2 ANNEXURE I INSTRUCTION FOR FILLING UP THE FORMS BIDS FOR OFFERING PREMISES ON LEASE The following are available on Bank's website i.e. under procurement news: 1 Detailed Terms & Condition which have to be accepted by owner of the premises & submitted along with the Technical Bid. 2 Format on which Technical Bid is to be submitted 3 Format on which Financial Bid is to be submitted i. All the forms viz. Technical and Financial Bids to be filled in a legible hand. ii. Technical and Financial Bids to be put in separate sealed envelopes for each premises. The envelopes are to be super scribed as Technical Bid for (Branch Name) and Price Bid for (Branch Name). iii. The Name, address, mobile and telephone number of the bidder to be mentioned on each envelope. iv. The rates should not be mentioned neither in the Technical Bids nor in the Terms & conditions letter. v. A copy of terms & Conditions letter duly signed in token of your acceptance of the same to be enclosed along with the Technical Bid. vi. Please note that Bids received with changes/amendments in the standard Terms & Conditions are liable to be rejected. vii. Technical Bid not accompanied by duly accepted Terms & Conditions letter as enclosed herewith are liable to be rejected. viii. If the Bids are submitted by an authorized agent they should be countersigned by the owner or an authority letter specifying clearly the powers & responsibilities of the agent to be enclosed. ix. Site plan of the area offered to be enclosed with the Technical Bid. x. We clarify that only the built-up area is to be mentioned. xi. 4 Name : Address : Mobile : Land line no. : ID : Date : The Assistant General Manger State Bank of India Regional Business Office-I A-3 Industrial Estate Patrika Road Kota Dear Sir, LEASING OF PREMISES AT : ACCEPTANCE OF TERMS & CONDITIONS With reference to your advertisement dated I/We agree to the following terms : i) I/We will hand over the possession of the building after getting it constructed / renovated as per Bank's requirements. The rent will be applicable from the date of physical possession of the building complete in all respects to the entire satisfaction of the Bank to be verified by Bank's Engineer / Bank's competent authority and receipt of Completion Certificate / Building use Certificate. I/We will obtain Completion Certificate /Building use Certificate from the concerned authority and rent will be start after handing over theses certificate to the Bank. ii) I/We offer Sq. feet built-up area having on Ground Floor. No rent will be charged for parking area in front / side / back of the building and basement parking. iii) All taxes present and future i.e. House Tax, Property Tax and any other government, Semi- Government, local body Taxes & Society Services Charges and Maintenance Charges to Local Welfare Committee etc will be borne by me/us. iv) I/We will construct/ renovate the building as per the Bank's requirements i.e. locker Room, Record Room, Cash Room, Server Room, UPS Room including ATM/E-Lobby room as per plan approved by the Bank and a ramp for disabled at the entrance of around 3' width to be provided along with SS Railing in addition to steps. v) I/We will construct a RCC wall of 12 thick reinforced with 12 mm tor 150mmc/c both ways place staggered, Ceiling & floor slab-12 thick reinforced with 12mm tor steel at 150mmc/c both ways placed staggered for Locker room and Locker room floor slab to be strengthened to take the load of locker safes as per the Bank's/RBI specifications. Cash Room will be 6 thick RCC wall or 9 thick brick was as per plan. I/We also provide Cash room door, Record room door, Toilet block door, Pantry room door, UPS room door etc as per specification provided by Bank. However, Locker room door and ventilators to be supplied by Bank but it should be fixed by me/us.

