1 Facility Information for CareConnect Classrooms A few days prior to your scheduled date you will receive a CareConnect Training Enrollment Notification via . Please pay close attention to the location of your classroom and use the following guide to assist your navigation. It is important that you arrive to your class minutes ahead of the start time. Additional maps are available through the PeaceHealth website by selecting the Locations button on the main page and choosing the specific location you need. You may also want to reference this special page created just For New Caregivers which provides information on what to expect as you journey through your first few weeks at PeaceHealth. Note: Refreshments are not provided in CareConnect classes so please bring a snack and covered drink. You will want to dress in layers to accommodate for temperature fluctuations (both warm and cool). Click on the applicable location below to jump to that section in the document. 2 FOR OUR EUGENE/SPRINGFIELD/COTTAGE GROVE/FLORENCE CAREGIVERS Eugene, Springfield, Cottage Grove, and Florence facilities make up our Oregon Network. All Oregon Network caregivers and providers are expected to attend CareConnect training in Eugene or Springfield, depending on the class. There are three different campuses where your training may occur. Sacred Heart Medical Center University District (UD / UDMC) Support Services Building (SSB) Address: 1255 Hilyard St, Eugene, OR Parking: Park in the PeaceHealth Employee Parking at 13 th Avenue & Hilyard Street Walking Directions: From the main lobby of Sacred Heart University District, take the elevators on your right to the 3rd floor (elevators are approximately 20 feet inside the lobby on the right). Once on the 3rd floor, make a right out of the elevator, and another immediate right. Follow this hallway across the sky bridge, continuing to the right at the closed double doors. An overhead sign states Support Services Bldg. with a right arrow. Follow this hall to the end and turn left. CHI Workroom: Enter through the 1st door on the right (Center for HealthCare Improvement, CHI). The CHI Workroom is across from the reception desk, 1st door on left. Siskiyou Conference Room: Make another immediate left to the elevator. Take this elevator (or stairs) to the 4th floor. Make a left when exiting the elevator or stairs. Follow this hall to the end and turn right. Siskiyou Conference Room is the 2 nd door on the right. SSB PC Lab: After making a left at the end, go straight down the hallway until you stand in front of the Learning and Development suite. Do not enter into the suite, rather turn to your left for the SSB 3rd Floor PC lab (labeled Financial Analysts). Steens Conference Room: After turning left into the building, take an immediate left. Take the elevator (or stairs) up to the 4 th floor. Make a left out of the elevator and the Steens Conference Room will be the 3 rd door on your left. Page 2 of 19
3 Physician and Surgeon North (NE Bldg.) Address: 677 E. 12 th Ave. Eugene, OR Northeast Building PSN 190 PC Lab: From the Sacred Heart University District lobby at 1255 Hilyard Street, take the elevator or stairs to the 3rd floor. At the Espresso PRN coffee shop, take the sky bridge. (Follow the parking sign.) You have the correct sky bridge if it is crossing a street. Continue to the sign that says, Outpatient Rehab and make a right. Once you pass the Outpatient Rehab department on the right, continue to the elevators of the North Building. Take the elevator or stairs to the first floor. In the lobby of the North Building, go towards the open hallway, do not enter through any doors. Make a right at the sign N-190 (Conference Room PS 190). Walk down the hall to the last door on the left which is PSN 190. From the 3 rd floor patient parking lot at the Physician and Surgeon South building, walk away from the elevators toward the reception desk across from Oregon Imaging Center. Turn left at the next open hallway with the sign pointing toward Outpatient Rehab. Walk straight, past Outpatient Rehab, continuing to the elevators of the North Building. Take the elevator or stairs to the first floor. Out of the elevator, turn right, then right again, walking down the open hallway. Make a right at the sign N-190 (Conference Room PSN 190). Walk down the hall to the last door on the left which is PSN 190. Page 3 of 19

5 Sacred Heart Medical Center Riverbend (RBMC) Address: 3333 Riverbend Dr., Springfield, OR Parking: Park in PeaceHealth Employee parking areas. Walking Directions: When you enter the main lobby of RiverBend Hospital walk past the large stone fireplace toward the back-wall staircase. Take the left side staircase up to the second floor. (the main lobby of RiverBend hospital and the stair case located behind the fireplace) Once at the top of the stairs, continue straight down the hall toward the double doors (located near the woman s restroom). If closed, these doors require a badge scan. PC Lab 21A: Once through the double doors, 21A is the second door on the left. PC Lab 21B: Once through the double doors, 21B is the first door on the left. Page 5 of 19

7 Riverbend Annex (RBA) Address: 123 International Way, Springfield, OR Parking: If you work at RBA you will park in the PeaceHealth Employee assigned parking spaces. If you do not normally work at RBA, you may park in the spots marked Visitor outside of the main entrance doors. This is a badge access facility. Unless you already work at RBA please arrive at least 5 minutes before class starts and wait in the lobby for a training team member to guide you to the classroom. Classrooms in this facility and how to find them (if you already work at RBA): Training Ellipse: From the front lobby of RBA, go through the double doors to the right of the front desk. Turn right and go down the hallway until you see a Meditation Room sign on your right. Take a right at the sign. The Ellipse room is located across from the Meditation Room through the glass doors on the right.) 8 FOR VANCOUVER/LONGVIEW CAREGIVERS Vancouver and Longview facilities make up our Columbia Network. All Columbia Network caregivers and providers are expected to attend CareConnect training in Vancouver. There are three different buildings where your training may occur. PeaceHealth Southwest Medical Center (SWMC / PHSW) Medical Center Physicians Building Address: 505 NE 87 th Ave, Vancouver, WA (this is also known as the 505 Building, or Pabco Building) Parking: See the map on page 11 and park in any Employee Parking area. Walking Directions: Enter the 505 building from 5th St and take the elevators (straight ahead) or stairs (to the right of the elevator) down to the lower level. Computer Training Room 1 (LL-20): Once downstairs, turn left toward the wall and Computer Training Room 1 will be immediately through the glass door on your right. Computer Training Room 2 (LL-20): Once downstairs, Computer Training Room 2 will be straight across from the stairway door, across and slightly to the left of the elevator. Computer Training Room 3 (LL-20): Once downstairs, turn right. Computer Training Room 3 will be on your left. B38: Once downstairs, turn left toward the wall and follow the long hallway until you get to the Imaging Department (LL-50). Go through the Imaging waiting room and through the door next to the phone on the wall (you are actually entering the clinic). Take a left and follow this hall as it turns to the left, then right. Keep walking until the hallway comes to a T and turn left. B38 will be on your right. Page 8 of 19

