the building without the permission of the Board of Directors. Notice boards are available in the laundry rooms and mail areas for the use of building residents. 6. Smoking or carrying of lighted cigarettes, cigars or pipes is not permitted in the elevators or public areas. It is against the law. Eating and drinking are not permitted in the elevators or public areas. 7. The elevator doors may not be obstructed. 8. Garbage and litter shall not be placed in any public area of the building, other than the compactor rooms and designated recycle containers. 9. Children may not play in the lobby, public halls, stairwells, elevators or on the lawns of the building. No pets are allowed on the building lawns at any time. 10. Pigeons, squirrels, other birds and animals are not to be fed from the windowsills, fire escapes, roof, garden areas or sidewalks in front or back of the building. Any violation of this rule will result in a fine. 11. No article shall be placed in the halls or on the staircase landings nor shall anything be hung or shaken from the doors or windows or placed upon the windowsills of the building. 12. No awnings, window air conditioning units or ventilators shall be used in or about the building except such as shall have been expressly approved by the Lessor or the managing agent, nor shall anything be projected out of any window of the building without similar approval. 13. In no event shall pets be permitted on elevators or in any of the public portions of the building unless carried or on a leash.

5 MOVING IN AND OUT 1. Residents, both unit owners and lessees (renters), must obtain a letter of authorization from the managing agent before they will be permitted to move into or out of the building. No move, in or out, will be permitted by the building staff without this written authorization. 2. Moving in and moving out is permitted between the hours of 9:00 AM and 6:00 PM Monday through Friday, and Saturday 10:00 AM to 6:00 PM. Moves are not permitted on Sundays or holidays. Move ins and move outs must be completed by 6:00PM. 3. Any move into or out of the cooperative must be scheduled and approved by the Superintendent and the Managing Agent at least three days in advance so elevator padding may be installed. 4. At the time of scheduling a move, whether into or out of the building, the Management company must be given a certified check or money order in the amount of $500 payable to Seminole Owners Corp.. This sum will be returned after the move unless inspection shows that damage was done to any of the public areas, in which case the cost of repair or replacement will be subtracted from the $500 and the balance will be returned. If the cost exceeds $500 the owner will be responsible for the excess costs. 5. Pianos and other large objects shall be moved into and out of the building by means of the stairways. Windows and the top of the elevators shall not be used for moving items into or out of the building or from one floor to another. 6. All moves into and out of the building will take place through the service entrance. 7. Failure of the unit owners(s) and their lessees to comply with these moving rules, reference in 1 6 above, will automatically result in an administrative fee for the unit owner. Final determination regarding any violation of this section remains with the Board of Directors. APARTMENT ACCESS 1. Residents of the cooperative must permit entry to their apartment by the building superintendent, the managing agent, and agents of the coop including any contractor or workers authorize by the coop, upon reasonable notice and at reasonable hours of the day. Such access will be for the purpose of inspecting the apartment for damage or possible leaks to or from another unit and to make such repairs or improvements as may be necessary for the protection and maintenance of the building property and safety of the residents. The coop agents may enter a unit at any time in the event of an emergency. 2. It is recommended that residents provide a set of apartment entrance keys to the superintendent. The keys will only be used in the event of an emergency. If keys are not provided, superintendent, emergency personnel, or coop agents may be required to break the lock(s) or the door itself in an emergency. If keys have not been made available, all costs attributable to an emergency entry shall be the responsibility of the resident.

