8 SECTION 7 - DECLARATION i) I/we wish to apply for a new sewer diversion and/or requisition as described for the premises referred to in section 2 of this form, under sections and/or 185 of the Water Industry Act ii) iii) I/we acknowledge my responsibilities and duties as set out in the Clients duties of the CDM Regulations 2015 and accept that these apply to me. It will not be possible to apply for the streetworks notice until the South West Water agreement is signed and full payment is received for each scheme. vii) I/We agree, that for the purposes of the Water Industry Act 1991 and the Data Protection Act 1998, the information provided in this form and in any supporting documents, may be held on a computer and processed by South West Water Ltd and its servants and agents for all purposes connected with the Company's statutory water and sewerage undertakings. viii) I/We understand that full payment for the scheme will be required within 15 working days of receiving an offer letter.
