Shown Not Shown Not Applicable REQUIRED SUBMITTALS CHECK LIST FOR SITE DEVELOPMENT PLANS I. GENERAL DEVELOPMENT AND PROPOSED IMPROVEMENTS A. General information. 1. Name of project; 2. Statement of intended use of site; 3. Legal description of the property, size of parcel in acres or square feet, and sealed boundary survey of the site or survey incorporated into the site plan and sealed; 4. Name, address, and telephone number of the applicant and written authorization by the owner authorizing the submittal; 5. Name, address, signature, and registration (sealed plans) of the professionals and firm preparing the plan; 6. Date, North arrow and scale, number of sheets; the scale (not smaller than one inch to 50 feet shall be designated, unless the project is a large project and permission is requested for a larger scale. However, in no case shall the scale be smaller than one inch equals 100 feet; 7. Vicinity map, showing relationship of proposed development to the surrounding streets and thoroughfares, shall be at an overall scale of not less than one inch equals 2,000 feet (1" = 2,000'); 8. Linear dimensions of the proposed building; 9. Existing topography with a minimum of one foot contour intervals or a special purpose elevation study. USGS datum; 10. Finished grading elevations; 11. All existing and proposed building restriction lines (i.e., highway setback lines, easements, covenants, rights-of-way, and building setback lines, even if more restrictive than those specified by the zoning regulations); 12. Any formal commitments, including, but not limited to contributions to offset public facilities impacts. B. Building and structures: 1. Intended use; 2. 3 3. Height of building; 4. Number of dwelling units and density for multifamily site development plans; 5. For restaurants, entertainment or similar establishments, show number of seats and occupancy load; 6. Square footage for proposed development gross square footage, square footage of each story, etc; 7. Proposed location of freestanding sign; (all signage shall be by separate permit); 8. Type of building construction; 9. Any proposed fencing, walls, etc. (see "Landscaping"). C. Streets, sidewalks, driveways, parking areas, and loading spaces: 1. Engineering plans and specifications for streets, sidewalks, parking areas and driveways; 2. All parking spaces designated, including bicycle parking; 3. Number of parking spaces; 4. Number and location of handicapped spaces; 5. Number and designation of loading spaces; 6. Number of square feet of paved parking and driveway area; 7. Surface materials of driveways and cross sections of proposed parking, driveways, and/or street improvements; 8. Fire lanes; 9. Location of proposed driveway(s) and median cut(s) and listing of agencies or jurisdictions for permitting of same; 10. Traffic circulation plan including pavement marking and traffic controls; 11. Location of sidewalks, including handicapped access provisions; 12. Coordination of walkways, bikeways, driveways, etc., with facilities in adjacent developments; 13. All proposed streets and alleys; 14. Extension or construction of service roads and onsite access must be shown where applicable; 15. Traffic impact analysis when the proposed development is anticipated to generate 3,000 ADT's or greater or when determined required by city manager or his designee: a. The developer shall prepare, or have prepared, and provide the city with a traffic impact analysis; b. The analysis of traffic impacts will provide the following findings and appropriate methodologies utilized in determining the findings: (1) Total projected average weekday trip ends for the site in question; (2) Design capacity of the accessed road(s); St. Cloud, Florida, Code of Ordinances Page 2
4 (3) Average projected peak hour (including time of day) trip projections for the site in question; (4) Analysis of projected on and off site traffic patterns (i.e., turning movements); (5) Projected percentage of truck traffic; (6) Recommended improvements made necessary by the development; (7) Other related information as required by the city manager or his designee; c. The traffic impact analysis shall be prepared by a registered professional traffic engineer. D. Drainage. Engineering plans and specifications for collection and treatment of storm drainage, including a description of the preservation of any natural features, such as, lakes and streams or other natural features; any permits required by other agencies or jurisdictions. E. Dredge and fill. If any dredging or filling operation is intended in development of the area, the developer's engineer shall consult with the city manager or his designee to ensure conformity with dredge and fill requirements (see Article IX). F. Soils. 1. Indicate soil classifications on the site development plan as identified by the United States Department of Agriculture Soil Conservation Service in the county soil survey and Soil Survey Supplement. When multiple soils are present, a soil classification map shall be provided as an overlay for comparison with proposed development activities. An applicant may challenge this designation by securing competent expert evaluation, at the applicant's own expense, demonstrating that the identified soils are not classified correctly. If said determination is concurred by the city manager or his designee, the soils shall be correctly identified for the purpose of this code; 2. Soil analysis by a qualified soil engineer or firm shall be furnished upon request of the city manager or his designee. G. Erosion control. Provisions for the adequate control of erosion and sediment, indicating the location and description of the methods to be utilized during and after all phases of clearing, grading, and construction. H. Limits of floodplain. Indicate flood zone and community map panel number(s) per the appropriate Flood Insurance Rate Map(s) (FIRM) established by the Federal Emergency Management Agency (FEMA). If the development is in more than one flood zone, delineation lines shall be shown. I. Proposed water, reclaimed water, sewer, and electric facilities. 