2 Birthday Party Specials It s always fun at the YMCA! Take advantage of this great package deal and let the YMCA be the host of your party. Included: 1 hour room rental 1 hour activity of Play Time Total of 2 Fun Filled Hours Party Host- On Request 30 min set-up/ clean-up (before and after party) Not included: food/birthday cake, utensils, invitations, swimsuits/towels, serving bowls/plates Rooms available: Downtown Y: Indoor Pool, Conference Room or Zone Northwest Y: Gym, Studio, or Zone Price: Maximum 25 guests YMCA Member $100 Community Participant $200 Who counts as a guest? Anyone ages 2 to 102 years of age If you've selected the Downtown Pool as your activity, you may add the Snail Slide for $50. Important: Our staff can lead group games OR you may choose open play. Gym availability is seasonal. Parties are welcome to bring in any type of food they choose. Rental agreements and deposit must be turned in no later than one week prior to event date. Agreements turned in less than one week prior to your event will not be accepted. This birthday party special is for small groups of people. The numbers and cost mentioned above are enforced. For larger groups, please see "General Facility Rental Information". Pizza Option: Papa John s Cheese, pepperoni, sausage $6.00 per pizza + $8.00 delivery fee and tip.

3 General Facility Rental Information Facility rentals are open both to YMCA members and Community Participants The following areas are available for rentals: Downtown Y Pool (Sat & Sun only, 1 hour max) Gym, The Zone Conference Room Northwest Y Gym The Zone or Studio Basketball Courts Tennis Courts (Only for After Prom) If you've selected the Downtown Pool as your activity, you may add the Snail Slide for $50. After-hours rentals available Conference Room during Business Hours $25.00 an hour. To ensure proper staffing, all rental agreements must be turned in a minimum of one week prior to rental date. Agreements turned in with less than one week notice will not be accepted. A $25 deposit is needed to reserve a rental. A $100 deposit is needed to reserve an after-hours rental. Rental fees must be paid in full by the Friday morning prior to event. Rentals will not be honored if payment is not received and the deposit will not be refunded. Should any damages take place, or should the YMCA need to spend extra time cleaning up after your rental, you will be charged accordingly. Please provide one adult chaperone per ten participants.

4 After Hours Rental Information (Facility Rentals are open both to YMCA members and Community Participants) Downtown Y Pool Gym Conference Room The Zone Northwest Y Basketball Courts Tennis Courts Studio The Zone Price for 1 hour Maximum 40 guests* YMCA Member $100** Community Participant $200** If you've selected the Downtown Pool as your activity, you may add the Snail Slide for $50. *$5 for each additional person after the maximum of 40. **Additional hours: $100 for members $200 for community participant Lock-In and After Prom Information Group Rate: $500 Lock-Ins and After Prom are available for groups of up to 200 participants. There will need to be adequate adult supervision for your group. The YMCA will have staff present for the safety of your group. Details, times, and activities are flexible. 5 Rental Agreement Food and beverages must remain in The Zone or Conference Room All participants and supervisors must follow the directions of YMCA Staff. Alcoholic beverages, illegal substances, or tobacco products are prohibited on the YMCA premises. Product solicitation prohibited. Damages to property will be the responsibility of the renting participant. Staffing will be provided during your event for your assistance. Completed rental application must be signed, and confirmed by YMCA director. Participants may arrive ½ hour prior to party and have ½ hour to clean up after the party Participants are expected to stay in areas included in the rental agreement. The YMCA reserves the right to cancel the rental in advance; however, if the YMCA dismisses a rental for violation of agreement, no refund will be issued. I, the undersigned, being over the age of 18, in consideration of being permitted to utilize the facilities, services, and programs of the YMCA of Muncie, Inc. as a spectator or participant in any on-site or off-site programs, or the use of any facilities or equipment, hereby forever release, waiver, acquit, discharge and agree to hold strictly harmless the YMCA, its agents, employees, and liability arising out of or claimed to have been incurred or sustained by the undersigned, while the undersigned is in, at, upon or about the premises or while engaged in any activity within any or the facilities, or participating in any program, or using any equipment, owned, operated, maintained, or affiliated with the YMCA. Photo Release I, authorize the YMCA of Muncie permission to use any photographs taken of me in any and all publication and other media/mediums and for any other purpose and manner without restrictions, whether now known or hereafter existing. Medical Authorization In the event of any emergency, I hereby authorize and give permission to any physician, hospital, or other healthcare provider as may be designated by the YMCA, in its discretion, to transport, treat, hospitalize, provide emergency medical treatment for, and to order, authorize and administer injection(s) anesthesia or surgery to me.
