(561) Purchase/Lease Application Checklist Below is a list of items needed to process association approval to Purchase/Lease a home. All payments must be certified/cashier s check or money order ONLY (no personal checks or credit/debit cards accepted.) Please indicate with a check mark that the needed items are enclosed. Applications cans be mailed or dropped off to our office address above. Failure to provide all information and payment will result in application being returned. Incomplete applications will not be accepted. NEEDED ITEMS: Executed Purchase/Lease Application Copy of the purchase/lease contract Legible copy of driver license(s) for all adults that will be living in the home $150 Non- refundable application fee made payable to Alden Ridge Homeowners Association, Inc. $250 Non-refundable fee made payable to Tallfield Management * For Leases Only: A $500 refundable lease deposit (paid by owner) made payable to Alden Ridge Homeowners Association, Inc. Two-page screening and authorization forms per adult (required to run national criminal/credit check) Pet registration form for each pet Acknowledgement page Rules and Regulations Form A Personal Interview of the prospective purchaser, lessee or occupant may be required by the Association Screening or Investigative criteria is available from the Association upon request. Maintenance assessments, estoppel fees and property violations must be taken care of by the Owner at or prior to closing and/or prior to any change in occupancy OR a prospective buyer may assume such obligation to cure outstanding property violations after closing provided that such assumption is in writing and upon terms and conditions acceptable to the Association. An application to lease will not be accepted for processing unless such application first complies with the Association's rules and regulations on leasing of Lots which govern when an Owner may lease their Lot and the number of Lots that may be leased In Alden Ridge at any one time. *NOTE: The $250 fee covers background screening for up to two adult applicants. Any additional residents 18+ years of age are subject to background screening, and an additional $50 fee per adult is required. Application must be submitted a MINIMUM of 30 days prior to intended occupancy. Occupancy prior to approval is strictly prohibited. Processing can take up to 30 days from when a complete application is received. Page 1

3 I/We understand and agree that restrictions in the Governing Documents provide, among other things, that I/We only use my/our Unit or Lot for residential purposes and no business use is allowed. I/We agree to observe and abide by the terms and conditions stated in all of the Governing Documents of the Association. I/We understand that any change in the occupancy of a Unit or lot by someone other than the owner of record and his/her/their family members requires that we submit this application. If I/We are to reside in the property of more than thirty {30) days in any twelve month period starting from the first day I/we occupy the Unit or Lot then I/we understand that my/our occupancy will be considered a lease by the Association under its Governing Documents even though I/we are not paying rent of any consideration of value to the owner. 6 2) A non-refundable processing fee of $250 for up to two adult applicants (each adult $50 thereafter) made payable to Tallfield Management must accompany the application. $150 Non- refundable application fee made payable to Alden Ridge Homeowners Association, Inc. 3) A copy of a valid driver s license for all adults who will reside in the residence. 4) For Lease Only: A $500 refundable lease deposit (paid by owner) made payable to Alden Ridge Homeowners Association Inc. 5) Adult applicant will have a national background check and a consumer credit report run by Tallfield Management. Each adult must fill out the two-page screening/authorization forms. I understand that the Association has adopted written criteria for reviewing these reports and that I may obtain a copy of these criteria from the Association upon signing this Application 6) Applicant(s) agrees to comply with all By-Laws and Rules & Regulations of Alden Ridge Homeowners Association Inc. 7) All pets must be in compliance with the pet rules and regulations. 8) That Applicant(s) agree on behalf of all persons who may use the Unit, which they seek to purchase /lease / occupy for themselves, to abide by Association's Governing Documents, including the Declaration of Covenants and Restrictions for Alden Ridge, the Rules and Regulations, any and all amendments thereto, and all applicable Florida Statutes. 9) That no persons other than those listed on this Application will reside in the Unit and Applicant(s) and Owner (s) agree that anyone moving into the Unit at a later date will be registered with the Association and a background investigation and credit check done at the Applicant's expense. 10) That Owner(s) hereby authorizes the Association, through its agents and or assigns, to enter upon the Lot and Unit to conduct an inspection to ensure compliance with the Governing Documents and such entry shall not be deemed a trespass. 11) Owner(s) hereby authorize the Association to evict a Lessee at Owner(s) expense, as the agent of the Owner, in any case where the Lessee falls to abide by Its Governing Documents. This authorization shall also apply to any occupant of a Unit or Lot who remains or proposes to be in residency in the Unit or Lot for more than thirty (30} days in the absence of the owner and/or members of owner's family. Any evictions proceedings shall follow the procedures set forth in Chapter 83, Florida Statutes. 