5 vi) The lease period will be 20 years & lease agreement will be executed for an initial period of 10 years with option in bank's favour to renew for further period of 2 block of five years (not more than 10 years) with condition to enhance in rent after completion of every five years from the date of 15% to 20% as decided by both the party at the time of negotiation. After completion of 20 years revised rent will be decided with mutual consent of both the parties. vii) I/We will paint front elevation and all external Walls/ ceiling with plastic paint of approved shade, one coat before applying POP and second coat after applying POP and thereafter, as per the Bank's instructions, shall be done after every 3 (three) years failing which the Bank shall be at liberty to get the same done at your risk and as pr actual cost. Whitewash, color wash, paint and or varnish all such parts thereof once in 3 years and to repair or update and fit all the connections of electric installations and carry out other essential repairs and in default the Bank shall be at liberty to execute such repairs, color or white washing, painting varnishing etc. after giving 15 days notice and Bank can recover the costs from the rent to be paid to me/us. The repairs which are in the nature of day to day maintenance will be carried out by me/us at cost thereof will be paid by me/us. viii) I/We will at my/our cost, arrange for separate three phase electricity meter/sub meter having three phase electric connection from 30 to 45 KV of load depending on actual requirement of the Bank and electric points including power/ac points required for the Branch as per Bank's requirements. Actual electricity consumption charges and water charges shall be borne by the Bank. ix) Necessary arrangements for continuous water supply and independent underground and overhead water tank of sufficient capacity will be provided along with electric pump for lifting of water to have continuous water supply. x) Proper sewerage connection/drainage shall be arranged by me/us for the Bank. xi) All internal & external doors (except locker room door & ventilator) and windows shall be provided by me/us with M.S. Grill 12mm Sq bars placed at 3 c/c both ways as per specification provided by Bank & aluminium section glass door or stiffen Glass as suggested by Bank and cost will be borne by me/us. xii) All the expenditure of the lease deed will be borne by me/us and Bank in ratio 50:50. xiii) The Bank shall be at liberty to fix and/or remove at any time strong room doors steel fittings, iron doors fixed to the safe room, electric fittings, expanded metal compartments, fixing wooden partitions, electrical fittings, air conditioners, exhaust fans, counters, temporary structures, other fittings etc., as per the needs and requirement of the LESSEE and /or make such other conditions and alterations on the premises which will not affect the permanent structures in the said premises, during the continuance of the said term of lease or within a reasonable time of the expiration thereof. xiv) I/We will provide vitrified tiles flooring (Johnson & Johnson) (salt & pepper shade) having size 24 x24 in the Bank Hall and marble?ceramic tiles of size 12 x18 with highlighter on walls up to a height of 7' and 12 x12 on floor in toilet area and 1 st class bathroom fittings including wall tiles as suggested by Bank's engineer. As per Provision,I/We will provide (1) Gents Toilet (two urinals, one European W.C., oval shaped washbasins over granite stone slab counter and other modern accessories), (2) Ladies toilet (one European W.C. One washbasin and other modern accessories. Sanitary fitting of first quality such as PARKO/ZIM/JAQUAR or equivalent should be provided in the toilets. I/We will provide Parking space and inner pathways with Paver Tiles in required design and shades.

6 xv) I/we will arrange to provide all sanitary fittings as per requirements of the Bank. xvi) This offer is subject to the obtaining No objections certificate/building use certificate by me,us from the appropriate authority regarding commercial use of the proposed building. xvii) /We will have no objection for putting up VSAT/Antenna/pole/tower (of any height) as per Bank's requirement on the terrace, ATM/CDM/Swayam etc machines in the premises and to install core banking servers and other accessories, Generator Sets (any fuel), Air conditioners without any additional charges. I We agree to provide necessary/additional power load from the Electricity Board/Electricity Dept. Consent sought by Bank, if any, shall not be unreasonably withheld in the case of above such alternations as shall be necessary or required by the Bank for the purposed of carrying on its business. I/We give roof right for installation of VSAT/Antenna/pole/tower (of any height) as per Bank's requirement on the terrace/ground as may be required by Bank at free of cost. xviii) I/We will carry out structural civil, sanitary and electrical repair/maintenance works, if required at my cost and I will also ensure the roof remains water-tight. In case the above repairs are required and I/We fail to attend to the same, the Bank will carry out necessary repairs at my/out risk and cost. xix) There shall be no objection if a glow sign in front or at top of the building & put up. Affix, display and maintain such sigh-boards, placard, posters, advertisements, neon signs in installed by the Bank as per the Bank's requirement. xx) I/We will give approx.100sq ft. of pucca open/covered are for placing generator dpending on Bank's requirement or more at roof top or any other place approved by the Bank for which no rent will be charged. xxi) Main door-rolling Shutter of heavy with collapsible grill door and iron door for cash room will be provided by me/us as per specification provided by Bank. xxii) The Bank shall have the right to vacate/de-hire/sub-lease in full or part at any point of time by giving one month's notice. This option will not be available to the owner/s. xxiii) Record/stationery room, cash Room, UPS room, Server room, kitchen/pantry and ATM Room will be constructed with 9 thick in brick, duly plastered, on both sides. Brick walls and locker room will be constructed with RCC. The kitchen/pantry should be provided with cooking plot form of 2 0' side with a kitchen sink. xxiv) The Flooring of Banking hall & other rooms should be provided with vitrified tiles. All the rooms Double polish Kota stone. The shade/quality approved by bank. xxv) I/We will produce/submit the structural stability certificate from licensed structural consultant regarding safety and withstand load before handing over possession to the bank on account of bank's records if necessary. xxvi) That in the event, I/We deciding to sell the demised premises, during the tenancy or at the expiration of the same, I/We shall in the first instance offer them to the Bank at the lowest price which is acceptable to me and the Bank shall within one calendar month from the date of receipt of such offer accept or reject such offer. If Bank rejects such offer, I/We may sell the premises but Bank will continue to occupy as per lease agreement.

7 I undertake to construct the building under the supervision of a qualified Architect in accordance with the above specifications before handing over the possession to the Bank. A certificate to this effect and Strong Room Certificate will be provided by me duly signed by the Architect. In case it is found at any stage after the building is taken over by the Bank that any of the above work has not been executed, I undertake to carry out the same on my cost.
