1 Mr. C is an 86-year-old veteran with end-stage heart failure and moderate dementia. After surviving some of the toughest fighting in World War II, he went on to earn a law degree and establish a successful practice. Mr. C never talked about the war with his family. They never thought to mention it to his doctors or the nursing home staff. Mr. C is haunted by memories from more than half a lifetime ago. Observing his frequent tears and periods of profound depression his family and the nursing home staff realize something is wrong but they don t know how to comfort him. Mrs. J is a 67-year-old veteran who served as a nurse in Vietnam. She has been told by her doctor that she has less than six months to live. For the past two years, Mrs. J has been receiving treatment at a Department of Veterans Affairs (VA) Medical Center an hour away. Although she still wants to maintain contact with VA staff, she is no longer able to make the trip to the VA Medical Center and wishes to spend her remaining time in the comfort of her home. Mr. L is a 56-year-old veteran with end-stage liver disease and a limited prognosis. He lost a leg in Vietnam and has struggled with alcohol abuse and depression since returning to the United States. Now homeless and with no income or family support, he is in a VA hospital after collapsing outside an office building. A PUBLICATION OF THE HOSPICE & PALLIATIVE CARE GROUP, INC. Ensuring Quality End-of-Life Care for Our Nation s Veterans These stories represent just a few of the more than 600,000 veterans who are expected to die each year well into the next decade. Many of these veterans could benefit from hospice care, an end-of-life care choice that provides dying patients and their loved ones with comfort, compassion and dignity. Hospice care involves a patient-centered, team-oriented approach to expert medical care, pain management, and emotional and spiritual support. Care can be provided in a number of settings including patients homes, inpatient hospice units, hospitals, nursing homes, and long-term care facilities. The butterfly that emerges has a whole new life in a different form. CONTENTS Ensuring Quality Care Niagara Hospice volunteer and navy veteran, Gene Brayley, visits with army veteran Ralph Bishop at Niagara Hospice House. Mr. Bishop served in Washington guarding presidents Truman and Eisenhower. Veterans helping Veterans Niagara Hospice is recruiting veterans to volunteer to visit with hospice patients who are also veterans. Veterans share a unique camaraderie that can be especially beneficial to hospice patients. A hospice volunteer training will be held in early December. Interested men and women are asked to call the Niagara Hospice volunteer office at to learn more. Volunteers are also being recruited to craft patriotic quilts and lap blankets for Niagara Hospice s veteran patients. If you quilt, knit or crochet please consider helping our volunteers. The emotional and spiritual components of hospice care can be especially meaningful to veterans who often face issues near the end of life relating to their military experiences. Support is even more crucial for veterans who do not have a strong support network of family and friends. We Honor Veterans, a project of the National Hospice and Palliative Care Organization (NHPCO) in collaboration with the Department of Veterans Affairs (VA) has invited hospices, state hospice organizations and VA partners to join a pioneering program focused on respectful inquiry, compassionate listening and grateful acknowledgment. By recognizing their unique needs, community providers like Niagara Hospice, in partnership with VA staff, will help guide America s veterans and their families through their life stories toward a more peaceful ending. A Fragmented System Anyone who has tried to navigate the health care system in America knows that often there is fragmented communication and coordination of services among health care providers, public and private insurance companies, and patients and families. Even though the Department of Veterans Affairs (VA) is the largest integrated health care system in the country there are still issues. So how can we ensure veterans receive quality end-of-life care? It s an important question to answer--particularly when considering these facts from the VA: More than 1,800 veterans die every day. This represents a quarter of all deaths in America. Approximately 85% of veterans do not receive care through the VA health care system. Most veterans still die in the community; only 4% of veteran deaths occur in VA facilities. These statistics highlight the importance of partnerships among VA and community health providers as well as organizations that serve veterans. There is a great need for education about hospice care and how it can be accessed. Hospice care is part of the basic eligibility package for veterans enrolled in the Veterans Health Administration (VHA). If hospice care is appropriate for enrolled veterans and has been approved by a VA physician, VA medical centers will either provide hospice care directly in their facilities or purchase it from community hospices. All Medicare-eligible veterans, whether or not they are enrolled in VHA, have access to hospice care through Medicare. Veterans not eligible for Medicare may have hospice benefits through Medicaid or other private insurance. However, like 90% of all Americans, most veterans simply don t know that these options exist. The need for education extends beyond the public to community hospice and VA providers as well. Niagara Hospice has established a partnership with the Veteran s Administration in Buffalo to ensure quality end-of-life care is available to all Niagara County veterans. Goals of the partnership include educating veterans and the community about the hospice benefit that every veteran is entitled to; customizing hospice care plans specific to the special needs of veterans; and recruiting community veterans to serve as hospice volunteers who will be paired with Niagara Hospice patients who are also veterans. If you are a veteran and would like to learn more about volunteering for Niagara Hospice, call the Volunteer Services office at Record 1.56 Million Patients Receive Hospice Care The National Hospice and Palliative Care Organization recently released the following report on Hospice Care in the U.S. An estimated 41.6 percent of all people who died in the U.S. last year were under the care of a hospice program, reports the National Hospice and Palliative Care Organization (NHPCO). This figure is up from 38.8 percent in 2008 and 35 percent in This rising statistic shows that more Americans facing a life-limiting illness are opting for care that addresses medical, emotional, and spiritual needs at the end of life, said J. Donald Schumacher, NHPCO president and CEO. And that is the compassionate care