Directory information This directory lists active Providence Health Plan providers located in Oregon and southwest Washington as of June We make every effort to ensure our directory is accurate, but changes do occur. We encourage you to visit the frequently updated online provider directory at or call customer service to verify a provider s participation status. Some features of the new directory include fewer clicks to get to your results; advanced search options; the ability to print directory results on demand; and enhanced provider information, such as hours, clinic websites and provider bios. The online directory is frequently updated and available at Personal health care provider A personal health care provider may be a general or family practitioner, internist, pediatrician or, in some cases, an obstetrician or gynecologist. Personal health care providers perform exams, provide preventive care and treat many routine concerns in their offices. We encourage you to choose one provider who can serve as the primary contact for your health care needs; however, you are not required to select a personal health care provider. If you are looking for a new personal health care provider, we recommend calling the provider s office to confirm whether he or she is accepting new patients. You also will need to request that your medical records be transferred from your previous provider s office. Nationwide network of participating providers In addition to the participating providers listed in this directory, we also have a nationwide network of health care professionals, hospitals and facilities that allow you to receive in-plan benefits while you are outside of the Providence Health Plan service area. 5 Provider Directory Specialist care Your personal health care provider may determine that you require a specialist s services. Your provider will discuss the decision with you and may recommend a participating specialist. While you may choose a participating specialist on your own, we recommend that you also notify your personal health care provider, who will share medical information with your specialist. You may see a specialist at any time without a referral. You receive the highest level (in-plan) of benefits when you use participating providers for covered health care services. You also benefit from having coordinated care meaning providers in our network bill us directly and work with us to arrange your care. Women s health care provider Women s health care providers include physicians specializing in obstetrics or gynecology, nurse practitioners, certified nurse midwives or physician assistants specializing in women s health care. Emergency care When possible, we ask you to call your personal health care provider before seeking emergency care, to ensure the quality of your care. If, however, you believe that taking the time to call your personal health care provider would threaten your life or cause serious damage to your health, go to the nearest hospital emergency room or call 911. Mental health and chemical dependency services If you are in need of emergency mental health or chemical dependency services, go directly to a hospital emergency room. Non-emergency inpatient, residential and day treatment mental health and chemical dependency services are covered benefits only when you receive authorization prior to services rendered. These services are provided for the same or less cost than services offered by participating providers.