6 APARTMENT OCCUPANCY AND USE 1. No unit may be used for any illegal purpose. 2. No unit may keep or harbor a dog, an endangered species or any animal that reasonably poses a safety issue to other building residents. 3. No unit may be used for commercial purposes without the approval of the Board of Directors. The Board has the right to make inquiries of any resident where there is a reasonable basis for concluding that a residence is being occupied in violation of the Bylaws or these House Rules. 4. Residents may not make or permit any disturbing noise in the building that will interfere with the rights, comfort or convenience of others. No musical instrument, radio, or television shall be played above a reasonable sound level, and until a reasonable hour. Residents also may not make or permit any disturbing odors, especially from smoking, whether inside an apartment or in the public areas. 5. A resident who intends to have guests in his or her apartment while the resident is away shall give the management advance written notice of the guests names, the apartment number in which they will be staying, and the date of the visit. 6. Nothing shall be installed outside any window of the building, including window boxes or planters, radio or television aerials or dish antennas. If an air conditioner is to be installed in your apartment, the installation must comply with NYC Building Department codes and regulations. All air conditioners must be installed using air conditioner support brackets and are subject to the inspection of the building Superintendent or Managing Agent. Any violation of this rule will result in an administration fee. 7. Trash, mops, dust cloths and other items may not be thrown or shaken from windows or fire escapes. No items may be placed on outside windows, window ledges or fire escapes. 8. At least eighty (80) percent of the floor area of each residential unit (excluding kitchens and bathrooms) must be covered with rugs, carpeting or equally effective noise reducing material and padding. Violation of this rule will result in an administrative fee. 9. Residents shall not install, own or operate washing machines in any unit of the buildings. Any violation of this rule will result in an administrative fee and the removal of the machinery at the owner s expense. 10. Residents shall keep entrance doors closed especially at times when noise or cooking odors might infiltrate the hallways. 11. Bathrooms and sinks may not be used for any purpose other than those for which they were constructed. The cause for repairing any damage caused by misuse of same shall be paid by the shareholder. 12. Residents shall not waste water and shall repair all leaky faucets.

7 13. Doormats shall not be permitted to be put in the common area directly in front of the resident s door. 14. All units must have a working smoke detector and a carbon monoxide detector. REPAIRS, ALTERATIONS AND RENOVATIONS 1. All renovations, alterations, and improvements must be submitted to the managing agent for written approval prior to the commencement of any work. 2. Residents may request the services of the Superintendent for ordinary and necessary repairs. Service Work Order forms are available from the doorman or Superintendent. The Managing Agent will bill the owner for repairs that are not the responsibility of the cooperative. This shall include the cost of plans or the use of a licensed professional. 3. Prior to starting the alteration copies of insurance certificates from individual tradespersons (i.e. contractors, plumbers, electricians, etc.) must be obtained and given to the managing agent. All insurance certificates must name Seminole Owners Corp and the Managing Agent as co insured. 4. When these requirements have been fulfilled, the managing agent will issue an Alteration Agreement. This agreement clearly stipulates compliance with submitted plans and outlines pertinent regulations regarding the accomplishment of the work. Demolition or construction may not begin until the Alteration Agreement has been signed by both the managing agent and the unit owner and returned to the managing agent. 5. Unit owners must use only licensed and bonded workmen when major repairs or any structural alterations or renovations are made in an apartment. 6. Unit owners shall inform the Superintendent in advance when outside contractors or workers will be entering the building to work in an apartment. Such work will be done during the hours of 8:00 AM and 5:00 PM, Monday through Friday. 7. Unit owners will be responsible for removal of all garbage generated by any renovation or repair. The Unit Owner will also be responsible for paying costs for any damages caused to the common areas of the building by their contractors or workers. 8. The Board of Directors may require a unit owner to return an apartment to its prior condition, at the owner s sole expense, if unauthorized alterations or renovations have been made by either a unit owner or a lessee. BUILDING SAFETY AND SECURITY 1. A doorman will be at the designated station to control access to the building daily. Residents who are known to him or her will be allowed to pass freely in and out of the building. All other persons entering the building are to be cleared by the doorman before being allowed to enter the inside lobby.

8 2. The doorman will announce visitors and delivery personnel by intercom. When cleared by a resident, they will be allowed to enter the building. 3. Residents must be home to receive large deliveries (e.g. furniture, large appliances, etc.) when they are delivered. 4. The doormen are responsible to see that the above security procedures are followed at all times. They are instructed not to leave the door except in cases of extreme emergency. Doormen are not to run errands for residents during their working hours or to make deliveries to resident s apartments. 5. Any agent, contractor or work person authorized by the cooperative may enter any apartment at any reasonable hour for the purpose of inspecting such apartment to ascertain whether measures are necessary or desirable to control or exterminate any vermin, insects or other pests, or to determine if there is any violation of tenancy, occupancy function, enforcement of house rules, security, safety or health standards. 6. Common doors to the building are to remain closed and secured. No one is to prop the doors open or disengage the locks or use any other means to allow access to the building without a key. No one is to tamper with or compromise the security features of the building. 7. As mandated by law, New York City Health Code Section requires the installation of child guards if a child ten (10) years of age or younger is living in an apartment. If child guards are needed, the shareholder or their lessee must contact the managing agent to arrange for the installation. The shareholder will be charged $10 per window guard installed. Even if there are no children aged 10 or younger in a unit, guards may be installed where they are desired by the resident. It is also important to remember that the child guard form must be filled out every year. 8. In case of fire, please refer to the fire safety notice. A copy of this notice is attached. REFUSE DISPOSAL 1. All wet and non recyclable garbage is to be securely wrapped or bagged in plastic or other waterproof material that is completely drip free before it leaves the apartment. This garbage package should be the proper size to fit easily into the chute, which fees into a compactor. This debris is not to be left on the floor of the trash chute areas on any floor or in the basement area. 2. Bulky objects (large carton, crates, wood and other solid matter) shall not be stuffed into the chute. Residents shall consult with the staff as to the appropriate procedure to follow in disposing of these items. 3. Newspapers and magazines, clean recyclable plastic and cans, and all glass should be placed in the appropriate recycling containers on each floor.