that hospice and palliative care brings to patients and families in need. NHPCO s annual publication, Facts and Figures: Hospice Care in America reports on hospice trends and provides updated information on the growth, delivery, and quality of hospice care in the US. Findings include: A record 1.56 million people with a life-limiting illness received care from the nation s hospice providers. The average length of service remained steady at 69 days. Median length of service was also steady at 21.1 days (that means half of hospice patients received care for less than three weeks and half received care for more than three weeks). Routine home care accounted for 95.9 percent of patient care days. At 68.8 percent, the majority of hospice care was provided in the home, whether the patient s home is a private residence, nursing home, or residential facility. Findings from the report are based on data from NHPCO s annual survey, the National Data Set, and NHPCO membership data. Secondary sources include Medicare Provider of Services certification data, Medicare hospice cost report data, state-mandated data submissions and state association membership surveys. Jack Marshall, Spiritual Care Counselor The assignment was simple, take a person you identify with and identify his or her spirituality. The course was Spirituality and Aging and one student chose Georgia O Keefe. The artist had no particular religion and had been involved in a notorious affair that ended the marriage of her sponsor. How could this woman have any spiritual side? my students asked. She painted for a future she believed in. Modern intellectual and writer, Tony Judt, was born in England, lived on an Israeli kibbutz just before the June War of 1967 and came to see himself as a secular Jew without belief in God. Judt suffered from ALS (Lou Gehrig s Disease) and toward the end of his life discussed his controversial work, impending death and his spirituality. He sighted something of the human spirit and how important it was to honor the duty of giving hope and support to the generations to come. Judt, a controversial public intellectual, who tried to make sense and raise the moral issues of our time died recently at 67. I listened to his interview on Terry Gross Fresh Air on National Public Radio while I was on the road. Later that day I visited one of my non-religious believers; atheists, who are convinced that the human spirit is worth believing in. A patient once told me, I m not into God and religion. I am into the here and now; I don t have too much of a tomorrow to look to. So I have to do what I can, right now without worrying about the past. I can t change it, but I can leave something Niagara Hospice will hold an Open House on Veterans Day in honor of all veterans and their families. 4 Spiritual side, continued from page 3 good for the future, for the kids and the generations to come after me. It is easy to see how living in the moment with the intention of gifting the future gives hope and purpose to our patients, even if they do not necessarily believe in a here after. There is something here that non-religious believers share with people of religious faith and that we can put to use. Their scope of concern is for the here and now as they live in the moment. In my reading I find that every major religion has its own version of the golden rule ; Do unto others as you would have them do unto you; a yearning for justice and for doing the moral good. While our faith perspectives may vary, our concern for our fellow sojourners and the generations to come is a very common bond. For believer and non-believer, the common understanding of the human spirit is far more powerful than we might expect. Jennifer Peth 4 The Voyager A Grief Shared By Jennifer Peth, MS, Outreach Counselor There is a Honduran proverb that advises, A grief shared is half grief. The Niagara Hospice Outreach Department knows firsthand the great power of healing, and the comfort that is found when individuals in grief come together to share their feelings and experiences. Niagara Hospice s Circle of Hope program provides a wide variety of grief and loss workshops and support groups that are open to the entire Niagara County community. When we are grieving we often feel isolated, like there is no one who understands how we feel, and no one who will listen to us express our grief. At a grief support group the bereaved can meet with others who have also lost loved ones, who are familiar with the difficult and painful experience of grief, and who are open and ready to listen to the grief feelings of others as well as to share their own. Attendees of support groups are often surprised by the instant connection that they feel with other group members. Even people who are normally very private with their emotions may find themselves opening up in group, finding safety and comfort in the honest and caring group atmosphere. There is no way to get around grief; it is a painful and difficult journey that we must travel from start to finish. However, if we come together and share our pain with others who are also navigating the journey of grief, it can help lighten the load we all have to carry. Niagara Hospice staff and volunteers participated in the Lewiston Kiwanis Club Peach Festival Parade last September. The theme for our float was: Thank you Niagara County you are the PEACH in our pie! The themed peach orchard and giant peach pie with a baker (Corey Voelker) won third place in the float competition. 14 Events 2010 Gooch Fest 2010 benefits Niagara Hospice Jesse Gooch and family held Gooch Fest 2010 last July to help support Niagara Hospice. The day-long event featured four bands, food, raffles and more raising over $1,200. Thank you to Jesse and the Gooch family for their dedication to Niagara Hospice. Over 360 butterflies were released in memory or honor of loved ones. The event is open to the community and continues to be one of the biggest events for Niagara Hospice. Niagara Wheatfield Junior Achievement students start up shop Niagara Wheatfield students created their own businesses for their spring marketing class. The students seen here (at left) sold Niagara Wheatfield wristbands and lanyards and then donated $400 in profits to Niagara Hospice. Presenting the donation to Special Events Coordinator Mallory Zabel are Niagara Wheatfield Students (L to R) Courtney Amendola, Dino LoSsracco, Robert Dorato, Stephen Janik, Ryan Elnicki, Amanda Breier and Sawyer Mosher. 14 The Voyager Eternal Brothers Motorcycle Club ride for a cause Eternal Brothers Motorcycle Club held a motorcycle road race last August to benefit Niagara Hospice. The event started with a lineup of motorcycles riding in memory of loved ones. The event continued with friends, family and community members who gathered for food and raffles. The event raised $2,000 for Niagara Hospice.
