Signed disclaimer/release and affidavit Other: REQUIREMENTS/ STIPULATIONS LISTED BELOW REGARDING SPECIFIC ITEMS. PAINTING: You will need the following if you are requesting to paint your home. Please remember you cannot start any work on your house until you get HOA approval. Completed ARC form with brief description section completed. Copy of your contractors valid Florida business license, proof of liability insurance and workers compensation. If you choose to paint your house on your own you must state that in the brief description section. In the brief description section of the ARC form you must put the following. Color of the body of the house (paint color name and paint color number), color of house trim (paint color name and paint color number), color of your front doors and garage doors (paint color name and paint color number) Copy of a picture of your house with arrows showing what color goes where. Copy of Picture of your neighbors home(s) to the left and right showing color of house and trim. Homes next to each other may not be painted the same color. Copy of picture of color swatch/patch labeled showing what each swatch/patch will be painted. (i.e. body, trim and doors). Copy of the signed contract with your contractor. Any additional information that the ARC form requires. 2

The approval of the homeowners association should not be construed as an approval of any state or local building code requirements. For any encroachment(s) To comply with the conditions of acceptance, (if any); and to complete the project according to the approved plans. If the modification is not completed as approved, said approval will be revoked and the modification shall be removed by the owner at the owner's expense. Applicant further acknowledges that drainage swales have been designed and established between home (side yard) to carry storm water off the lot and maintain positive drainage away from home. The association and/or developer shall not be responsible for any associated cost incurred. The homeowner is responsible for all associated costs. The homeowner is not to disturb the slopes of the lakes in any way, shape or form. For installation of pool, extension of patio, addition of screen enclosure or construction of an addition to your home, the homeowner must have the property inspected prior to any work commencing at the cost of $200 paid out of the $4,000 deposit to Caulfield & Wheeler; and inspected at an additional cost of $200 also paid out of the $4,000 deposit to Caulfield & Wheeler when the work is completed. A final approval must be signed off by Caulfield & Wheeler. I also understand that the ARC does not review and assumes no responsibility for the structural adequacy, capacity or safety features of the proposed construction, alteration or additions; or for performance, workmanship, or quality of the work of any contractor or of the completed alteration or description. I agree to abide by the decision of the architectural review board or board of directors. If the modification is not approved or does not comply, I may be subject to court action by the association.