9 4. Under no circumstances should flammable items, lighted cigarettes or cigar stubs, any chemical substance, aerosol cans, carpet sweepings, items containing naphthalene, camphor balls or flakes, floor scrapings, oil soaked rags, empty paint cans or any other inflammable, explosive, highly combustible substances be thrown into the compactor chute. Such action is against the law and the offender is subject to the penalty prescribed by law. Residents should consult with the staff about the proper procedure to follow in disposing of these items. 5. Persons using the compactor chute must make sure that items placed in the chute drop to the compactor. If they do not, the item is to be broken down into smaller units. The chute door is to be completely closed following its use. LAUNDRY ROOM 1. The laundry room is for the personal use of the resident only. It should not be used for any commercial purpose. 2. Persons using the laundry room shall keep the room and equipment neat and clean. Lint or other residue in the washer or dryer should be removed by the resident after every use. 3. Laundry must be removed promptly when the washing and drying cycles are completed. 4. Laundry carts are provided to carry clean laundry from the washers to the dryers only. They are not to be used to sort dirty clothes. Carts should not be taken from the laundry room. 5. The laundry room shall be available to residents from 7:00 AM to Midnight, seven days a week. MAINTENANCE PAYMENTS 1. All shareholders are obligated to submit their monthly maintenance payments to the Managing Agent by the tenth day of each month. A late fee will be assessed when payments are received late. The penalty and fee will be billed by the Managing Agent to the shareholder following monthly statement bill. SUBLET POLICY 1. The Building does not currently permit subletting. Any violation of these rules will constitute a breach of the resident's lease and all appropriate legal remedies will be sought.

10 FIRE SAFETY NOTICE IN THE EVENT OF FIRE, STAY CALM. NOTIFY THE FIRE DEPARTMENT AND FOLLOW THE DIRECTIONS OF FIRE DEPARTMENT PERSONNEL. IF YOU MUST TAKE IMMEDIATE ACTION, USE YOUR JUDGMENT AS TO THE SAFEST COURSE OF ACTION, ABIDED BY THE FOLLOWING INFORMATION: YOU ARE IN A COMBUSTIBLE (NON FIREPROOF) BUILDING If The Fire Is In Your Apartment Close the door to the room where the fire is and leave the apartment. Make sure EVERYONE leaves the apartment with you. Take your keys. Close, but do not lock, the apartment door. Alert people on your floor by knocking on their doors on your way to the ext. Use the nearest stairwell to leave the building. DO NOT USE THE ELEVATOR. Call 911 once you reach a safe location. Do not assume the fire has been reported unless firefighters are on the scene. Meet the member of your household at a pre determined location outside the building. Notify the firefighters if anyone is unaccounted for. If The Fire Is Not In Your Apartment Feel your apartment door and doorknob for heat. If they are not hot, open the door slightly and check the hallway for smoke, heat, or fire. Exit the apartment and building if you can safely do so, following the instructions above for a fire in your apartment. If the hallway or stairwell is not safe because of smoke, heat, or fire and you have access to a fire escape, use it to exit the building. Proceed cautiously on the fire escape and always carry or hold onto small children. If you cannot use the stairs of the fire escape, call 911 and tell them your address, floor, apartment number and the number of people in your apartment. Seal the doors to your apartment with wet towels or sheets, and seal air ducts or other openings where smoke may enter. Open windows a few inches at top and bottom unless flames and smoke are coming from below. Do not break any windows. If conditions in the apartment appear life threatening, open a window and wave a towel or sheet to attract the attention of firefighters. If smoke conditions worsen before help arrives, get down on the floor and take short breaths through your nose. If possible retreat to a balcony or terrace away from the source of the flame heat or smoke.