10 Health Education Center Address: 600 NE 92 nd Ave, Vancouver, WA (This is also known as the HEC and is attached to the hospital, accessible via the outside entrance off of 92 nd Ave, or from inside the hospital). Parking: See the map on page 11 and park in any Employee Parking area. Walking Directions: From Employee Parking located off of 87 th Ave, you can get there by walking outside the building. As you pass the front of the 92nd Avenue entrance, continue and you will see the Health Education Center entrance (you can also enter the building through the 5th Street entrance and follow the signs to the Health Education Center). Mother Joseph Library (also known as the MOJO Library): Once inside the Health Education Center, take the staircase near the windows. 12 System Services Center (SSC) Address: 1115 SE 164 th Ave, Vancouver, WA Parking: If you work at the SSC you will park in the PeaceHealth Employee assigned parking spaces. If you do not normally work at SSC, you may park in the spots marked Visitor outside of the main entrance doors. This is a badge access facility. Please work with your manager to ensure that you have access necessary to arrive at your classroom. Columbia PC Lab: Enter SSC and take the stairs to the second floor and turn left at the reception desk. Go through the middle set of double doors and turn right once inside. The Columbia PC Lab will be on your left. Various other conference rooms: Ask your manager or the receptionist for instructions to other conference rooms. 13 FOR BELLINGHAM/SEDRO-WOOLLEY/FRIDAY HARBOR CAREGIVERS Bellingham, Sedro-Woolley, and Friday Harbor (Peace Island) facilities make up most of our Northwest Network. These caregivers and providers are expected to attend CareConnect training in Bellingham. There are three different buildings where your training may occur. St. Joseph Medical Center (STJO) St. Luke s Health Education Center (HEC) Address: 3333 Squalicum Pkwy, Bellingham, WA Parking: The is a large lot at the HEC building. You may park in any unmarked spots. Walking Directions: Note that this location is about a 5 minute walk from the main hospital campus. Enter the building from the main entrance under the covered portico. You ll enter facing the doors to Conference room. A. HEC Conference Room D: Turn right and the entrance to Conference Room D is directly ahead at the bottom of the stairs HEC Conference Room H: Turn right and the entrance to Conference Room H is on the left (across from the men s room) Photo of St. Luke s Health Education Center (HEC) Page 13 of 19
14 Marble Building Address: 2980 Squalicum Pkwy, Bellingham WA Parking: There is parking in the back of the Marble Building. You may park in any unmarked spots, or in the employee parking lots at the main hospital across the street. Marble Training Room: Enter building through South entrance (which are the doors that face Squallicum Parkway). The room is the first door on your right. Photo of the Marble Building Page 14 of 19

16 Cordata Main Address: 4545 Cordata Pkwy, Bellingham, WA Parking: There is a large lot at the Cordata building. We suggest you park toward the back of the building. You may park in any unmarked spot. Walking Directions: Enter building from the South. You will be on the first floor, directly behind the elevators. Turning either direction will take you around to the center of the building. Cordata Conference Room 4 (Computer Lab): Take the elevator to LL. Exit the elevator and head to the right. You ll be facing conference room 2 (double doors). To the left is a single door with a hallway to its left. Head down that hallway, take your first right. 17 FOR KETCHIKAN AND CRAIG CAREGIVERS Ketchikan and Craig facilities make up our Southeast Alaska Region. These caregivers and providers are expected to attend CareConnect training in Ketchikan or virtually. There are two different classrooms where your training may occur. Ketchikan Medical Center (KEMC) Address: 3100 Tongass Ave, Ketchikan, AK Parking: Keene Conference Room: Enter the main entrance of the hospital at the address listed above. Turn right down the long hallway. Once you arrive at the stairwell, turn left. Go to the end of the hall and there will be an elevator on the righthand side. Take the elevator to the 3 rd floor. Once at the 3 rd floor, turn right. Follow that hall to the end where you will find the Keene Conference Room. KMOB Training Room: o From the hospital entrance, turn left, go past the lab and radiology waiting area to the elevators on the right. Take the elevator down to the ground. Out of the elevators, take a right past the cafeteria to the double doors. There is a wall mounted button on the righthand side that will open the double doors. Go through the double doors and take a right to a second set of double doors. There is a wall mounted button on the lefthand side to open the double doors. After exiting the second set of doors, you will be in a parking garage. Follow the blue line through the parking garage to the elevator in the middle of the garage. Take that elevator to the first floor. Take a left out of the elevator and towards the registration desk of the orthopedic clinic. Using your badge access key, go through the door on the far left. The KMOB Training Room is at the end of the hall on the left. (If you do not have badge access to the doors, one of the Front Desk Employees can let you in the door.) o From the street, take the elevator to the first floor.