12) That Owner(s) are responsible for the lessee and/or guests of such Lessee, the occupant and/or guests of the occupant in regard to violations of the Governing Documents, including fines, and damages to common area or improvements to the common area. The Owner also agrees to reimburse the Association for any fees paid to the Association's attorney relating to such violations including fines by these persons to enforce the Governing Documents whether or not suit if filed by the Association. 7 13) The parties hereto understand that the Association has restrictions on leasing and that those restrictions including a requirement that an owner have title to a Unit or Lot for at least 2 years prior to leasing it. There is also a limitation on the number of units that may be leased within Alden Ridge at any one time. When computing the number of leases the Association is also including those persons who are occupying a unit for more than thirty (30) days in any 12 month period in the absence of the owner and/or members of the family of the owner. The parties understand and acknowledge that this Application may be denied for failure to comply with these restrictions. 9 Alden Ridge Homeowners Association Inc. Screening Criteria The Association shall utilize the following criteria in the approval/disapproval of all prospective tenants, purchasers, or occupants of Lots' as set forth in the Declaration of Covenants and Restrictions for Alden Ridge: 1. All applicants must not have a record of financial irresponsibility, including without limitation prior bankruptcies, foreclosures, evictions monetary judgments or bad debts, and the applicant must appear to have adequate financial resources available to meet his/her obligation to the Association. 2. All applicants must have an average credit score of at least 695 for approval; provided however, the Association, in its discretion, may approve an applicant with a lower score based on reasonable explanation. Exceptions: Foreign Nationals with no credit rating in the United States of America, however, such applicants will be required to produce a valid Visa or Passport. As it relates to the approval of a lease of a Unit, persons other than Foreign Nationals who have no credit rating must have a co-signer or the lease agreement and the co signer must submit an application, application fee and otherwise meet the Association's established criteria. For doubtful sale situations, in the Association's discretion, a one-year prepaid assessment fund may be held in escrow to secure the payment of assessments. 3. The Association shall disapprove if any person seeking approval has been convicted of, or pled guilty or no contest to a felony involving violence to persons or property, sale, distribution, or use of controlled substances, or a felony demonstrating dishonesty or moral turpitude or has been charged with any such felonies and the person was not acquitted or the charges were not dropped; including any person with status as a registered sex offender or sexual predator in any state. Notwithstanding the foregoing, the Association may, in its discretion, disregard convictions or charges which occurred ten {10) years or more prior to the date of the applicant's application. 4. The Association shall disapprove if the Owner has a history of leasing his/her Unit without obtaining prior approval from the Association and/or refusing to control or accept responsibility for the occupants of his Unit. 5. The Association shall disapprove if the application or any documents obtained from the applicant on their face indicate that the person seeking approval Intends to conduct himself or use the Lot in a manner inconsistent with the covenants and restrictions applicable to the Association, including, but not limited to, county occupancy restrictions. 6. The Association shall disapprove if any of the person(s) seeking approval failed to provide the information requested, fees or the personal interview required to process the application in a timely manner or included materially inaccurate or materially false information in the application, including, for example, prior residential occupancy problems involving unreasonable disputes and/or litigations against one's neighbors or prior associations. 10 7. The Association shall disapprove if any of the persons seeking approval have a history of conduct which evidences disregard for the rights and property of others. 8. The Association shall disapprove if assessments or any other charges against the Lot and/or Owner have not been paid in full at the time the application if considered or in the case of a closing if they will not be paid before or at the time of closing. 9. The Association shall disapprove if the Owner falls to give proper notice to the Board of Directors of his Intention to lease, sell, or otherwise transfer his Lot and/or change the occupancy of their Lot. 10. The Association shall disapprove if the lessee(s) or intended occupants take possession of the Lot prior to approval by the Board of Directors as provided in the governing documents. 11. The Association shall disapprove if the owner is in violation of any provision of the governing documents of the Association at the time the application is considered. Page 10

You also agree that a fax or photocopy of this authorization with your signature be accepted with the same authority as the original.