1. Water. Size, material, and location of water mains, valves, and fire hydrants, including engineering plans and specification; 2. Reclaimed water system. Size, material, and location of reclaimed water mains, and valves, including engineering plans and specifications; 3. Sanitary sewer system. Size, material, inverts of manholes, slope of the sewer, and location of lines, including engineering plans and specifications, with submittal of a profile where required. 5 J. Solid waste. 1. Location(s) and access provisions for refuse service, including pad, screening, fencing, and landscaping. 2. Types of service requested (Curbside Class II, Dumpster Class III, Recycling, Compactor). K. Landscaping, arbor, recreation, and open space. 1. Tree survey, including names of species, size at DBH and application for arbor permit, if applicable; 2. Landscaping plan, preliminary irrigation system plan to include water source, line location, in ground hook ups, and provision for maintenance including size, type, and location of all landscaping, screens, walls, fences, and buffers; 3. Recreation and open space areas, if applicable. L. City of St. Cloud Standard Detail Sheets. II. EXISTING IMPROVEMENTS (On-site, adjacent to site, and across or opposite any public rights-of-way.) A. Driveways and median cuts within 150 feet of the site in both directions on both sides of the street; B. Sidewalks, streets, alleys, and easements (note widths and type); C. Drainage systems to include natural and structural (size and materials, invert elevation); D. Locations of nearest water mains, valves, and fire hydrants (size and materials); E. Size and locations of valves for reclaimed water mains; F. Sanitary sewer systems (size, invert elevation, etc. to be included); G. Gas, power, and telephone lines, where applicable; H. Existing electric services. III. OTHER (Any additional data, maps, plans, or statements, as may be required, which are commensurate with the intent and purpose of this code.) A. Development agreement. If determined necessary by the city to secure the future performance of any conditions imposed by the city or representations made by the developer, an executed development agreement in a form acceptable to the city may be required. In such an event, the developer shall be required to pay all costs involved in the preparation and recording of such agreement. B. Additional required submittals only where facilities are dedicated to the public. The approval of the site development plan shall, where facilities are dedicated to the public, be made only pursuant to certification of adequacy of the following list of required submittals by the city manager or his designee and/or city attorney, as appropriate; 1. Title Certificate. A certificate of ownership signed by a licensed attorney-at-law or an abstract company licensed to do business in the state shall be required when property is conveyed to the city, in form approved by the city attorney, and showing: St. Cloud, Florida, Code of Ordinances Page 4
6 a. Parties executing site development plan are owners of the land embraced by the site; b. All mortgages, liens, or other encumbrances; c. That all taxes and assessments are paid to date; d. Description of site is correct; e. No conflicting rights-of-way, easements, or site development plans exist; 2. Bonds. The approval of any site development plan shall be subject to the applicant guaranteeing the installation and maintenance of the required improvements, where these facilities are to be conveyed or dedicated to the public, by filing a performance bond or bonds executed by an approved surety company in the amount of 110 percent of the construction costs, including landfill. Maintenance bonds shall be in the amount of ten percent of the construction costs. Costs for construction shall be (1) estimated by the applicant's engineer, or (2) a copy of the contractor's bid shall be provided. All cost documents shall be signed and sealed where appropriate. The amounts of the performance and maintenance bond must be approved as adequate by the city manager or his designee and/or the official responsible for utility services. In lieu of performance bonding, improvements may be installed following site development plan approval and preceding issuance of the certificate of occupancy, subject to the approval of the city manager or his designee and/or the official responsible for utility services. In all cases where public improvements are installed prior to issuance of the building permit, a maintenance bond as described in the Appendix to Article V must be submitted to the city manager or his designee. A completed maintenance or performance bond form shall be submitted when utilizing a cashier's check or certified check as the form of guaranty in lieu of a surety. A certificate of occupancy cannot be issued until the maintenance bond is approved and accepted by the city. Bonding requirements may also be met by the following, but not limited to: a. Escrow deposit Cashier's check Certified check Irrevocable and unconditional letters of credit b. Notwithstanding anything contained in this section, bond requirements may be met by the use of other submittals as approved by the city council. C. Concurrency. 1. Concurrency application; 2.
