2 The Neighborhood Improvement Program requires that you provide a minimum of $90 and up to a maximum of $360 (it all depends on the cost of your home improvement repairs) of your own funds for the homeowner match requirement. Can you provide these funds? Yes / No The Neighborhood Improvement Program requires that you provide a certain number of hours of Sweat Equity Hours working on home improvement project and/or resolve any city code violations if there are any. Homeownership Workshop Each qualified applicant will be required to take part in a Post Purchasing Counseling workshop where such topics as: financial debt management, foreclosure prevention and homeowner responsibilities will be discussed. This workshop will be arranged by Neighborhood Nonprofit Housing Corporation with date to be announced. Funding Recapture The $9,000 grant does not have to be repaid if the house remains owner-occupied for five years. If the home is sold to a buyer that meets certain income requirements during the first five years, the grant may be able to be transferred to the new buyer. If these conditions are not met, the grant must be repaid in full if sold or refinanced within the first five years. A copy of your most recent pay stub (or other documentation for other sources of income) showing year to date income for all members of your household over age 18 residing at the home on a full-time basis. A copy of Federal Tax Returns for the past two years for all members of your household over age 18 residing at the home on a full-time basis (first two pages only). Copies of latest W-2 (or 1099) forms for all members of your household over age 18 residing at the home on a full-time basis. One of the following documents to show ownership of your home: -A copy of your deed showing title to your home. -A copy of your payment coupon from your mortgage company. -A copy of your latest tax notice from Box Elder County. 4 QUALIFIED EXTERIOR IMPROVEMENT FORM Total improvement costs can vary depending on which individual home projects for each home are deemed more critical and needed. The maximum funding for each home will be $9,000. Each qualified household will be required to participate 1% to 4% in a matching fund requirement (this amount will be based on the cost of the improvements). In addition to the improvements listed below, you will be required to resolve any Brigham City s code/public nuisance violations, including removal of weeds, unlicensed vehicles, fire hazards and clear view obstructions. A Brigham City Inspector will do an on-site inspection to determine if there are any violations that must be dealt with. Please check only the improvements you believe are necessary on your home and rate them in order of preference. Neighborhood Nonprofit s Construction Supervisor will use the information to prepare a visit, a scope of work and a cost estimate. ELECTRICAL SERVICE Replacement of electrical service that does not meet International Building Code requirements RANK ROOFING REPAIRS Replacement of cracked, peeling, curled or lost shingles Replacement of worn or aging soffit and/or fascia Repairs to a leaking roof EXTERIOR WALLS Repairs and painting of exterior wood siding and trim Installation and/or replacement of substandard siding EXTERIOR WINDOWS & DOORS Repairs/replacement of broken windows Replacement of windows that do not meet the Model Energy Code Replacement of substandard exterior doors CONCRETE/WOOD WORK Replacement of broken concrete walkways, driveways, and front porches Installation of ramp for wheelchair accessibility OTHER EXTERIOR REPAIRS Explain 4

5 NORTHERN UTAH S NEIGHBORHOOD IMPROVEMENT PROGRAM LETTER OF AUTHORIZATION The home owner(s) certifies that all information given to Brigham City and Neighborhood Nonprofit Housing Corporation in order to qualify for Northern Utah s Neighborhood Improvement Program is true and complete to the best of the home owner(s) knowledge and belief. Sources and amounts of income, homeownership status, and loan documents may be verified by Brigham City and Neighborhood Nonprofit Housing Corporation. The home owner(s) will at all time(s) hold Brigham City and Neighborhood Nonprofit Housing Corporation harmless. Brigham City and Neighborhood Nonprofit Housing Corporation will not, in the provision of services, or in any other manner discriminate against any person on the basis of race, color creed, religion, sex, national origin, age, familial status, disability or any other protected classes. In accordance with funding requirements from Federal Home Loan Bank of Seattle; seniors (individuals over the age of 62), persons with disabilities and single parents may be given preference in receiving grant awards. 7 NORTHERN UTAH S NEIGHBORH00D IMPROVEMENT PROGRAM PARTICIPANT GUIDE (Do not turn in; keep this guide for your information) Attend the orientation meeting to receive an application packet. Confirm that you meet the program criteria explained to you in the meeting and as shown below: The home must be within Brigham City limits and meet the following criteria: Existing single family detached homes are eligible. Mobile and modular homes, condos, twin homes, town homes and new construction are not allowed. Your annual pre-tax combined income for all members of your household over age 18 residing at your home on a full-time basis cannot exceed the HUD income limits, as shown below: Household Size Annual Income 1 $26,640 2 $30,480 3 $34,266 4 $38,040 5 $41,100 6 $44,160 7 $47,220 Household income is the gross amount (before any taxes and deductions) of wages and salaries, overtime pay, commissions, tips and bonuses for all adults of the household, as well as the gross amount of all unearned income (example SSI) from all members of the household. Submit your completed application with all documentation in a sealed envelope addressed to Brigham City Hall, 20 North Main, Brigham City, Attn: Community Development Department. All applications are due and must be submitted to Brigham City Hall by 5:00 p.m. on May 2, Late applications will not be accepted. A complete application includes the following: 1. Application materials consist of: Application Form Qualified Exterior Improvement Ranking Form Letter of Authorization Demographic Information 7

Must provide one of the following documents to show ownership of your home: A copy of deed showing title to your home A copy of your payment coupon from your mortgage company A copy of your latest tax notice from Box Elder County. (If you pre-qualify for BRAG s Weatherization Program, you will be contacted and asked to fill out a separate application with supporting documentation. You must provide documentation of all sources of income from the previous month in order to be pre-qualified for this funding.) The process of selecting qualified homeowners will be determined on how they rank based on the scoring criteria system once all applications have been turned in. Points will be awarded based on; income level and family size with additional points awarded based on whether there is a family member with a disability, a single parent head of household or a senior citizen. A site visit will be performed by a Brigham City Inspector to check for code and public nuisance violations. Neighborhood Nonprofit s Construction Supervisor will prepare a scope of work detailing the work to be completed and the estimated cost. Once these inspections are completed, you will be contacted by a Neighborhood Nonprofit Housing Corporation representative and an appointment will be made to discuss the results of the inspections, scope of work and to complete your sweat equity requirement. At this appointment you will be required to provide the homeowner funding match, which is calculated based on your income level and the amount of funding you will receive. See the chart below for exact requirements: Income Level Match Requirement 50 to 60% AMI 4% of funding (i.e. $9,000 = $360) 40 to 49.9% AMI 3% of funding 30 to 39.9% AMI 2% of funding Below 30% AMI 1% of funding After your meet with Neighborhood Nonprofit, you will complete your sweat equity hours. Sweat equity requirements are calculated based on the anticipated funding you will receive. For example: Funding Amount Sweat Equity Requirement $4, hours $5, hours $6, hours $7, hours $8, hours $9, hours 8

9 You must complete the sweat equity requirement within 45 days of the final application to remain eligible for the funding. You will be given a Sweat Equity Form that will list the work to be completed and the date it must be completed. Each qualified homeowner will need to complete Post-Purchase Homeownership Workshop which will be arranged by a Neighborhood Nonprofit Housing Corporation representative. This workshop will need to be taken before any home improvement work can be started. Once all of the items above are completed, the qualified work on your home will be scheduled. A deed of trust will be recorded on your home for the amount of funding used on your project. This deed states that the funding does not have to be repaid if the house remains owner-occupied for five years. However, the funding must be repaid if the owner refinances a current mortgage, rents the home, or sells to a non-qualified buyer within that five year period. Enjoy your home improvements!
