1 Rental Application form Date: Address Applying For: Main Terms: On Vacant properties the start date for any properties shall be within 5 days of approval of the tenant by the Landlord/agent. End dates Shall be that which is approved and accepted by the Landlord. Application Fee: I/We acknowledge an application fee of $50.00 is being charged in connection with my/our application and that the fee is non-refundable regardless of acceptance of application. This is in addition to the security deposit and/or last month s rent required to lease the property. *All adults intending to live at the property need to fill out an application per person & pay $50.00 dollars for each application* Receipt of Deposit: The additional Deposit of $ is due at the time of application. If applicable our pet policy includes a $ nonrefundable pet fee with an additional $ per month in pet rent. If the Applicant(s) has pets the $ nonrefundable pet fee will be collected at the time of application. The security deposit and pet fee will be paid separate from one another. This deposit and pet fee is received subject to Agent s approval and acceptance of this rental application submitted by Applicant(s). This application is not considered a lease nor is there a binding contract to rent the premises until all parties sign a lease. If Agent approves the application, Applicant(s) shall then promptly sign a lease for the premises within a 48-hour time frame. At the time of Agent s approval of application the above deposit will be applied towards the full security deposit required under said lease. In consideration of Agent s promise to fully review the application and complete further processing, once application is approved, Applicant(s) acknowledge full understanding that deposit is not refundable should Applicant(s) elect not to execute lease or occupy premises as agreed. Applicant(s) shall cooperate with application process if additional information is necessary. If Applicant(s) are not approved to rent the premise mentioned above a full refund of the security deposit and pet fee will be issued. If Applicant(s) is approved pending receipt of completed a notarized Co- Signer Application, it must be received in our office within 48 hours from the time the Applicant(s) is notified of this status. If Co-Signer Application is still not received, the holding fee shall be returned to Applicant(s) along with an Adverse Action Letter stating property is no longer being held for Applicant(s).

2 If Applicant(s) is applying for the rental unit site unseen, Applicant(s) acknowledge the inherent risks involved with such action. If Applicant(s) are approved for the rental, Applicant(s) agree to execute a lease on the subject unit/home AS IS stated above. Tenant understands that any representations made by the agent are of opinions and guestimations. Tenant agrees to do their own research outside of Chamberlin Investment Group, LLC before they sign the lease or apply for the property. Information Disclosure Authorization I/We hereby authorized you to release to Chamberlin Property Management, LLC, for verification purposes, information concerning: Employment history, dates, title, income, hours worked, etc. Banking and savings accounts of record. Mortgage Loan Rating/Rental History (opening date, high credit, payment amount, loan balance, payment record, monthly rental rate, condition of property at check-out, etc.) Any information deemed necessary in connection with a consumer credit report for a real estate transaction. This information is for the confidential use of Chamberlin Investment Group Property Management, LLC. and the property owner in compiling a rental application credit report, as well as rental, employment and credit history. A photographic or scan of this authorization, (being a photographic or scanned copy of the signatures of the undersigned), may be deemed to be the equivalent of the original and may be used as a duplicate original. Is the animal a service animal? Do you currently work from home? (Our lease stipulates that the property will only be used for residential use. businesses may be run out of the unit without prior authorization.) How many times a month would you typically host a party / get together with friends etc? Please write the number of nights you typically host events at home? Of 30 Are you and the other potential tenants on the lease students? Do you currently smoke cigarettes? Do you currently smoke Marijuana? Do you grow Marijuana? Are you comfortable having a contractor visit your home while you are not there to fix a repair? Are you handy and comfortable conducting minor repairs or fixes on your own? We conduct periodic inspections to check the safety of the property (smoke alarms, tripping hazards etc ) Are you open to allowing us to conduct these inspections while you are not home? We begin pre-leasing of our units in January. This means if your lease is not renewed for another year, showings will begin in January and will continue until the unit has been pre-

4 leased. Will this pose any problems for you? With the above statement about our pre-leasing policy, we have a 2 hour showing notice. This notice is not often used, as we like to give our tenants as much notice as we can. However are you willing to work with us, if this 2 hour notice has to be enacted? If you choose to not extend your lease in January, are you willing to work with us to help re-lease the unit as quickly as possible by keeping your unit in showing condition? (clean and tidy) *Have you ever had any problems with the return of your security deposit? *Have you ever been involved in a litigation regarding your tenancy at a rental unit? What is the square footage of the unit that you are moving from? An overabundance of furniture or personal items limits accessibility poses a safety risk to you and inhibits acceptable use of the property. Would you be willing to send us pictures of some of your furniture / personal belongings to make sure that they will comply within our acceptable parameters? Every unit has the possibility to be sold during your tenancy, are you comfortable with us granting real estate agents, inspectors and appraisers access to the unit? / With 2 hour notice for the unit to be in showable condition?
